1.Studies on the metabolism of C(14)-proline in some parasitic helminths.

A comparison of the absorption and incorporation of C(14)-proline into protein by the 7 kinds of helminth parasites is presented. The radioactivity of free amino acid fraction is greater than that of protein fraction in all the worms, and only a small amount of exogenous labeled proline is incorporated into tissue protein. In general, the pattern of C(14)-proline uptake and its incorporation into protein shows rapid linear increase during the period of 15 to 30 min and reaches the maximum at 60 min after incubation, and then the equilibrium state was maintained throughout further incubation.

4.Statistical Observation on Malignant Tumors of the Urinary Tract.

Statistical observation was made on 117 cases of malignant tumors in the urinary tract who had been admitted to Department of the Urology, St. Mary's Hospital from January 1, 1966 to May 31. 1972. Statistical observation included age, sex, clinical manifestation, annual distribution, organ distribution and methods of operation. The results were as follows; 1. Male patients were more common than female patients, and most of them were over the age of 40 years. Hematuria was the most common symptom, and other symptoms were pain, mass and dysuria. Operations were performed in 79 patients, and remaining patients were treated with radiation, chemotherapy and hormone therapy. 2. The urinary bladder was the most commonly involved organ, and other organs were in order of kidney, prostate and penis. Histologically, transitional cell carcinoma in the bladder was the most common and next in order was of renal cell carcinoma, Wilms' tumor and squamous cell carcinoma of penis.

5.Korea University Medical Network System-Laboratory (KUMNET-LAB) (I).

BACKGROUND: Korea university Gyro Hospital has run the KULAB system successfully since 1985, the increasing amount of hospital information made us the need for new system with larger capacity. We began to transform the KULAB, LIS only to the wly developed KUMNET-LAB, OCS system and we report our experience. METHODS: We used 5290 (Sequent, USA) with main memory of 128 megabyte and hard disk of 6.3 gigabyte as main completer, ORAGLE 7.2 (Oracle, USA) as database, fifteen 486 PCs, eight printers and seven barcode printers in the laboratory. A LAN was built. Pro-C was used in interface between the outpatient cashier and LIS. The Power builder (PentaKorea, USA) and Forms (Oracle, USA) and visual basic were used in interface between LIS and autoanalyzer. It took one month to analyse laboratory work flow and 2 months to programming by 2 programmers from the commercial vendor, ICM (Information & Communication Management Inc.). All laboratorian had taken part in job analyses and evaluations. RESULTS: Our laboratory information system was successfuly implemented without any major problem. The specimen type and emergency status were added to the test code. And the field indicating the sequential position on screen was added to the master to control the position of the test codes on screen. Bar code system was introduced to ample identification as well as specifically assigned specimen number. CONCLUSION: The KULAB system had been used from 1985 through 1995 was successfully transfered to the more efficient new LIS, KUMNET-LAB which is one of the basis of the OCS system (KUMNET).

6.A Study on the Effect of Various Dosage of Khellin on Epidermal Melanocytes and a Comparison with Trimethylpsoralen in Systemic Photochemotherapy.

Ninety black C 57 BL mice were divided into control and photochemotherapy groups. Control group was subdivided into a UVA 1rradiation only subgroup and a khellin only administration subgroup, while the phototherapy group was subdivided into one TMP and two khellin administration subgroups with dosages of 1.5mg/kg, 5mg/kg and 20mg/kg, respectivly. UVA was administered three thimes a week in a dose of 4J/cm for 4 weeks. Skin biopsies were taken at 0, l, 2, 3 and 4 weeks and the split DOPA stain was employed to observe pigment production. The number, size and circumference of the melanocytes were assessed. In summary, our results inclicate that khellin, which is slightly less effective than TMP at the same effective dose, is quite effective for pigment production and the degree of the production is dose related.

7.Referral Patterns and Needs for Specialist Care among Patient Referred from Health Center.

This study was conducted to assess the referral patterns to specialist from general practitioners in health center and perceived needs of referred patients for specialist care in health center. The study subjects were 249 patients who visited to health centers and were referred to other medical facilities. The data were obtained from questionnaire survey which was conducted in Kyongjuu-city Health center, Seongju-Gun Health center and Koryong-Gun Health center in Kyongsangbuk-Do, from June 10 to october 17, 1995. The total referral rate was 2.7%. The proportion of patients who wished to be referred to medical specialists was 85.9%, and the proportion of patients referred by general practitioners in health centers was 14.1%. Among the patients who wished to be referred to medical specialists, 45.9% visited directly to health centers, 34.6% visited health centers via local clinics and 19.5% visited health centers to get referral permission only. The reasons for getting referral permission in health centers were easy geographical accessibility(34.6 %), easy to get referral permission in health centers(27.l%), and very difficult to get referral permission in local clinics(l6.4%). Among the diseases of referred patients, diseases of the musculoskeletal system and connective tissue were most prevalent on a whole, but diseases of nervous system and sensory organs were comparatively high among the patients who wished to be referred to medical specialists and infectious and parasitic diseases were comparatively high among the patients referred by general practitioners in health centers. The most favorable medical facility was general hospital including university hospital in both groups of patients who wished to be referred to medical specialist and the patient referred by general practitioners in health centers. Regarding the needs for specialist care in health center, 75.2% patients who wished to be referred to medical specialists and 74.3% of patients referred by general practitioners in health center wanted the specialist care. The most frequently requested speciality is internal medicine(47.1%), and then orthopedics and general surgery. Based on above results, this study revealed that the majority of patients referred from health center wished to be referred to medical specialists at their own will, so, referral system at health center level should be changed. And if specialist care in health center be provided, the medical care by internist could be provided first, and then that of orthopedics and general surgery could be provided. These kinds of medical cares could be covered by local clinicians as a part-time job on a voluntary basis.

8.Prosthetic Replacement in Femoral Neck Fracture

Twenty seven fractures of the femoral neck in 26 elderly patients treated by primary replacement with the endoprosthesis were reviewed. The end results of 18 hips of 17 patients who were followed up at least over 6 months were as follows. The average age was sixty-seven years. Seventeen fractures occurred in women and ten in men. Of these 18 hips, eleven hips were excellent or good. Poor results were in two hips; one developed deep seated infection and the other patient who could walk with crutch after bilateral endoprosthetic replacement because of Lt hip pain. It is considered that this method is the best treatment in the rehabilitation of elderly patients after high femoral neck fracture, because of its low morbidity and high success rate in returning the geriatric orthopaedic patients to independence.

9.Clinical study of segmental vitiligo.

This is an analysia of 126 cases(14.1%) of segmental vitiligo among 892 vitiligo patient who had visited vitiligo special clinie in Severance Hospital. The results are summarized as follow : 1. There were 53 males(42.1%) and 73 females(57.9%). 2. The mean age of onset was 15.4 years, the mean age on the first visit was 19.3 years, and mean duration of the disease was 4.8 years. 3. Mode of onset was single in 86.5% and the disease was table in 57.1% of patients at the visit. 4. The mean percentage of depigmented lesions was 3.3% and less than 5% of body surface area was involved in 86.5% of patients. 5. The most common site of involvement was head and necl(59.6%), especially face(43.7%) and the trigerminal dermatome was most commonly involved. 6. Poliosis was observed in 39.7%. 7. Family history of vitiligo was obtained in 11.1% of patients. There was no precipitating factors in 126 cases prior to development of vitiligo. 8. Koebner phenomenon was found in 4% of patients. 9. Association with diseases of a proven or suggested allergic or immunologic etiology including atopic dermatitis, halo nevus, uveitis, thyroid disease, lopecia areata and premature graying of hair was found in 10.3% of patients.

To our knowledge, rectal cancer arising at the anastomotic site after surgery for Hirschsprung's disease has not been reported. We report a case of mucinous adenocarcinoma arising at the anastomotic site after Soave operation 26 years ago.
