1.A Case of Acute Diffuse and Total Alopecia of the Female Scalp.

Diffuse alopecia areata is the least common clinical type of alopecia, and this diffuse form lacks the characteristic hairless patches of alopecia and it begins as diffuse hair loss. Diffuse alopecia areata has been poorly characterized. In 2002, there was a suggestion to define this form of alopecia areata as acute, diffuse and total alopecia of the female scalp (ADTAFS). ADTAFS is characterized by a marked female predominance, tissue eosinophilia and a uniquely short clinical course. We report here on one case of ADTAFS.

2.Spontaneous Spinal Epidural Hematoma: Case Report.

The case of 17-year-old boy is presented, in whom back pain progressed to paraplegia. A spinal epidural hematoma was removed at surgery with complete recovery. No traumatic or febrile episode was obtainable. The importance of early diagnosis and early spinal decompression is stressed.

3.Anterior Callosal Disconnection Syndrome.

It has been known that right ACA occlusions can cause callosal disconnection syndrome. A 61-year old right-handed man was admitted because of a left ACA occlusion. MRI showed infarction of the medial frontal cortex and the anterior two-thirds of the corpus callosum. He presented with weakness and gait initiation failure in the right leg with grasp reflex, suspicious alien hand sign, and tactile anomia in the right hand. He was diagnosed with transcortical motor aphasia. He was unable to successfully complete written tasks in response to dictations and writing down spontaneous answers. He wrote down incorrect words and demonstrated paragraphism with his left hand. He could copy simple items but not written words and complex items with his left hand. Finally, he had difficulties in writing answers in response to complex verbal and written commands with his left hand, but preserved the ability to simple verbal commands, somato-sensory, and visually guided tasks. We attribute these results to the anterior callosal disconnection of the right sensorimotor cortex from the left language area.

6.Identification of Autoantibodies to Melanocytes and Characterization of Vitiligo Antigen in Vitiligo Patients.

BACKGROUND: Recently, it has been suggested that autoantibodis face of melanocytes are prevent in the sera of vitiligo patients. However, these autoantibodies exist, whether they are specific for vitiligo a vitiligo patients possess them. In addition, the specificity of the iti lecular weight of the antigen are all unsolved areas demanding further. OBJECTIVE: To investigate the possible role of autoimmune microvitiligo, this study was designed to verify the presence of auto and vitiligo antigen from the surface of melanocytes, the specificity of gene specific antigens on the sunever, it is not known whether ents, and what percentage of goantigen and the exact moier research. anisms in the development of bodies in vitiligo patients, the utoantibodies and vitiligo anti. METHODS: Indirect immuvofluorescent microscopy, flow cytoriiety, and ELISA was done to compare the reactions between melanocytes and sera. SDS-PAC island immunoblotting were used for the identification of vitiligo antigen. RESULTS: Vitiligo sera showed more prominent fluorescence and higher optical density on the surface of melanocytes than normal sera. Forty-four percent of vitiligo sera was directed to melanocytic surface antigen with a molecular weight of 65kDa. The sition assay using rabbit antimelanocytic antibody showed an inhibition of the reaction betw er vitiligo sera and melanocytes in ELISA and immunoblotting. CONCLUSION: A surface antigen of 65kd was identified from melanocytes and 44.4% of the vitiligo sera showed positive reactions to this antigen.

7.A Case of Multiple Nevus Sebaceus.

The nevus sebaceus is an organoid nevus generally noted on the scalp or the face as a single lesion. The frequent association of nevus sebaceus with other appendage tumors and with apocrine glands suggests that nevus sebaceus is derived from the primary epithelial germ. We report a 20-day-old boy with multiple nevus sebaceus on his face, scalp and external ear. The lesions were symmetrical and midline involved yellowish plaques. On the microscopic view of biopsy specimen from the face and scalp, there is a dilated keratin-filled infundibula showing a few buds of undifferentiated cells representing malformed hair germs. The dermis contains many fibroblasts and immature hair structures. Numerous sebaceous glands and eccrine glands lie in upper dermis. Diagnosis was confirmed by clinical appearance and histological picture.

8.Focal Increased Tc-99m MDP Uptake in the Nutrient Foramen of the Femoral Diaphysis on Bone SPECT/CT

We present images of an 83-year-old female with a history of osteoporosis and bilateral total knee replacement arthroplasty, referred for bone scintigraphy and single-photon emission computed tomography (SPECT)/computed tomography (CT), owing to left knee pain. No trauma to, or intense exercise of, the kneewas reported. The bone scan and SPECT/CT revealed a focally increased Tc-99m methylene diphosphonate (MDP) uptake in the medial cortex of the left femoral diaphysis with matched linear radiolucency on CT images. This was misinterpreted as atypical femoral stress fracture; however, focal stress reaction injury to the nutrient foramen was confirmed on contrast-enhanced magnetic resonance imaging.
