3.A case of Psoriasis with Generalized Pustules Treated with Tetracycline.

Four clinical patterns of generalized pustular phase were apparent and have been named the Zumbusch, annular, localized and exanthematic types. Other reported 10 types of pustular psoriasis including the poriasis with generalized pustules. We report a case of psoriasis with generalized pustules in a 18-year-old women, who has numerous scattered pustules with mild fever and leukocytosis. She was treated sucessfully with tetracycline 2.0 gm a day for 4 wks.

4.A Case of Premature Ovarian Failure Assosiated with Gonadal Dysgenesis.

Prenature ovarian failure is a condition causing amenarrhea, hypoestrogenism, and elevated genadotropins in women younger than 40 years. A karyotype should be performed as part of basic laboratory evaluation for all patients with premature ovarian failure and prodromal premature ovarian failure. Development of a malignancy in a dysgenetic gonad is of major concern. The presence of a fragment of the Y chromosome is thought to be a key to the oncogenic potential of these gonads. The search for the testicular determining factor(TDF) has engendered much confusion about which part of the Y chromosome plays a role in malignancy. This was initially postulated to be the H- Y antigen. More recent data, however, localize the area near the centromere of the Y Chromosome, on the long arm(Yq). Malignant potential is clearly not linked to the testicular determining factor itself(SRY). This is a critical point in clinical medicine. Feilure to display SRY or a closely related sequence does not rule out the presence of the segment of the Y chromosome postulated to be associated with the development of malignancies. We have experienced a case of premature ovarian failure with chtomosomal abnormality involving Y chromosome fragment. So we report this case with a brief review of literatures.

5.A Case of HAIR - AN Syndrome.

Polycystic ovary disease is a heterogenous endocrinopathy with many interacting causal factors. One potential such factor is chronic hyperinsulinemia. multiple, independent lines of evidence suppart the contention that chronic hyperinsulinemia causes ovarian hyperandragenism. This evidence includes: (1) mutations in the insulin receptor gene that cause severe hyperinsulinemia appear to be associated with ovarian hyperandrogenism, (2) insulin stimulates ovarian thecal and sttomal androgen seaetion in vitro, and (3) in some experimental models, manipulation of circulating insulin concentrations results in changes in circulating androgens. Although the association between hyperinsulinemia and hyperandrogenism remains to be fully explained at the molecular level, chronic hyperinsulinemia appears to be an important cause of hyperandrogenism. We have experienced a case of HAIR AN syndrome showing hyperandrogenism, insulin resistance and acanthosis nigricans in infertile patient. So we report this case with a brief review of literatures.

6.Cytologic Findings of Breast Carcinoma in Fine Needle Aspiration: Comparison with Histologic Findings, Stage and Lymph Node Metastasis.

In order compare cytologic findings of breast carcinoma in fine needle aspiration cytology (FANC) with histologic findings and prognostic factors including histologic grading, lymph node metastasis and stage, 79 cases of infiltrating ductal carcinoma diagnosed by FANC and confirmed by histology were analysed. We especially attempted to correlate nuclear grade, cellularity and smear pattern with histologic grade, type, status of lymph node metastasis and stage. The results are as follwos : 2. Individual cell pattern was more frequently identified in high histologic grade and scirrhous or solid-tubular type than in low histolgic grade and papillotubular type. 3. Cellularity increased with higher histologic grade and lymph node metastsis. However cellularity was low in scirrhous type. 4. There is no relationship between nuclear grade and histologic type, between smear pattern and lymph node metastasis or stage, and between cellularity and stage. These results suggest that cytologic findings of breast carcinoma such as nuclear grade, cellularity and smear pattern are indicative of histologic findings in relation to histolgic grade and type. Especially, nuclear grade of FANC may yield valuable prognostic information.

7.Comparison of Diagnostic Accuracy of Three- Dimensional MR Cholangiopancreatography and ERCP in Various Extrahepatic Biliary Lesions.

PURPOSE: To evaluate the diagnostic role of a three-dimensional MR cholangiopancreatography(MRCP) over endoscopic retrograde cholangio- pancreatography(ERCP) in various extrah-epatic biliary disease. MATERIALS AND METHODS: MRCP and ERCP were performed in 45 consecutive patients with suspected extrahepatic biliary diseases. MRCP was obtained using a reverse fast imaging with a steady-state free precession (reverse FISP : PSIF) sequences, and then images were reconstructed by standard MIP algorithm. The predictability of biliary dilatation and level of obstruction of MRCP was evaluated using ERCP as a gold standard. The accuracy distinguishing malignant from benign lesions, and overall diagnostic accuracy were compared between MRCP and ERCP. RESULTS: The sensitivity, specificity and accuracy of MRCP in predicting biliary dilatation were 94.6%, 75.0% and 91.1%, respectively. The level of obstruction was accurate in 87.0% with MRCP. The sensitivity, specificity and accuracy MRCP and ERCP in distinguishing malignant from benign lesions were 76.2%, 87.5% and 82.2% and 71.4%, 83.3% and 77.8%, respectively. The overall diagnostic accuracy was 60.0% with MRCP and 55.6% with ERCP. CONCLUSION: 3D MRCP shows a good diagnostic value compared to that of ERCP, and can replace a ERCP.

8.Job Stressors in Subway Workers and Firemen.

OBJECTIVES: To determine the job stress factors of subway workers and firemen and to develope appropriate stress management systems for the works places. METHODS: The study subjects comprised 1,826 male subway workers and 538 male firemen residing in Daegu metropolitan city. The Korean Occupational Stress Scale (KOSS) questionnaire was used. RESULTS: The median values of the study subjects were lower than the reference values. The categories of physical environment and insufficient job control were more stressful to firemen while the organization system category was stressful to both subway workers and firemen (p<0.05). There was no statistical difference in the stress factor of the interpersonal conflict category. Among the firemen, the physical environment category was more stressful to field work department workers than to desk workers (p<0.05). CONCLUSION: The study results suggested that interpersonal conflict and organization system categories are stressful to male subway workers and firemen. Especially, effective control of firemen's physical environment stress factor is needed.

9.A Case of Kallmann Syndrome Inherited in Autosomal Dominant Mode.

OBJECTIVE: To report the pedigree of Kallmann syndrome inherited in autosomal dominant mode with variable expressivity. MATERIAL AND METHOD: Case report. RESULTS: The patient had amenorrhea and anosmia but did not have a sign of absolute hypogonadotropic hypogonadism. Her father had an anosmia and her two elderly sisters also had an anosmia but delivered babies uneventfully. Her two male siblings did not show any signs of hypogonadotropic hypogonadism. CONCLUSION: Kallmann syndrome has many different modes of inheritance such as autosomal dominant, autosomal recessive, and X-linked form. So the careful investigation of family pedigree is required.

10.Adherence Of Oral Bacteria On Chitosan: Added Denture Base Materials In Vitro.

The purposes of this study were to evaluate the adherence of bacteria on various denture base resin materials and effects of chitosan, added to denture base materials on bacterial adherence. PMMA denture base resin such as heat-cured Vertex-RS, self-cured Vertex-SC and 4-META denture base resin such as heat-cured Meta-Dent, self-cured Meta-Fast were used in this study. Samples were divided into two groups: the denture base resin with chitosan, without chitosan. Streptococcus mutans and Lactobacillus casei were used in this study. The surface of samples was observed by SEM. When chitosan was added to M17 and MRS broth, viable cell count of bacteria was reduced. Viable cell count of Streptococcus mutans on the samples decreased as follows: Meta-Dent, Vertex-SC, Meta-Fast, Vertex-RS. Viable cell count of Lactobacillus casei on the samples decreased as follows: Vertex-RS, Meta-Dent, Meta-Fast, Vertex-SC. The resin with chitosan showed lower adherence of bacteria than without chitosan. The images of SEM showed that the surface of the resin with chitosan was rougher than that of without chitosan. These results showed that the denture base resin materials with chitosan have rougher surface than without chitosan,but less bacteria adhered on them.
