1.COMPARATIVE ANALYSIS BETWEEN LEVATOR RESECTION AND METHODS USING FRONTALIS ACTION 2-4mm OF LEVATOR FUNCTION IN BLEPHAROPTOSIS PATIENTS.

The choice of operative procedures was determined by many factors, but the levator function is considered as the most important factor. Fox recommended that when the levator function was 2 to 9 mm, the patient should be treated by the levator resection and when the levator function was below 2 mm, the patient should be crated by frontalis suspension. But Collin recommended that when the levator function was below 4 mm, the brow suspension procedure got the better result. Beside that, many other authors recommended various opinions. Therefore, when levator function of the patients was between 2 and 4 mm, the choice of operative procedure was much confusing us. The aim of this study is to clarify which is better Procedures between levator resection and the frontalis transfer or orbicularis oculi muscle transfer among the patients with 2 to 4 mm of levator action. From Jan. of 1991 to Dec. of 1994, among 26 patients with 2 to 4 mm levator function, 13 cases of 10 patients were operated by levator resection, 21 cases of 16 patients were teated by frontalis transfer or orbicularis oculi muscle flap. The results were evaluated with the average 27 months of follow-up and we compared the result of the levator resection procedure with that of frontalis muscle transfer or orbicularis oculi muscle transfer. The preoperative average amount of ptosis is about 2.7 mm in cases with levator resection, about 4.0 mm in cases with frontalis muscle transfer or orbicularis oculi muscle flap transfer. The postoperative average amount of ptosis is about 1.7 mm in cases with levator resection about 2.14 mm in cases with frontail muscle transfer or orbicularis oculi muscle flap transfer and so the postoperative improvement of amount of ptosis is about 1.0 mm in cases with levator resection, about 1.86 mm in cases with frontails muscle transfer or orbicularis oculi muscle flap transfer. The major complication of levator resection method is undercorrection. However, the eyelid deformity due to excessive upward traction was more frequent in frontalis muscle flap technique.

3.Radiologic assessment of Endoscopically reconstructed ACL using Bone - patellar tendon - bone.

Endoscopic ACL reconstruction using bone-patellar tendon-bone has been considered the gold standard in the field of reconstructive ACL surgery. Technically, graft must be placed at isometric point. But it is difficult to evaluate the placement of graft postoperatively. The purpose of this study is to determine the radiological ideal position of graft by comparing postoperative results with the graft placement. Seventy cases of endoscopic ACL reconstruction were reviewed and classified according to the femoral and tibial graft position on radiologic imaging. The femoral graft position was classified in relation to angle of graft on anterior position view and distance from posterior margin of graft to the inner surface of posterior cortex on lateral view. The tibial graft position was classified in relation to intercondylar eminence on anterior posterior view and lateral view. Knee score (modified Marshall, Lysholum), manual anterior instability test (Lachman test, Pivot shift test) and arthrometer measurement were checked to evaluate postoperative results in each case. The results of this study implicate that knee joint in which femoral graft was oriented at direction of 11 o clock centring around 68 degree respect to tibial joint and placed within 3mm from posterior cortex showed higher knee score and lesser laxity. In cases of tibial side, the graft oriented to intercondylar eminence (AP view) and placed anterior to intercondylar eminence (lateral view) showed higher knee score and lesser laxity.

5.Comparison of the Clinical Results of the Fixation Techniques to Femur in ACL Reconstruction using Hamstring Double-Loops: Bioscrews vs. Semi-Fix .

PURPOSE: In order to know the clinical usefulness of the Bioscrew(Linvatec, USA) and the Semi-Fix (Arthrex, USA) technique for femoral fixation in ACL reconstruction using hamstring double loops, we scrutinized and compared the 24 knees. Materials and Methods: We compared Lysholm score, Hospital for Special Surgery(HSS) knee rating scores, Lachman tests, Pivot-Shift tests and KT side to side difference in 24 knees after arthroscopic ACL reconstruction using hamstring double loops from August 1996 to January 1998. We have used fourteen bio-absorbable interference screws and ten Semi-fixs for femoral tunnel fixation. RESULTS: We took the data; the Lysholm score of 96.1 and 94.1; HSS score of 98.6 and 98.1; KT differ- ence of 1.3 +- 2.13(mm) and 1.2 +- 1.89(mm) after ACL reconstruction using Bioscrew or Semi-Fix respec- tively. Femoral tunnel enlargement showed 2.51 +- 1.03(mm) and 1.95 +- 2.33(mm) respectively. And we got the similar results of Pivot-shift test, Lachman test and anterior drawer test after reconstructive surgery. CONCLUSION: Since we got the similar good data in comparative clinical study between the Bioscrews and the Semi-Fix technique, we anticipatd that either the use of Semi-fix technique or the biodegradable interference screws would be bright in the fixation to femoral tunnel in ACL reconstruction using ham- string double loops.

6.The Clinical Study of Discoid Meniscus: Types of Tear and Its Treament

The methods of treatment of discoid meniscus have been in controversy and there are so many reports which represent different results. We analyzed the results of the 91 knees with discoid menisci of 84 patients who had been treated with arthroscopic partial or total meniscectomy, open partial or total meniscectomy, semi-arthroscopic total menisceetomy form June 1982 to Dec. 1987. The puropse of this paper is to present the indication and prognosis of the each method of treatment. There were 45 cases of male, 46 cases of female, and 10 patients were affected both knee. 2 patients who had served arthroscopic partial meniscectomy were reoperated by arthroscopic total meniscectomy due to recurrence of symptom. We classified the types of tear into 7 classes and observed the clinical characteristics and results. The methods we applied were arthroscopic total meniscectomy 40 cases, semi-arthroscopic total meniscectomy 25 cases, arthroscipic partial meniscectomy 16 cases, open partial meniscectomy 3 cases, open total meniscectomy 2 cases, and conservative treatment after arthroscopic diagnosis 5 cases. The follow up cases were 56, and period of follow up was ranged form 11 months to 5 years and 4 months (av. 2 years and 8 months). The results were followings : l. 71.4% of the patients were placed between 11 years and 30 years old, and there was no sexual difference. 2. 11.9% of the patients were affected bilaterally. 3. The commonest clinical symptom and physical sign was clik (67%) and quadriceps muscle atrophy (48.4%) 4. Diagnostic accuracy of knee arthrogram was 88%, but it could not demonstrate the type of tear. In arthrographic finding, the older the patient, the thinner the meniscus was. 5. The commonest type of tear was type V(34.1%) and it was followed by type II (19.8%). 6. There was no difference between the long term results of arthroscopic and semi-arthroscopic total meniscectomy. 7. The results of total meniscectomy is better than partial meniscectomy.8. The method of treatment should be chosen according to the type of terar, so partial meniscectorny is recommended for type II, IV and total meniscictomy is recommended for type III, IV, V and VI. 9. Less hospitalization, earlier recovery, and better result were obtained by the arthroscopic or semi-arthroscopic total meniscectomy.

7.Arthroscopic Surgery for the Acute Anterior Cruciate Ligament Injury

The treatment of acutely torn anterior cruciate ligament(A.C.L) remains a controversial subject. The purpose of this paper is to show the arthroscopic technique of A.C.L. primary repair with or without augmentation with semitendinosus tendon. Twenty-one patients with acute A.C.L. tear were treated with repair or augmented repair under arthroscopic control from April 1989 to August 1993. Five A.C.L. torn at femoral attachment with intact distal stump were treated with arthoscopic repair alone & 16 A.C.L. torn at substance near the femoral attachment with arthroscopic repair & augmentation with semitendinosus tendon. The patient population included of 16 males and 5 females with an average age 28.3 years. Fifteen patients injuried during sports activity. Postoperative results were evaluated by Lysholm knee score & Hospital for Special Surgery knee scale in average 26 months (range from 12 months to 64 months). All patients were rated as good or excellent result except one patient who had fair result. At follow-up study, testing with KT1000 arthrometer revealed 17 patients had less than 3mm of side to side difference. Second look arthroscopy was performed in 15 patients from 4 to 29 months after repair or augmented repair. Eleven out of these 15 A.C.L. were well healed with good tension 8r. revascularization of graft. Remaining 4 A.C.L. were healed with scar tissue. On the based of this study, arthroscopic repair or augmented repair is very effective treatment for acutely torn A.C.L. near the femoral attachment.

8.Meniscorrhapht in Discoid Meniscal Tear

We reviewed 24 patients who had performed arthroscopic meniscal repair for the treatment off discoid lateral meniscal tear between january 1989 and December 1992. The follow up ranged from 1 year 6 month to 4 year 6 month(average 32month). The mean age was 17.9(range,8-50)years, the patients under 20 years old were 17(cases,71%).Common preoperative clinical symptom was pain on the knee joint(21 cases,87.5%) and click(19 cases,79%).Positive McMurry test was the most common physical examination finding(19 cases, 79%). All the lateral discoid meniscal tears were confirmed by arthroscopy. The mean interval between symptom onset and operation was 18.7 months(range 1 month to 10 year). The types of tear were peripheral (14cases, 58%), complex tear(4cases), longitudinal tear(2cases), transverse tear(2cases), horizontal tear(2cases)and the most common location of tear was posterior horn(1 1cases, 45%). The mean number of suture was four. In operative method, Scases(33%) were performed only meniscorrhaphy and meniscorrhaphy combined with partial meniscectomy was done in 16 cases(67%). According to Ikeuchi's grading system, the result was satisfactory in 21cases, fair in three cases. Second look arthroscopy was done in eleven cases(46%) between post operative 4 months to 2 years(mean, 8.3 months). The arthroscopic examination revealed complete healing of repaired meniscus in eight cases(73%). The remained three cases which were not completely healed was fair result.

9.Three cases of meconium peritonitis.

Meconium peritonitis is an aseptic peritonitis caused by spill of meconium in the abdominal cavity through one or several intestinal perforations which have taken place during intrauterine life or early neonatal life. We experienced three cases of meconium peritonitis with ileal perforation in two cases 1 day-old male neonate and 2 day-old male neonate, respectively, which had the chief complaint of vomiting and abdominal distension. Literatures are reviewed, briefly.

10.Rupture of Mitral Papillary Muscle Resulting from Blunt Chest Trauma: A Case Report.

We experienced a case of mitral incompetence due to rupture of anterolateral papillary muscle in a 56-year-old male who complained of abdominal pain and mild dyspnea after being struck by car. Clinical manifestation immediately following injury was minimal, but heart failure progressed rapidly 3 days after injury. Echocardiopraphic evaluation revealed ruptured anterolateral papillary muscle and severe mitral regurgitation on color flow imaging. There was no evidence of coronary artery disease on coronary angiography. Operation revealed that the haed of anterolateral papillary muscle was torn out of its insertion. Mitral valve replacement with mechanical prosthesis was performed on the 50th day after injury.
