1.The effect of the bioresorbable collagen membrane on the regeneration of bone defect by using the mixture of autograft and xenograft bone.

STATEMENT OF PROBLEM: In cases where bony defects were present, guided bone regenerations have been performed to aid the placement of implants. Nowadays, the accepted concept is to isolate bone from soft tissue by using barrier membranes to allow room for generation of new bone. Nonresorbable membranes have been used extensively since the 1980's. However, this material has exhibited major shortcomings. To overcome these faults, efforts were made to develop resorbable membranes. Guided bone regenerations utilizing resorbable membranes were tried by a number of clinicians. Bio-Gide. is such a bioresorbable collagen that is easy to use and has shown fine clinical results. PURPOSE: The aim of this study was to evaluate the histological results of guided bone regenerations performed using resorbable collagen membrane(Bio-Gide.) with autogenous bone, bovine drived xenograft and combination of the two. Surface morphology and chemical composition was analyzed to understand the physical and chemical characteristics of bioresorbable collagen membrane and their effects on guided bone regeneration. MATERIAL AND METHODS: Bioresorbable collagen membrane (Bio-Gide.), Xenograft Bone(Bio-Oss), Two healthy, adult mongrel dogs were used. RESULTS: 1. Bioresorbable collagen membrane is pure collagen containing large amounts of Glysine, Alanine, Proline and Hydroxyproline. 2. Bioresorbable collagen membrane is a membrane with collagen fibers arranged more loosely and porously compared to the inner surface of canine mucosa : This allows for easier attachment by bone-forming cells. Blood can seep into these spaces between fibers and form clots that help stabilize the membrane. The result is improved healing. 3. Bioresorbable collagen membrane has a bilayered structure : The side to come in contact with soft tissue is smooth and compact. This prevents soft tissue penetration into bony defects. As the side in contact with bone is rough and porous, it serves as a stabilizing structure for bone regeneration by allowing attachment of bone-forming cells. 4. Regardless of whether a membrane had been used or not, the group with autogenous bone and Bio-Oss. filling showed the greatest amount of bone fill inside a hole, followed by the group with autogenous bone filling, the group with blood and the group with Bio-Oss. Filling in order. 5. When a membrane was inserted, regardless of the type of bone substitute used, a lesser amount of resorption occurred compared to when a membrane was not inserted. 6. The border between bone substitute and surrounding bone was the most indistinct with the group with autogenous bone filling, followed by the group with autogenous bone and Bio-Oss. filling, the group with blood, and the group with Bio-Oss. filling. 7. Three months after surgery, Bio-Gide. and Bio-Oss. were distinguishable. CONCLUSION: The best results were obtained with the group with autogenous bone and Bio-Oss. filling used in conjunction with a membrane.

2.Potentiating Effects of Bay K 8644 on the Relaxation Induced by Ultraviolet or Visible Light in Porcine Coronary Artery.

BACKGROUND: This study was aimed at defining the varying responses of porcine coronary artery(PCA) to various wavelengths of ultraviolet irradiation, and at relating them to the changes in cyclic GMP contents. METHODS: The ring preparations of PCA with intact or removed endothelium were irradiated with the ultraviolet or visible light of wavelengths(240-520mm) from xenon lamp of a spectrofluorometer, and the changes in vascular tension were recorder on polygraph. For cyclic GMP assay, rat thoracic aorta was frozen after irradiation and homogenated. The supernatant was extracted with water-saturated ether and the cyclic GMP contents were measured with radioimmunoassay. RESULTS: Ultraviolet irradiation relaxed the preparations(UVR-relaxation) in resting state and those precontracted by prostaglandin F2alpha, the maximal relaxation occurring at 410nm, and the magnitude depending on the duration of irradiation. The UVR-relaxation was not affected by removing the endothelium, while it was markedly potentiated by pretreatment with Bay K 8644. The Bay K 8644-induced potentiation of UVR-relaxation was abolished by hemoglobin and slightly reduced by wrapping the rings with aluminum foil. Cyclic GMP contents in the increase was markedly potentiated by pretreatment with Bay K 8644. CONCLUSION: The observations suggest that UVR-relaxation in procine coronary artery is caused by activating the nitric oxide-cyclic GMP system, which is most sensitively activated by UVR of 410nm and that its potentiation induced by Bay K 8644 may be related nitrous substance released from the agent upon UVR.

3.Clinicopathologic Features and HBsAg and HBeAg Expressions in Hepatitis B Virus-associated Glomerulopathy.

Morphometry of nuclei of the benign and malignant prostatic lesions was performed to study the relationship between nuclear size and shape and the prognosis of prostatic adenocarcinoma. Fifty one cases of prostatic adenocarcinoma and 13 cases of benign prostatic hyperplasia were included to evaluate area, perimeter, Dmax, Dmin, and 5 form factors of the nuclei by image analyzer (Zeiss Ibas 2000) using hematoxylin-eosin stained slides. All analytic factors of nuclear size and shape were significantly different between benign lesions and adenocarcinomas. Increased nuclear size was associated with nu- clear irregularity, presence of metastasis, advanced clinical stage, and high Gleason's grade and score of prostatic adenocarcinoma. On Kaplan-Meier method, survival was decreased with older age, no hormonal treatment, stage D, high Gleason's grade and stage as well as with larger size and irregular shape of the nuclei. In conclusion, morphometry of nuclei of the prostate can be a helpful tool to differentiate between benign and malignant lesions. Nuclear morphology is thought to be associated with prognosis of prostatic adenocarcinoma.

6.Clinical Analysis of Supracondylar Fracture of The Humerus in Children

130 cases of the suprarcondylar fractures of the humerus in children were analysed. 60 cases were immobilized with plaster splint without closed reduction. 22 cases were closedly reduced and immobilized with plaster splint. 47 cases were closedly reduced and were fixed percutaneously with K-wires. 39 cases were operatively reduced and were fixed with K-wires. The following results were obtained. 1. The average age was 7.9 years, and the male and female ratio was 2.7: 1. 2. The extension type was 96.2% and the flexion type was 3.8%. 3. The left side (64.6%) was more frequently involved. 4. As associated complications, nerve injury (4.6%), vascular injury (2.3%), open fracture (2.3%), and associated fracture (2.3%). were developed. During treatment incomplete ulnar nerve palsy (2.3%), pin tract superficial infection(3.1%), severe cubitus varus deformity(4.6%), severe limitation of elbow motion(11.6%) were also developed. 5. Final results in the Holmberg's 4 Groups according to Mitchell & Adams' criteria showed that the more the displacement of fracture is increased, the more the poor result is also increased. 6. In spite of the selection of the more severe cases for surgery, the results of the operative cases were better than those achieved by closed reduction in the easier cases. 7. In the displaced fractures, immediate open reduction brings the satisfactory results and minimizies the sequellae.

7.Interfascicular Nerve Grafting in the Treatment of the Peripheral Nerve Injury

The authors performed the interfascicular nerve grafting in the gap of injured nerves using the microsurgical technique to obtain the accurate coaptation of the fascicles. The authors experienced the 33 cases(26 patients) of the interfascicular nerve grafting in the median(11), ulnar(9), radial(2), common peroneal(3) and digital(8) nerves at the Department of Orthopaedic Surgery, College of Medicine, Kyung Hee University from Jan. 1980 to June 1984. The patients were followed up from 12 to 26 months (average 20 1/2 months). The following results were obtained; 1. The shorter the length of the donor nerve and the younger the age of the patients are, the better the recovery of the nerve function is. 2. The medial and lateral cutaneous nerves of forearm as donor nerve are suitable as well as the sural and superficial radial nerves. 3. The end result of each patient was evaluated by Seddon's classification. The excellent motor recovery was 72.0% (18/25) and the excellent sensory recovery was 81.8% (27/33). 4. The interfascicular nerve grafting is an excellent method in treating the widely separated,severed peripheral nerve injury.

8.Surgical Treatment in Congenital Ulnar Drift of Fingers

The Congenital Ulnar Drift of the fingers is a relatively uncommon and has symmetrical malformations of hand, face and foot characterized by MP joint flexion contracture and ulnarward deviation of the fingers and microstomia, In tarsal deformity, there are clubfoot and congenital vertical talus with rocker bottom deformity. In treatment, the report of the surgical treatment was rare. We experienced congenital ulnar drift of fingers in 2 patients in one family, which were treated by dome resection corrective osteotomy of the metacarpals. Therefore remarkable symptomatic improvement was obtained.

9.Intraspinal Synovial Cyst: A Case Report

A 25 year old patient who had low back pain and sciatica was diagnosed as having a lumbar intraspinal extradural synovial cyst adjacent to a facet joint between the third and fourth lumbar vertebrae. The extradural defects defined with positive contrast myelography and postmyelographic computed tomographic scanning were posterolateral to the dural sac. Biopsy revealed the findings compatible with synovial cyst. Treatment consisted of laminectomy and surgical excision of the cyst.

10.Giant Cell Tumor Involving the Sixth Cervical Spine: One Case Report

While a giant cell tumor most frequently involves the distal end of the femur and proximal end of the tibia, involvement of the cervical spine is quite rare. Recently we observed a patient with a giant cell tumor affecting the sixth cervical vertebra. This is regarded as a clinically malignant tumor because its nature and difficulty in diagnosis and treatment We easily confirmed the extent of the lesion by CT scanning with Metrizamide. The treatment was palliative tumor ressection. This case of a giant cell tumor affecting the sixth cervical vertebra is reported with a brief review of the literature.
