3.Bone Tunnel Enlargement after Endoscopic ACL Reconstruction by Autogenous Bone - Patellar Tendon - Bone Graft.

Radiographic increase in the size of tibial and femoral tunnels has been observed following the reconstruction of the ACL with a bone-patellar tendon-bone autograft. The purpose of this study is to determine if any differences exist in the amount of enlargement of the bone tunnel with the clinical results and to know the factors which affected to the enlargement of the bone tunnels. Total 27 patients were retrospectively reviewed for tunnel enlargement radiographically at one year after operation. Anteroposterior and lateral x-ray were obtained and the tunnel were measured by two independent observers. The measurements were made at the widest part of the tunnel. The distance between tibial interference screw and knee joint line also measured. Correction for magnification was performed by comparing the measured width of the interference screw used for fixation of the graft with its actual width. Statistical analysis was performed with Wilcoxon rank sum test. The radiographic tunnel enlargement was an average of 1.7+/-1.3mm for the femur and 1.9+/-0.8mm for the tibia. The proximal migration of the tibial interference screw was an average of 2.3++/-1.1 mm. There was no statistically significant correlation between the changes in tunnel diameter and either the modified Hughston knee score, Lysholm knee score, or the joint laxity measured by a KT-2000 arthrometer, Lachman test. There were no correlations between the mild proximal migration of the tibial interference screw and the clinical results. Conclusively, the tunnel enlargement and mild proximal migration of the interference screw did not appear to affect the functional outcome adversely. It needs longer follow up for the evaluation of etiology and natural history of this tunnel enlargement.

Isolated situs in versus of the stomach with otherwise normal position of the thoracic and abdominal vescera isan extremely rare anomaly occurring in two distinct forms. Majority of cases are associated with eventration of the diaphragm and are reported as being confused with spontaneous pneumothorax of pyopneumothorax at base of the right lung. The right sided stomach may produce interesting and confusing changes in liver scan. We have experienced 2 cases of the isolated dextrogastria.

8.A Study on Real-Time Arrhythmia Detection System(II): Part II. Signal Process Algorithm.

We designed software system to detect intermittent arrhythmia from the patients automatically. To overcome the difficulties on real-time processing, we used four variavles; heart rate, QS width, morpholigy and axis. Templete generation routine was designed to produce patient's individual normal ECG pattern and store in the computer. The most important point of automatic detection is to discriminate the Q and J point. It is done by linear phase differentiation, the point that five point gradient is within a minimum value of 6Hz curve gradient is defined as Q and J points. The morphology calculation is simplified by simple addition and substraction, making real-time processing by modified cross correlation method. We find that both value of morphology and QS width will be used as a parameter of datermining the abnormalities of ECG signal automatically.

9.A STUDY ON THE FACTORS AFFECTING TO TOOTH WEAR.

This study was performed to investigate the factors related to tooth wear. For this study, 78 patients with temporomandibular disorders and 76 dental students without any signs and symptoms of temporomandibular disorders were selected as the patients group and as the normal group, respectively. Preferred chewing side, Angle's classification, lateral guidance pattern, head and shoulder posture wee observed clinically. Electromyographic activity of anterior temporalis and masseter muscle were recorded with BioEMG and occlusal status were recorded with T-Scan . Wear facet area of each tooth was measured from working model of upper arch corresponding to the occlusal status from T-Scan, Wear facet area were measured with planimeter in mm2. Total area were divided into incisal, canine, posterior tooth area. Anterior wearfacet area was incisor area plus canine area, and unilateral area was anterior area plus posterior area. The data collected were analyzed by SAS statistical program and the results of this study were as follows: 1. There was no significant difference between the two groups in total werafacet area, and male subjects showed tendency to have larger area in the normal group but female subjects showed tendency vice versa. 2. There was no significant difference related to preferred chewing side and Angle's classification, however, some difference was observed by lateral guidance pattern. Anterior wear facet area in subjects of canine guidance was the largest in the three subgroups. 3. Subjects with head tilting to right side had larger posterior and total area, and subjects with higher shoulder in right side had larger canine and anterior area than any other subgrous. 4. Electromyographic activity of masseter muscle was more correlated with wear facet area than anterior temporalis muscle, and tooth contact number and force were significantly correlated with wear facet area, but the most important factor affecting tooth attrition was age.
