1.An Experimental Study of the Effects of Cementing Stage and the Presence of Synovial Fluid and Physiologic Saline at the Interface upon the Tensile Bonding Strength of Acrylic Bone Cement

An experimental study of the tensile bonding strength of acrylic bone cement was carried out by using the tubular bones of 12 mongolian dogs. Following observations were made: 1. The later the cementing stage, the weaker was the tensile bonding strength of bone cement. 2. When saline or synovial fluid was interposed between the contact surfaces of the recipient bone and the cement, the tensile bonding strength of bone cement decreased by more than 50% of that when the bonding surface was dry. 3. When the bonded bone was stored in Hartmans solution, the tensile bonding strength of bone cement was reduced to 15–25% of the strength when stored in air.

2.Nonspecific Bronchial Reactivity Determined by Tidal Breathing Method and Chest Auscultation: A Comparison with Dosimeter Method-.

As a preliminary step to determine if we can perform methacholine challenge test in young children, We investigated the feasibility of modified technique of methacholine challenge test in which tidal breathing and chest auscultation were used instead of dosimeter and measurement of lung function in older children. The results are as follows: 1) Values of PC20 measured by the tidal breathing method correlated significantly with those of PC20 measured by dosimeter 2) Ratio of values (PC20) obtained wth both methods was not related to age. 3) The minimal concentration of methacholine which caused wheezing on chest ausculation (PCW) correlated closely with PC20, but was 44% greater on average. 4) Expiratory wheezing is associated with greater fall of FEVI from baseline than inspiratory wheezing. Thus wheezing on chest auscultation can be used to detect bronchial obstruction at the end point for bronchial provocation testing in older children during the quiet tidal inhalation of methacholine. This may be applied to measurement of airway reactivity in young children who are unable to perform routine pulmonary function tests.

3.A Study on the Photoprotective Effect of alpha - Tocopherol and brta - Carotene in Guinea Pig.

The mechanism responsible for the formation of sunburn cells in mammalian skin is unknown. However it is suggested that the reactive free radicals and oxygen species generated by UV radiation are causing oxidative reactions in certain keratinocytes that are manifestated in the form of dyskeratotic cells and free radical scavengers such as a-tocopherol acetate or p-carotene were thought to be photoprotective against sunburn radiation. In the present study, we evaluated the effect of single and multiple applications of n-tocopherol acetate(a-TCA) and p-carotene on the formation of sunburn cells. We also determined the effect of these two antioxidants on the sunburn reartion by UVB irradiation. The results were as follows : 1. Both a-tocopheroJ acetate and p-carotene were photoprotective and prevented the formation of sunburn cells and sunburn reaction. 2. Topical p-carotene was less photoprotective than u-tocopherol acetate. 3. Topical use of a-tocopherol acetate and p-carotene in dose range exceeding 1,000 pgcm could provide photoprotective effect if the UVB exposure doses were less than 300 mJ/cm'( 3 MED)

4.Alopecia Neoplastica due to Breast Cancer.

We report a case of alopecia neoplastica-a rare form of cutaneous metastasis-in a 43-year-old Korean woman, who presented with multiple red, bald patches of 1 month's duration on her scalp. About 2 years previously, she received a modified left radical mastectomy and adjuvant chemotherapy for breast cancer. Histopathological findings of the bald lesions revealed diffuse infiltration of hyperchmmatic tumor cells arranged singly and in strands and cords in the upper and lower dermis. We started chemotherapy with adriamycin, but she died 13 months later.

6.THR with Hydroxyapatite - coated Hip Prosthesis.

Synthetic hydroxyapatite is a safe, nontoxic, biocompatible, and osteoconductive material. Hydroxyapatite-coated implants have the property of achieving a very strong bond with living bone in a relatively short period. From March 1992 to June 1994, 140 hydroxyapatite-coated primary total hip prostheses were implanted. This study included 60 cases that could follow up minimum 3 years. At the time of mean follow up, 45 months after operation, the mean Harris hip score was 92 points. Only two patients complained of thigh pain at last follow up. On radiographic evaluation, radiolucent lines were seen around the uncoated distal part of the stem in 46 percents of the implants. No radiolucent line was observed around the coated area of the prosthesis. Cancellous condensation was seen in 95 per cents of the implants at the Gruen zone 2 and 6 area. Cortical hypertrophy was present in 32 percents. Fourteen cases had the gaps at the bone to acetabular cup interface in the immediate postoperative period. The gaps disappeared in 12 months after operation. Five implants were revised because of polyethylene wear, cup migration and periprosthetic fracture. The clinical outcome was excellent. But this result was obtained in the relatively short-term follow up. Longer-term data will be the only true test of the value of hydroxyapatite coated implant.

7.Study on the Factors Related to the Cognitive Function and Depression Among the Elderly.

To investigate the factors which affecting the cognitive function and depression of the 65 or more age group, the authors surveyed for the subjects in the region of Taejon and nearby Taejon area. 729 studied subjects were tested for cognitive function with MMSE and depression with GDS. The main results were followings; In the studied subjects, the rate of normal cognitive function was 56.8%, the rate of mildly impaired was 24.l% and the rate of severe impairment was 19.l%. The cognitive function level was closely related to the depression score. As the age increased, the cognitive function was more impaired. Sexual difference was also existed in the cognitive function level and the depression score. After adjusting the effect of age, the variables such as sex, marital status, education level, past job, instrumental ability of daily living, regular physical exercise, frequencies of going out the house, chest discomfort, visual and auditory disturbance, and dizziness had the significant relationship with cognitive function impairment. Among these variables instrumental ADL, age, visual disturbance, and sex showed statistical significance in the logistic regression model. In the multiple stepwise regression, the variables which had significant relationship to depression score were education level, frequencies of going out house, current job and house work activity, regular physical exercise, instrumental ADL, self-rated health and nutritional status, dizziness, visual disturbance, and chest pain. In conclusion, main characteristics which had close relationship to the cognitive function and depression symptoms in the studied subjects were physical function and self rated health status.

8.A Case of Midline Malignant Reticulosis Associated with Distant Skin Metastases.

Midline malignant reticulosis is a relentlessly progressive, localized, destructive inflammatory process of upper respiratory tract including the nose, nasopharynx, palate, paranasal sinuses, and midface. Histologically, it is characterized by a dense aceumulation of pleomorphic mononuclear cells with an admixture of inflammatory cells. We report a case of midline malignant reticulosis developed in a 64-year-old man, who showed a serosanguineous nasal discharge, perforation of nasal septum, persistent ulceration of hard palate, and disseminated lesions on the abdominal skin, which was confirmed by histopathalogical examination. In addition we studied infiltrated cells with atypical nuclei by immunohistochemical staining with a pannel of monoclonal antibodies. The results revealed that the atypical cells expressed the T-cell marker, but not the macrophage and B-cell marker.

9.Cerebral Metastases and Menifestation of the Alveolar Soft Part Sarcoma.

Alveolar Soft Part Sarcoma(ASPS) in an unusual tumor of soft tissue, it has invariably ended in death from disseminated disease, and the lung has been the most common site of metastasis, but the brain was the 3rd most common site of metastasis. That proportion seems 0.4% of all soft tissue sarcoma. Clinical, pathological, and radiological features of metastatic cerebral lesions resulting from ASPS are presented. Angioarchitecture, and radiotherapic finding and chemotherapy are discussed. The unusual juxtaposition of the tumor cells to the endothelium seems responsible for the highly vascular appearance of the lesion on angiography, the frequency of hematogenous spread, and the grave, although delayed, prognosis. The purpose of this paper is to be report ASPS which has never been reported in Korea yet, with some references.

10.Tremendous Skull Metastasis from Follicular Thyroid Carcinoma: Case Reort.

A rare and tremendous skull metastasis from the follicular thyroid carcinoma is reported with review of the articles. Follicular carcinoma of the thyroid with distant metastasis is considered a relatively progressive tumor associated with poor five-year survival rate. The insidious character of the primary growth of the thyroid carcinoma is the long latent period which supervenes between the recognition of the primary growth and its secondary metastasis. The clinicopathological features, plain X-ray and C-T findings are discussed.
