Background:
The public health issue of consuming
groundwater is a major concern because people often
extract groundwater directly from the aquifers either
through wells or boreholes without treating it with
any form of filtration system or chlorine disinfection.
Based on the Malaysian National Drinking Water
guidelines the current study was designed to provide a
better understanding on the variable factors that are
influencing the quality of well-water in an urbanised
village in Malaysia. Well water quality assessment
of heavy metals, chemicals, microbial and physical
parameters were carried out for Sungai Buloh Village
in the Klang Valley to ensure it was safe for human
consumption.
Materials and Methods:
Water samples were collected
from wells at four sites (Sites A,B,C,D), a river and a
tap inside a house in Sungai Buloh village. Soil was
sampled from the riverbed and area surrounding the
wells. Samples were collected every two months over
a one year duration from all sites. The water samples
were processed and examined for viruses, coliforms and
protozoa as well as for heavy metal contaminants.
Results:
The turbidity and colour ranged in the average of 0.57-0.13 Nephelometric Turbidity (NTU) and 4.16-
5.00 Total Conjunctive Use (TCU) respectively for all
sites except Site C. At Site C the turbidity level was
2.56 ± 1.38 NTU. The well-water was polluted with
coliforms (1.2 to 2.4 x 10

CFU/100 ml) in all sites,
E. coli
(0.12 - 4 x 10
2
CFU/100 ml CFU/ 100 ml) and
Cryptosporidium
oocysts (0.4 cysts/100 ml). All the
heavy metals and chemical parameters were within the
Malaysian Guidelines’ limits except manganese. The
average pH ranged from 5.44 - 6.62 and the temperature
was 28 ºC.
Conclusion:
In summary, the well water at Sungai
Buloh is considered unsafe for consumption due to
pollution. Therefore the major thrust will be to provide
better quality of drinking water to the residents of the village.

INTRODUCTION:
Three doses of SARS-CoV-2 mRNA vaccines have been recommended for cancer patients to reduce the risk of severe disease. Anti-neoplastic treatment, such as chemotherapy, may affect long-term vaccine immunogenicity.
METHOD:
Patients with solid or haematological cancer were recruited from 2 hospitals between July 2021 and March 2022. Humoral response was evaluated using GenScript cPASS surrogate virus neutralisation assays. Clinical outcomes were obtained from medical records and national mandatory-reporting databases.
RESULTS:
A total of 273 patients were recruited, with 40 having haematological malignancies and the rest solid tumours. Among the participants, 204 (74.7%) were receiving active cancer therapy, including 98 (35.9%) undergoing systemic chemotherapy and the rest targeted therapy or immunotherapy. All patients were seronegative at baseline. Seroconversion rates after receiving 1, 2 and 3 doses of SARS-CoV-2 mRNA vaccination were 35.2%, 79.4% and 92.4%, respectively. After 3 doses, patients on active treatment for haematological malignancies had lower antibodies (57.3%±46.2) when compared to patients on immunotherapy (94.1%±9.56, P<0.05) and chemotherapy (92.8%±18.1, P<0.05). SARS-CoV-2 infection was reported in 77 (28.2%) patients, of which 18 were severe. No patient receiving a third dose within 90 days of the second dose experienced severe infection.
CONCLUSION
This study demonstrates the benefit of early administration of the third dose among cancer patients.

5.Predictors of difficulty in intubation in patients with obstructive sleep apnoea

Objective: To evaluate predictors of difficult intubation in
patients with obstructive sleep apnoea (OSA).
Methodology: Prospective series of 405 OSA patients (350
males/55 females) who had upper airway surgery.
Procedures included functional endoscopic sinus surgery,
septoplasty, turbinate reduction, palate/tonsil surgery,
and/or tongue base surgery. Intubation difficulty (ID) was
assessed using Mallampati grade, Laryngoscopic grade
(Cormack and Lehane), and clinical parameters including
BMI, neck circumference, thyromental distance, jaw
adequacy, neck movements and glidescope grading.
Results: Mean age was 41.6 years old; mean BMI 26.6; mean
neck circumference 44.5cm; mean Apnea Hypopnea Index
(AHI) was 25.0; and mean LSAT 82%. The various laryngeal
grades (based on Cormack and Lehane), grade 1 - 53
patients (12.9%), grade 2A - 127 patients (31.0%), grade 2B -
125 patients (30.5%), grade 3 - 93 patients (22.7%) and grade
4 - seven patients (1.7%); hence, 24.4% had difficulties in
intubation. Parameters that adversely affected intubation
were, age of the patient, opening of mouth, retrognathia,
overbite, overjet, limited neck extension, thyromental
distance, Mallampati grade, and macroglossia (p<0.001).
Body mass index (BMI) (p=0.087), neck circumference
(p=0.645), neck aches (p=0.728), jaw aches (p=0.417), tonsil
size (p=0.048), and AHI (p=0.047) had poor correlation with
intubation. BMI-adjusted for Asians and Caucasians,
showed that Asians were more likely to have difficulties in
intubation (adjusted OR = 4.6 (95%Confidence Interval: 1.05
to 20.06) (p=0.043), compared to the Caucasian group.
6.Percutaneous transcatheter aortic valve implantation for degenerated surgical bioprostheses: the first case series in Asia with one-year follow-up.

INTRODUCTIONPercutaneous transcatheter aortic valve implantation (TAVI) has become an established therapy for inoperable and high-surgical-risk patients with severe aortic stenosis. Although TAVI in patients with degenerated surgical aortic bioprostheses (i.e. valve-in-valve TAVI) is increasingly reported in Western studies, such data is lacking in Asian patients. We describe the initial experience of valve-in-valve TAVI in Asia.
METHODSEight patients who underwent valve-in-valve TAVI due to degenerated aortic bioprostheses were enrolled. The mechanism of bioprosthetic valve failure was stenotic, regurgitation or mixed. All procedures were performed via transfemoral arterial access, using the self-expanding CoreValve prosthesis or balloon-expandable SAPIEN XT prosthesis.
RESULTSThe mean age of the patients was 71.6 ± 13.2 years and five were male. Mean duration to surgical bioprosthesis degeneration was 10.2 ± 4.1 years. Valve-in-valve TAVI was successfully performed in all patients. CoreValve and SAPIEN XT prostheses were used in six and two patients, respectively. There were no deaths, strokes or permanent pacemaker requirement at 30 days, with one noncardiac mortality at one year. All patients experienced New York Heart Association functional class improvement. Post-procedure mean pressure gradients were 20 ± 11 mmHg and 22 ± 8 mmHg at 30 days and one year, respectively. Residual aortic regurgitation (AR) of more than mild severity occurred in one patient at 30 days. At one year, only one patient had mild residual AR.
CONCLUSIONIn our experience of valve-in-valve TAVI, procedural success was achieved in all patients without adverse events at 30 days. Good clinical and haemodynamic outcomes were sustained at one year.

7.Extrapulmonary manifestations and complications of severe acute respiratory syndrome coronavirus-2 infection: a systematic review.

INTRODUCTION:
We aimed to describe the extrapulmonary manifestations of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection, including their frequency, onset with respect to respiratory symptoms, pathogenesis and association with disease severity.
METHODS:
We searched the MEDLINE and Embase databases for SARS-CoV-2-related studies. Meta-analysis, observational studies, case series and case reports published in English or Chinese between 1 January 2020 and 1 May 2020 were included. Reports with only paediatric or obstetric cases were excluded.
RESULTS:
169 articles were included. Early manifestations (preceding respiratory symptoms until Day 6 of onset) included olfactory and gustatory disturbance (self-reported in up to 68% and 85% of cases, respectively), gastrointestinal symptoms (up to 65.9%) and rash (up to 20.4%). From Day 7 onwards, hypercytokinaemia, paralleled multi-organ complications including acute cardiac injury (pooled incidence of 17.7% in 1,412 patients, mostly with severe disease and 17.4% mortality), kidney and liver injury (up to 17% and 33%, respectively) and thrombocytopenia (up to 30%). Hypercoagulability resulted in venous thromboembolic events in up to 31% of all patients. Uncommon disease presentation and complications comprised Guillain-Barré syndrome, rhabdomyolysis, otitis media, meningoencephalitis and spontaneous pneumomediastinum.
CONCLUSION
Although the systemic manifestations of SARS-CoV-2 infection are variegated, they are deeply interwoven by shared mechanisms. Two phases of extrapulmonary disease were identified: (a) an early phase with possible gastrointestinal, ocular and cutaneous involvement; and (b) a late phase characterised by multiorgan dysfunction and clinical deterioration. A clear, multidisciplinary consensus to define and approach thromboinflammation and cytokine release syndrome in SARS-CoV-2 is needed.
