Journal of the Korean Society of Coloproctology 1998;14(2):291-298
Eleven patients underwent posterior sagittal anorectoplasty(PSARP) as a secondary procedure. Two of them had rectovaginal fistula and another two had rectocutaneous fistula. Six of the rest complained of frequent fecal soiling and the last one had severe anorectal stricture after perineal anoplasty. Five patients had lived with colostomy until the second operations were carried out. The ages at the time of the secondary PSARP were between 7 months and 29 years. Distal colostogram and MRI were taken to evaluate distal colon, position of the rectum and voluntary muscle. All patients had normal sacrum except one who had anorectal stricture. Seven patients, six with fecal incontinence and the other one with rectovaginal fistula had mislocated anorectums. Three patients, two with rectocutaneous fistula, the other one with anorectal stricture, had abdominal approach to obtain enough length of colon for pull-through procedure. With the posterior midsagittal approach, we could manage all the problems, rectovaginal fistulas, rectocutaneous fistulas, strictures and malpositioned rectums, without difficulty. No patients had serious complications except wound infection in one. All patients were satisfied with the results after redo-PSARPs even though normal continence has been achieved in only one patient. Seven patients who had continuous soiling or rectocutaneous fistula, needed no more diapers even though four of them showed fecal staining under stressful condition and the other three showed intermittent fecal leaking less than once a day. The rest three of the patients maintained their continence with support of drugs and/or enemas because of constipation. The PSARP is a popular procedure as a primary operation; however, our results suggested that this procedure also gave us a good opportunity for management of serious complication developed after primary anoplasties.

Segmental wiring to treat the unstable fracture and fracture-dislocation of the thoraco-lumbar spine is more effective method than Harrington instrumentation as primary procedure which afforded immediate rigid internal fixation of the spine and permitted mobilization without external support and immediate rehabilitation. A clinical study was made of twenty seven patients who were hospitalized and treated at the Department of Orthopaedic Surgery, Wonju Christian Hospital from Feb. 1979 to Apr. 1983. The following results were obtained: l. Of twenty seven patients, nine(33.3%) had a flexion rotation, eight(29.6%) had a shearing, eight (29. 6%) had a flexion compression and two(7.5%) had a vertical compression by Holdsworths mechanism of injury and by Pauls classification of fracture, thirteen(48. 1%) had a flexion distraction fracture, twelve(44. 4%) had a translation fracture and two(7. 5%,) had a unstable burst fracture. 2. Fifteen(55. 6%) had a complete neural deficit and five(18. 5%,) had an incomplete neural deficit. Six(22.2%) of the fifteen patients with complete neural deficit and one(3. 7%.) of the five patients with incomplete neural deficit showed slight neurological recovery, but eight(29.6%) gained complete recovery. 3. Correction of kyphotic deformity was average 12. 4 degree in Harrington instrumentation and average 18. 0 degree in segmental wiring, and during the follow-up periods, there was a final loss of 6. 3 degrees of kyphotic correction in Harrington instrumentation and 3. 0 degrees in segmental wiring. 4. In external support, fourteen(51.9%) had body jacket cast and four(14.8%) had back brace in Harrington instrumentation, but all patients except one brace had no external support in segmental wiring. 5. In complication, two hook dislocations and two pseudoarthroses were occurred in Harrington instrumentation, but any complication except only one cases of wound infection was not occurred in segmental wiring.

BACKGROUND: Hypertension is a major and growing public health problem and it calls for life-long management. The antihypertensive agents are still playing major roles in the management of hypertension. We studied the effect of the long-term therapy with a new calcium channel blocker, felodipine in patients with essential hypertension. METHODS: We recruited 31 patients with mild to moderate essential hypertention and gave them 5mg (1 tablet) of felodipine(Munobal(R)) once daily for 12 months. We measured the blood pressure and the heart rate every month and tried to detect any untoward symptoms and signs during the whole observation period. Baseline work-up for the hypertensive patients were done as usual before and after the study. RESULTS: The average systolic and diastolic blood pressures were 162.7+/-9.7/99.6+/-7.4mmHg respectively before the administration and they came down to the level of 139.7+/-15.0/87.3+/-8.6mmHg respectively at the end of the 1st month (p<0.05). This trend was maintained during the whole observation period, so they were 141.2+/-11.0/85.6+/-8.2mmHg respectively at the end of the study (p<0.05). Untoward drug reactions such as gastrointestinal troubles, facial edema, leg edema, chest pain and headache were present in 11 patients (35%) but they were not serious. There was no case in which the drug was withdrawn. There was one case in which the level of the serum triglyceride increased after the administration of the drug. CONCLUSION: Felodipine has good and persistent hypotensive effect and it is safe and easy to use.

The Journal of the Korean Orthopaedic Association 1988;23(2):448-454
In the standing lateral position, the pelvis is tilted in some degree anteroinferiorly. But there was no accurate and standard measurement about the pelvis tilting. The pelvis tilting affects on the lumbar lordosis, weight bearing area of the hip joint, and the anteversion of acetabulum. In the standing true lateral view of the pelvis, 5 points were marked: midpoint of both anterosuperior iliac spines (M), upper margin of symphysis pubis (S), lower margin of symphysis pubis(S), sacral promontorium (SP), coccyx tip (C). And using the additional horizontal plane (H), we measured the 4 angles : angle I ( < MSH), angle II ( < SPSH), angle III(
