1.A Case of Weber-Cockayne Epidermolysis Bullosa Simplex - Ultrastructural Findings.

Epidermolysis bullosa simplex (EBS) is a group of autosomal dominantly inherited genetic disorders characterized by blistering due to mechanical- stress-induced degeneration of basal epiderrnal cells. Recently, it was discovered that EBS is induced by keratin 5 and 14 gene mutations. Weber Cockayne (W-C) EBS is the mildest type, with blistering concentrates primarily on palar and plantar regions, and basal cell cytolysis by keratin filament perturbations is present. Herein we report a case of W-C EBS with its ultrastructural findings. Electron microscopy showed cytolysis and separation of the basal epidermal cells, mainly at the subnuclear cytoplasm. The cyto- plasm of basal cells showed edema, loosening and intact rnitochondria. Besides the cytoplasmic changes, the nucleus also showed lytic degeneration. Characteristically, dense condensation of tonofilarnent was observed, which suggests that W-C EBS is. also a disorder of keratin.

2.Antidepressant and the Quality of Life of Depressive Patient.

This study investigated the antidepressant efficacy and it's impact on the quality of life of depressed patients. We performed Hamilton Depression Rating Scale(HDRS), and Montgomery-Asberg Depression Rating Scale(MADRS), and Health-related Quality of Life Questionnaire(HQLQ) to both tricyclic antidepressant(TCA) and sertraline groups. There were 16 subjects in the study. The tricyclic group had 9 subjects and the sertraline group had 7. The TCA and sertraline produced a similar degree of response. Both groups experienced a reduction of 70% or more in mean HDRS and MADRS total score after 6wks. In HQLQ, the TCAs group also showed improved bed disability days, alertness behavior, and social interaction, the sertraline group showed improved health perception, alertness behavior, home management, and social interaction. We suggested that the improvement of "Quality of life" were not in proportion to the clinical symptom's improvement. Therefore, clinicians should consider the benefit of antidepressant treatment in terms of quality of life.

3.Pharmacogenomics of Depressive Disorders.

The phamacotherapy of depression has reduced morbidity and improved outcome for many depressive patients. A wide range of classical and new antidepressants are available for their treatment. However, 30-40% of all patients do not respond sufficiently to the initial treatment and present adverse effects. Pharmacogenetics studies the genetic basis of an individual's ability to respond to pharmacotherapy. Recently, some reports on serotonin transporter gene polymorphisms and their influence on the response to antidepressive therapy provide an interesting diagnostic tool in assessing the chances of response to antidepressants. We also investigated the relationship between serotonin transprter polymorphisms(5-HTTLPR) and the long-term effect of the antidepressant treatment. 128 depressive patients were enrolled into 2nd year study. The therapeutic response of each subset was not different at 8th, 16th week. but the subset with homozygote(l/l) of long variant showed a better therapeutic response to antidepressant than the heterozygote(l/s) of long and short variant, which showed a better therapeutic response than the subset with homozygote(s/s) of short variant at 1st year and 2nd year after the antidepressant treatment. This result shows that the serotonin transporter polymorphisms may be related to the long-term effect of antidepressant treatment. The potential for pharmacogenomics. the use of genetic increasing attention pharmacogenomics will contribute to individualize drug choice by using genotype to predict positive clinical outcomes, adverse reactions, and levels of drug metabolism. Personalized medicine the use of marker-assisted diagnosis and targeted therapies derived from an individual molecular profile, will impact the antidepressant therapy and this approach will replace the traditional trial-and-error practice of medicine.

4.A clinical and pathological study of Becker's nevus.

BACKGROUND: Beckers nevus is a benign lesion that may be congental or acquired, macular or papular, hairless or hypertrichotie. OBJECTIVE: Our pcrpose was to describe the clinical findings of Eeckers nevus and to establish a method for the histologic diagnosis of Beckers nevus. MEHTODS: We reviewed PF skin biopsies and their clinical finding from 28 patients with Beckers nevus during a 10 year period from January 1981 to January 1991. Results : The results were summarized as follows. A Clinical characteristics 1. Sex distribution showed of male predominence with male female a ratio of 1.5:1. Age of onset was predominent in less that 5 years old and puberty. 2. The lesions were located on the trunk(24 cases), head & neck(8 cases), extremities(6 cases), and were repsilateral in 26 cases, bilateral in 2 cases. 3. The lesions were accompanied by hair in 17 cases(61%). The shaies of the lesions were reticulated (75 %), patch(18%), mottled(7%). 4. Hairs were distributed as follows : abdomen(100%), scapula(100 %), extremities (83%), cheek(80 %), neck(67 %), chest(40%). All lesions of the face had follicular papules. 5. Associated skin diseases were : smooth muscle hamartoma(3 cases), striae distensa(2 cases), steroid acne(2 cases), pigmented nevus(2 cases), and verruca plana(1 case). B. Histopathologic characteristics 1. The lesions were accompanied with plugs in 21 cases, which were follicular, acrosyringeal, epidermal. 2. Elongation of the rete ridge and hyperpigmented basal cells were observed in the epidermis in all cases. 3. The shape of the rete ridge in Beckers nevus has its characteristic appearance, which is single, bridging and reticulated. Single, bridging and reticulated shapes cexisted in 17 cases(61%). 4. Inflammatiry cell infiltrations were present in all of the upper drmis, in which melanophages(21 %), hair follicles(89%) and smooth muscle hamartoma(3cases) w ere observed CONCLUSION: These findings indicated that the shape of the rete ridge in Beckers nevus has its characteristic appesrance and can be useful for the diagnosis of Beckers nevus.

5.Surgical Correction of Foot Drop in Leprosy

Foot drop from paralysis of the muscles of the anterior and lateral compartments of the leg is common in leprosy. Surgical correction of foot drop in leprosy appears to have been neglected until Brand pointed out that anterior transposition of the tibialis posterior tendon to the intermediate cuneiform bone gave encouraging results. Many methods of correcting the functional problems associated with a foot drop have been tried with the object of providing a foot that is stable and socially acceptable. Fifteen cases of foot drop admitted to the Hospital of Leprosy Mission in Taegu, Korea during past 3 years from 1965 to 1968 were reviewed.

6.A Clinical Analysis on Malgaigne Fracture (15 Cases)

Malgaigne fracture is relatively rare and has many difficult problems for treatment. The author reviewed 15 cases of Malgaigne fracture treated at the orthopedic department of Kyungpook National University Hospital during the period from January 1963 to July 1969, and analyzed as to the cause of injury by mechanical forces, type of fracture, average duration for treatment, complication and end results. The clinical results were obtained as follows: 1. The causes of injury were mostly traffic accident (66.6%), cool miner accident (20%) and falling and stumbling(13.4%). 2. Males are roughly 5 times more frequent than females and almostly 4th to 5th decade of life. 3. Vertical shear type is the most common type in this series and had not been experienced the type of total pelvic disruption. 4. The most common complication is bladder and urethral injury and experienced a case of multiple long bone fracture. 5. All cases were treated by skeletal traction except 2 cases which were treated by skin traction. 6 Time of traction is 8 to 16 weeks (average 12 weeks) and observation was done 10 of 15 cases for 2 to 25 months (average 15 months).

9.A Case of Eosinophilic Pustular Folliculitis (Ofuji's Disease) in a Newborn.

We describe a case of eosinophilic pustular folliculitis in a 1-month-old Korean male infant. The patient was suffering from erythematous papules and pustules affecting the scalp, trunk, and extremities, detected at birth. The flare was accompanied by leukocytosis and eosinophila. Histopathology revealed folliculitis with a predominant eosinophilic infiltrate. Lesions showed partial response to topical steroid and disappeared in four weeks. Three months after the resolution of the lesions, he presented a similar clinical picture as the previous episode.

10.Clinicopathologic Study of Pustular Drug Eruption.

BACKGROUND: Pustular eruptions due to drugs are uncommonly reported. We studied the characteristics of clinical and histopathologic findings of pustular drug eruption. OBJECTIVE: We observed th.e causative agents, clinical featurs and histopathologic findings of pustular drug eruption and identified differential points of generlized pustular eruption. METHODS: We evaluated t,he clinical and histopathologic findings of 8 patients with pustular drug eruption and reviewed the literatures reported cases of pustular drug eruption. RESULTS: All patients diagnosed pustular drug eruption suffered from generalized pustular eruption associated with systemic symptoms such as fever, headachened myagia one to three days after treatment with causative agents. The causative agents of putular drug eruption are antibiotics such as ceftriaxone, analgesics and antipyretics. The pustule resolved after a few days of treatment with systemic corticosteroids and antihistamines. Laboratory findings revealed leukocytosis, neutrophilia, and analevated erythrocyte sedimentation rate, On histopatologic findings, we observed subcorneal pustuls containing neutrophils, eosinophils and some lymphocytes and spongiosis, exocytosis of acute iiflammatory cells. Perivascular infiltration of lymphocyte ancl edema of papillary dermis was also bserved in the dermis. CONCLUSION: Pustular drug eruption is characterized by generalized pustular eruption associated with systemic symptoms and histopathologic findings of that are sterile subcorneal pustules. Therefore differential diagnosis of other generalized pustular erupticns are relatively easy by careful history of medication, clinical and histopathologic findings.
