1.Factors Influencing Quality of Life in People with Mental Disabilities using Mental Health Centers.

PURPOSE: The study was done to identify factors influencing the quality of life of people with mental disabilities who use mental health centers. METHODS: Data were collected by questionnaires from 288 people with mental disabilities in Busan and Kyungnam Provience. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, multiple linear regression with the SPSS/WIN 14.0 program. RESULTS: The mean score for quality of life was 11.3+/-1.70, and the mean empowerment score was 59.7+/-8.44. The significant factors influencing quality of life were gender, age, residential status, number of support people, side-effects of drugs, service type, and empowerment. These factors explained 43.9% of the variance. CONCLUSION: The results suggest that in development and practice of program to increase the quality of life of people with mental disabilities a variety of factors should be considered.

2.Optical Opacity by Interaction of Silicone Material and Silicone Oil in Vitro.

Although the results of foldable intraocular lens are generally excellent, there are several complications. After cataract surgery with silicone lens, the patients with vitreoretinal surgery using silicone oil have not only visual acuity decrease, but also visual abberations such as halos and rainbow patterns. To report a newly defined complication, we studied the silicone lens-silicone oil interaction. The nonimplanted ilicone, acryl and PMMA lens, ten cases each, had been incubated in silicon oil at 36C for 9 months. After removal from the oil, each lens was examined and photograp hed by scanning electron microscopy every week. Appearance of lens surface and degree of adherence of the silicone oil to each IOL`s optic were estimated. Oily residue on the PMMA and acryl lens was removed easily and shown clear surface. In case of silicone lens, presence of distinct film or coating of oil was shown on the surface of both the optic and haptic component of IOL`s. They could not be removed with mechanical pressure with injected balanced saline solution and the oily residue remained strongly adherent to the optic surface. After 9 weeks, gelly-like adherence was appe ared on the silicone lens surface. As a result, the use of silicone IOL`s in patients with current vitreoretinal disease or those who are at high risk for future vitreoretinal disease that may require silicone oil as part of the therapy should be reconsidered.

3.The Effects of Video Programs of Cardiopulmonary Cerebral Resuscitation Education.

PURPOSE: The aim of this study was to identify the effect of video programs of cardiopulmonary cerebral resuscitation (CPCR) education of cardiopulmonary cerebral resuscitation of nurses. METHODS: The subjects of the study were 64 nurses working in a university hospital. Nurse's CPCR performance have been measured four times (pre-test, post-test at immediately, 3 months and 6 months after intervention). Data were collected from February to August 2013. RESULTS: There were significant differences in knowledge, attitude, self-efficacy, and performance between groups by measure time. And there were significant interactions in knowledge, self-efficacy, and performance between groups, within groups, except for the attitude. The video programs of CPCR interventions appear to be effective in the improvement of knowledge, self-efficacy, and performance, as compared to the control group. CONCLUSION: The video programs of CPCR education was an effective intervention to improve and retain the level of knowledge, attitude, self-efficacy and performance. And the video program of CPCR education have an advantage of self-learning effect for nurses with shift work. Therefore video programs of CPCR education will be utilized for continuing nurse's education.

5.Risk Factors and Clinical Outcomes for Vancomycin-Resistant Enterococcus Colonization on Intensive Care Unit Admission.

PURPOSE: The purpose of this study was to identify vancomycin-resistant enterococcus (VRE) colonization rate in patients admitted to the intensive care unit (ICU), associated risk factors and clinical outcomes for VRE colonization. METHODS: Of the 7,703 patients admitted to the ICUs between January, 2008 and December, 2010, medical records of 554 VRE colonized and 503 uncolonized patients were reviewed retrospectively. To analyzed the impact of colonization on patients' clinical outcomes, 199 VRE colonized patients were matched with 199 uncolonized patients using a propensity score matching method. RESULTS: During the study period, 567 (7.2%) of the 7,703 patients were colonized with VRE. Multivariate analysis identified the following independent risk factors for VRE colonization: use of antibiotics (odds ratio [OR]=3.33), having bedsores (OR=2.92), having invasive devices (OR=2.29), methicillin-resistant Staphylococcus aureus co-colonization (OR=1.84), and previous hospitalization (OR=1.74). VRE colonized patients were more likely to have infectious diseases than uncolonized patients. VRE colonization was associated with prolonged hospitalization and higher mortality. CONCLUSION: Strict infection control program including preemptive isolation for high-risk group may be helpful. Further research needs to be done to investigate the effects of active surveillance program on the incidence of colonization or infection with VRE in the ICU.

6.Disappearing Atypical Lentiginous Pigmentation of the Desmoplastic Malignant Melanoma.

A 38-year-old man presented with a black pigmented patch overlying his upper lip, a part of the gingiva and the hard palate. An intradermal tumor mass was palpated at the central portion of the pigmented patch of the upper lip. Diphenylcyclopropenone (DPCP) is a potent contact sensitizer that could stimulate non-specific immunity. We report a case of desmoplastic malignant melanoma (DMM) in which an atypical lentiginous pigmentation disappeared with topical DPCP immunotherapy and the dermal tumor mass was removed by a surgical wedge-shaped excision. Though an atypical lentiginous pigmentation of DMM sometimes undergoes extensive spontaneous regression, in our case it could be attributed to the therapeutic effect of topical DPCP immunotherapy rather than a spontaneous regression.

7.Overcoming Internalized Stigma in Persons with Psychiatric Disabilities.

PURPOSE: This phenomenological study was done to identify comprehensively and in depth the experience of overcoming internalized stigma in persons with psychiatric disabilities. METHODS: Data were collected from December 2015 to June 2016 with 10 persons who had psychiatric disabilities. The participants took part in interviews on their experiences of overcoming stigma. The collected data were analyzed using Colaizzi method. RESULTS: From the raw data obtained from the study participants, 12 themes and 5 theme clusters were identified. The 5 theme clusters were found to be ‘being locked up in one's own stigma’, ‘trying to escape from the self in constraints’, ‘overcoming oneself by oneself’, ‘getting to see the true self’, and ‘living the life desired’. CONCLUSION: Study findings indicate the necessity of developing a mediation program to reduce and overcome the internalized stigma in the persons with psychiatric disabilities. A national and social effort is necessary for persons with psychiatric disabilities to help them overcome the internalized stigma.

8.A Case of Trichilemmal Horn.

Trichilemmal horn(trichilemmal keratosis) is a rare keratinizing tumor that resembles a cutaneous horn. Histologically, it is characterized by an abrupt maturation of keratinocytes into lamellar keratin without the formation of a granular layer. We describe a case of a trichilemmal horn on the right cheek of a 78-year-old woman.

9.Dacryocystorhinostomy with Intraoperative Mitomycin C.

Nowadays endscopic transnasal dacryocystorhinostomy(DCR) has been done popularly in patients who had epiphora chronically because of lacrimal obstruction. The most frequent cause of DCR failure is obstruction of osteotomy site due to formation of granuloma. We used the mitomycin C to suppress proliferation of fibroblast to increase success rate. Between July 1996 and september 1997, 15 patients(16 eyes) who had visited ophthalmology department in Seoul Advantist Hospital made went endoscopic transnasal DCR and packed merocel socked with 0.2mg/ml between 3 months to 18 months(mean 11.5 months). One case showed recurrent epiphora 2 months after surgery, and success rate was 94%. In conclusion, endoscopic transnasal DCR with Mitomycin C should be considered to suppress proliferation of granuloma and increase success rate.

10.PALATELESS COMPLETE DENTURE FOR RESTORING GOOD TASTES : A CASE REPORT.

The Purpose of this article is to present a clinical case report for palateless complete denture. Despite the contravacy of palatal uncoverage in upper complete denture, palateless complete denture has some merits for upper edentulous patient. Following the uncovering of the palatal portion, the patient became easy to talk and restored the lost good tastes. He is happy despite of the decrease of the retention of the upper complete denture. Palateless complete denture is a compatible alternative for upper edentulous patients in cases of gagging, large palatal torus and restoring the lost good tastes. The clinical points are as follows : 1. The remaning alveolar ridge should be ovoid and have enough width and height for the support and retention. 2. The patient must have strong wish to the palateless complete denture. 3. Palatal beading made on the palatal peripheral border give good border sealing of the palatal flange and minimaized the prominence of the denture flange. 4. The peripheral border of the palatal flange should be reduced as thin as possible for more comfort. 5. Upper artificial posterior teeth should be arranged over the alveolar ridge crest and inner incline of the buccal cusp relieved for denture stability while chewing. 6. For stability of palateless complete denture, bilateral balanced occlusion should be sttained. Palateless complete denture will restore the lost good tastes and more comfortable and physiologic to upper edentulous patients and a good alternative to full palatal coverage complete denture in the properly selected cases.
