2.Tc-99m HMPAO White Blood Cell Scintigraphy of an Enterovesical Fistula Complicating Crohn's Disease.

Computed tomography (CT) seems to be the best imaging modality to diagnose an enterovesical fistula, but is not always able to demonstrate enterovesical fistula itself. In this case report, we present Tc-99m HMPAO white blood cell (WBC) scintigraphic findings of an enterovesical fistula complicating Crohn's disease. A 22 year-old male presented with a one-month history of urinary symptoms such as dysuria, hematuria, and frequency. The patient had intermittent right lower quadrant pain, diarrhea and hematochezia. Enterovesical fistula was highly suggestive in pelvic CT which showed air density in the urinary bladder, but cystoscopy failed to find an opening of the fistula. Tc-99m HMPAO WBC scintigraphy for evaluation of inflammatory bowel disease incidentally demonstrated enterovesical fistular tract. Crohn's disease was later confirmed by histologic examination of the surgical specimen. In our patient, Tc-99m HMPAO WBC imaging was helpful in determining the location of the fistula as well as assessing the disease activity and extent of the Crohn's disease.

3.MR evaluation of visceroatrial situs abnormality.

Thirteen patients with visceroatrial situs abnormalities were evaluated by magnetic resonance(MR) imaging. Eleven patients were confirmed surgically. Two patitnts were diagnosed by MRI and cardiac catheterization. Right isomerism was found in seven patients, left isomerism in two, and situs inversus in four. For the determination of situs, we evaluated the morphology of atrial appendages and main bronchi, the relationship between abdominal aorta and inferior vena cava(IVC), and the status of upper abdominal viscera. The bilateral atrial morphology was differentiated in 8 of 12 patients. The bronchial situs was determined in 11 of 12 patients. Juxtaposition of abdominal aorta and IVC was found in 6 of 7 with right isomerism. IVC interruption with azygos continuation was found in all two with left isomerism. Incidentally three cases of short pancreas were found. MR imaging showed all structures relevant for the assessment of situs, thus obvrating the need for performing additional diagnostic procedures. MR imaging, therefore, is a valuable tool in the clinical management of patients who are suspected of having a situs abnormality.

5.Antiatherogenic Effect of Naringin Independent of Lipid-Lowering Action in Hypercholesterolemic Rabbits.

BACKGROUND: Naringin, one of the flavonoids in citrus fruit peels, is known to have antioxidant and hepatotonic effects in animal studies. We evaluated the effect of naringin on 1) blood lipid profiles, 2) regression of fatty streak of aorta, and 3) liver toxicity in diet-induced hypercholesterolemic rabbits. METHODS: New Zealand White Rabbits (2.0 - 2.5 Kg) were divided to three groups; group without treatment, group treated with 100 mg/kg/d or 500 mg/kg/d naringin, and group treated with 1 mg/kg/d or 20 mg/kg/d lovastatin. They were fed on 0.25% or 1.0% cholesterol-containing diet for 8 weeks and then sacrificed. Blood samples were collected for measurement of total cholesterol, HDL-cholesterol, triglyceride, serum GOT and GPT. Aortas and livers were harvested for evaluation of fatty streak and pathologic examination. RESULTS: 1)Feeding of 1% cholesterol diet for eight weeks significantly increased the cholesterol level upto 20 folds. Neither lovastatin nor naringin did lower these marked hypercholesterolemia. But both naringin (500 mg/kg/d) and lovastatin (1 mg/kg/d) significantly reduced the area of fatty streak by 75% and 58%, respectively. Naringin was more effective in inhibition of fat infiltration into liver than lovastatin which showed hepatotoxicity as increase of serum GPT level (p=0.01). 2)Feeding of 0.25% cholesterol diet for eight weeks significantly increased the cholesterol level upto 17 folds. Total cholesterol and triglyceride levels tended to decrease by treatment with naringin (500 mg/kg/d) and lovastatin (20 mg/kg/d), but this decreases were not statistically significant. However, areas of fatty streak significantly decreased by treatment with naringin and lovastatin by 64 and 82%, respectively (p<0.05). Microscopic analysis revealed that foam cell infiltration into intima was significantly reduced by naringin and lovastatin. In contrast to lovastatin, naringin significantly reduced the level of serum GPT (p<0.05). CONCLUSION: Like lovastatin, naringin has strong antiatherogenic action which may not be associated with its very mild lipid lowering action. In contrast to lovastatin, naringin does have hepatoprotective effect.

8.Comparison of Anterior Segment Measurements between Scheimpflug-Placido Camera and New Swept-source Optical Coherence Tomography

Purpose:
We compared anterior segment measurements obtained using a Scheimpflug-Placido topographer (SIRIUS®, Costruzione Strumenti Oftalmici, Florence, Italy) and a new anterior module for a swept-source optical coherence tomography system (ANTERION®, Heidelberg Engineering Inc., Heidelberg, Germany).
Methods:
Anterior segment measurements were evaluated in 74 eyes of 101 patients with the two devices. Central corneal thickness (CCT), anterior chamber depth (ACD), corneal refractive power (K), J0 (Jackson cross-cylinder with axes at 180° and 90°), J45 vector (Jackson cross-cylinder with axes at 45° and 135°), and white-to-white corneal diameter (WTW) measurements obtained using the SIRIUS® system and the new anterior segment module of ANTERION® were compared.
Results:
The mean CCTs measured by SIRIUS® and ANTERION® were 558.69 ± 44.65 and 540.26 ± 36.57 μm, respectively. The difference was statistically significant (p < 0.005), and there were high correlations between the two methods (r = 0.915, p < 0.001). The mean ACD measurements were 3.34 ± 0.53 and 3.34 ± 0.51 mm (p = 0.856), respectively, for SIRIUS® and ANTERION®. The measurements were strongly correlated (r = 0.966, p < 0.001). The measurements of WTW, total mean K, and J0 obtained with the two systems differed significantly (p < 0.005, p = 0.017, and p < 0.005, respectively), with high correlations between the two systems (r = 0.875, r = 0.967, and r = 0.933, respectively; all p < 0.001).
Conclusions
There were significant differences in measurements of K, CCT, WTW, and J0 between the two devices. Agreement analysis suggests that SIRIUS® and ANTERION® should not be used interchangeably.

10.The Anterior Chamber Depth According to Age in Normal Urban Adult.

The anterior chamber depth is important in diagnosis and treatment of glaucoma, thus the research of the anterior chamber depth would help the understanding of angle closure glaucoma. Then the estimation of anterior chamber depth might help the diagnosis of certain cases of glaucoma. It has been reported by other authors in Korea, but there were not enough subjects to see the correlation between age and the anterior chamber depth. The purpose of this study was to measure the variation of the anterior chamber depth in relation to the normal aging process. The anterior chamber depth was measured in 2,524 eyes of normal subjects aged 20 years or older who visited our hospital to receive a health examination. Measurements of the anterior chamber depth were carried out conviniently with the Haag-Streit pachometer. Intraocular pressures were measured with the Pulsair non-contact tonometer, and the refractive errors of each subject were divided into hyperopia, emmetropia, and myopia. These data were analyzed statistically by Student's t-test. Female subjects had a smaller measurement for anterior chamber depth than male. The average anterior chamber depth of the emmetropes was smaller than that of the my opes and larger than that of the hyperopes. It was found that the anterior chamber depth decreased with age(0.015mm/year). The anterior chamber depth seemed not to be correlated with the intraocular pressure.
