2.Vascularized Bone Graft for Spine and Limb Reconstruction

We analysed the thirty three cases of the free vascularized bone graft performed in the Department of Orthopaedic Surgery, Yonsei University College of Medicine from February 1982 to December 1989 and would like to report the results as follows. 1. There were 27 cases in men and 6 cases in women and the mean age was 27.3 years. 2. We have performed the procedure for the reconstruction of the large bony defects developed after massive trauma in 13 cases, core decompression of the avascular necrosis of the femoral head in 8 cases, tumoral resection in 6 cases, treatment of congenital pseudoarthrosis in 3 cases, and bone infection in 3 cases. 3. The most common recipient site was tibial shaft (18 cases, 54%) and the most common donor site was fibular shaft (23 cases, 69%). 4. The average length of bone defect (or grafted bone) in 23 cases of vascularized fibular graft was 9.8 cm and in 9 cases of vascularized osteocutaneous iliac bone graft, it was 5.8 cm. 5. The follow-up period was ranged from 13 months to 5 years 8 months (mean, 2 years 2 months). 6. All patients were evaluated clinically and roentgenographycally on the basis of fuctional recovery and bony union. Of the 33 cases, 31 cases (93%) showed satisfactory functional recovery with sound bony union. 7. It is highly expected that the vascularized bone graft is a one of the better alternatives for the reconstruction of the large bone defect and prevention of the early state of the avascular necrosis of the femoral head from progession.

3.Synchronous Papillary Tumor of the Fallopian Tube and Endometrium: A Case Report.

Synchronous tumor of the fallopian tube and endometrium is an unusual co-occurrence of gynecologic malignancies. To the best of our knowledge, there have been no reports on synchronous papillary tumor of the fallopian tube and endometrium. In this report, we present the case of a patient who synchronously suffered with papillary serous adenocarcinoma of the fallopian tube and endometrium and the tumor showed characteristic frondlike projections on magnetic resonance imaging.

4.A Case of Herpes Zoster Ophthalmicus Complicated by Abducens Palsy.

The complications of herpes zoster ophthalmicus vary greatly among different patients. The major portion of the complications fall under four types; (1) keratitis, (2) iridocyclitis, (3) muscular palsies, and (4) optic neuritis. We have experienced a case of herpes zoster ophthalmicus complicated by abducens palsy, sectorial paralysis of iris sphincter, iritis, and transient increase of intraocular pressure. The case was reviewed and the pathogenesis of the complications were discus sed.

5.Efficacy of the Focused Ultrasound as an Additional Study to Characterize Small Hepatic Lesions on CT in Patients with Cancer.

PURPOSE: The purpose of this study was to evaluate the efficacy of focused ultrasound (US) to characterize small indeterminate hepatic lesions on contrast-enhanced CT in cancer patients. MATERIALS AND METHODS: Forty-three consecutive patients with proven cancers developed 70 indeterminate hepatic lesions that were less than 1.5 cm in size, as detected on contrast-enhanced CT. Two radiologists performed ultrasound focusing on the targeted area after referencing the location of the hepatic lesions on a previous CT image. When the hepatic lesions were visualized by focused US, the lesions were characterized as cysts, hemangiomas, or solid/metastases. Verification of the hepatic lesions was assessed by a histopathological examination or by follow-up imaging. RESULTS: Focused US detected 51 (73%) of 70 indeterminate hepatic nodules on CT and all of the nodules visualized on focused US could be characterized as cysts (n = 40), hemangiomas (n = 7), or solid/metastases (n = 4). All of the cysts and six hemangiomas were verified as benign lesions. One hemangioma was subsequently identified as a metastasis. Nineteen (27%) indeterminate hepatic nodules were not visible on focused US and 18 nodules were verified as benign lesions. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of focused US for differentiating metastases among indeterminate hepatic nodules less than 1.5 cm in size on CT were 66.7%, 100%, 97.1%, 100%, and 97%, respectively. CONCLUSION: Focused US may be the next appropriate examination to perform for cancer patients with indeterminate hepatic nodules detected on CT, Focused US may be a pivotal modality for defining prognosis and treatment.

6.The Difference of Body Mass Index According to Smart Phone Proficiency in Koreans over the Age of 60.

PURPOSE: The purpose of this study was to compare the difference of body mass index (BMI) to smart phone proficiency in men and women over the age of 60. METHODS: Patients were divided into three groups with high (n=33), average (n=34), and low (n=33) smart phone proficiency. Fitness characteristics related to smart phone usage were evaluated by measuring cardiorespiratory endurance, grip strength, eye-hand coordination. As well, smart phone proficiency was evaluated by a self-reported questionnaire and a smart phone usability task that was composed of two categories: usage of the smartphone device itself and usage of phone applications. The differences in BMI of the subjects was analyzed by analysis of covariance adjusting for independent variables including age, smartphone usage period, eye-hand coordination, education and income. RESULTS: There was a significant difference in BMI among the three groups after adjustment of age, eye-hand coordination, smartphone usage period, education and income. The results showed that the self-reported questionnaire showed a significant difference in BMI between high proficiency and low proficiency groups (high 24.88±2.46, low 23.37±2.56; p=0.037). Smart phone usability test results also showed a significant difference in BMI among the three groups (high 25.18±2.58, low 23.15±2.6; p=0.000 and high 25.18±2.58, middle 23.57.7±1.69; p=0.010). CONCLUSION: Our results suggest that high smart phone proficiency shows increased BMI in the elderly. This study suggests that people over the age of 60 who have high smartphone proficiency should be cautious of an increased BMI score.

7.Exploring Factors Related to Menopausal Symptoms in Middle-aged Women:A Study of Body Composition, Physical Fitness, and Physical Activity

Purpose:
The aim of study is to analyze the associations between menopause symptoms, including sleep quality and depressive symptoms, and body composition, physical fitness, and physical activity indices in middle-aged women.
Methods:
This cross-sectional study included 91 women aged 45 to 55 years. The study employed the Menopause Rating Scale to assess menopausal symptoms, the Pittsburgh Sleep Quality Index to evaluate sleep quality, and the Center for Epidemiologic Studies Depression scale to measure depressive symptoms. Cardiorespiratory fitness was assessed by breath-by-breath respiratory exchange during an incremental cycle ergometer, while muscle fitness was assessed by hand grip strength measurement. Physical activity was assessed using both the Global Physical Activity Questionnaire and Fitbit devices.
Results:
According to correlation analysis, indicators of obesity such as body fat and waist circumference were positively associated with menopausal symptoms. Conversely, muscle fitness and engaging in light physical activity were negatively associated with these symptoms.
Conclusion
This study identified physical indicators in Korean middle-aged women linked to menopausal symptoms.This study suggests that exercise programs for middle-aged women should include resistance training and promote physical activity to improve obesity markers and manage menopausal symptoms for healthy aging.
8.Feasibility of Adjustable Electrodes for Radiofrequency Ablation of Benign Thyroid Nodules

OBJECTIVE: The purpose of this study was to evaluate a novel radiofrequency ablation (RFA) application utilizing an adjustable electrode for treatment of benign thyroid nodules.MATERIALS AND METHODS: From April 2011 to December 2018, 21 patients underwent RFA treatment on 21 thyroid nodules, utilizing an 18-gauge internally cooled electrode equipped with a size adjustable active tip. The peripheral nodule portions were ablated with the moving-shot technique and a shorter active tip, and the nodule centers were ablated with the fixed technique and a longer active tip. We assessed parameters including characteristics of the treated nodules, use of variablesized active tips, volume reduction rate, therapeutic success rate, and post-procedural complications. The therapeutic success rate was defined as a > 50% volume reduction of the initial nodule volume at the 6- or 12-month follow-up.RESULTS: The treated thyroid nodules were large enough to cause symptoms (mean volume, 29.6 mL). Two types of active tips per session were used for all nodules. The mean volume reduction rate at the last follow-up was 68.3 ± 4.4% and our therapeutic success rate was 90.5%. Both symptoms and cosmetic scores decreased significantly. Minor complications in three patients were recorded during and after the procedure.CONCLUSION: This initial study demonstrated that an adjustable electrode for RFA of benign thyroid nodules effectively and safely resulted in volume reduction.

9.Development of VX-II Carcinoma Model in Rabbit Uterus: Evaluation with MR Imaging and Histopathology.

PURPOSE: To develop a VX-II carcinoma model in the rabbit uterus and to describe the MR imaging findings of an experimentally induced VX-II uterine carcinoma along with the histopathologic findings. MATERIALS AND METHODS: 13 New Zealand rabbits were included in this study. Two pieces of tissue suspension (1 mm3x2) of VX-II carcinoma were loaded in an 18 gauge disposable needle and the tissue embedded in the wall of each horn of the rabbits' uterus. We obtained the MR images at 2 weeks in group A (n=5), or at 2 and 4 weeks in group B (n=8). T2-weighted images were obtained using an extremity coil. On MR imaging, we measured the signal intensity of the tumor and the uterine wall. We also measured the size and shape of the tumor and we compared this with the histopathologic results. RESULTS: On MR images obtained 2 weeks after inoculation, all the rabbit uteruses (group A and B, n=13) show a thick tubular wall, and the uteruses demonstrated a high signal intensity on T2-weighted images. The thickened uterine walls were measured as 3-10 mm (mean: 6.5 mm). Peritoneal nodules were observed in 3/13 rabbits (23%), the nodules measured 1.5-1.8x3.0 cm; uterine masses were observed in 3/13 rabbits (23%), and they measured 0.6-1.5x1.3-1.5 cm. On MR images obtained 4 weeks after the inoculations (group B, n=8), rabbit uteruses that had VX-II carcinoma show thick tubular wall in all cases (n=8, 100%) and round uterine masses (n=6, 75%). The thickened uterine walls measured 5-14 mm (mean 7.6 mm) and uterine masses measured 1.3-3.0x1.5-6.0 cm. All the rabbit uteruses having VX-II carcinoma showed high signal-to-noise ratios along the thickened uterine walls or masses on T2WI. On the histopathologic exam after sacrificing the rabbit, a few viable tumor cells were found because of necrosis in the inner portion of the uterine tumors, and abundant viable tumor cells were found at the periphery of the uterine tumors. CONCLUSION:We can develop an animal model with uterine tumor in rabbit uterus using VX-II carcinoma. Experimentally induced VX-II carcinomas in rabbit uterus demonstrated central necrosis, and MR imagings of experimentally induced uterine VX-II carcinomas were well correlated with the histopathology.

10.The Results of Radial Shortening Osteotomy for Kienbock Disease Stage III.

PURPOSE: Radial shortening osteotomy and ulnar lengthening osteotomy for decreasing axial loading have been known to treatment for avascular necrosis of lunate bone. The purpose of this study was to evaluate the clinical outcomes of radial shortening osteotomy for Lichtman stage III Kienbock disease. MATERIALS AND METHODS: Between December 2001 and October 2008, thirteen patients with Kienbock disease underwent a radial shortening osteotomy at our institution. On the basis of Lichtman classification, six had stage IIIA and seven had stage IIIB. Radiographic measurement of the ulnar variance and the carpal height ratio were assessed preoperatively and at the follow-up. Patients were examined for wrist pain, range of motion at flexion and extension and grip strength both preoperatively and postoperatively. The clinical outcomes was evaluated through the modification of Evans scoring system. RESULTS: All thirteen had maintained the preoperative stage at the follow-up. In ulnar variance, negative variance was seven. The carpal height ratio was increased mean 0.018 at the follw-up. Pain in VAS was improved mean 3.6 at the follow-up. In range of motion of wrist flexion-extension, in the eleven which had limitation of motion preoperatively, all eleven showed improvement. In grip strength, among the ten which had decreased preoperatively, eight showed improvement and two showed no change at the follw-up. The clinical outcomes were good in eight, fair in three and poor in two. Among the five, negative ulnar variance of stage IIIB, three had good, two had fair clinical outcomes. CONCLUSIONS: We found that radial shortening osteotomy can prevent disease progression, also show good clinical results for stage IIIB Kienbock disease as well as stage IIIA.
