1.The Echocardiographic Study on the 13 Patients with the Hypertrophic Cardiomyopathy.

The echocardiographic study was performed to 13 cases with Hypertrophic Cardiomyopathy and 105 normal persons between Nov. 1980 and Feb. 1982 in Heart center, Paik Hospital, In-Je Medical College, Busan, Korea. The left ventricular functions in the Hypertrophic cardiomyopathy were compared with those in the normal. The results were followings: 1. There were 10 male and 3 female of 13 cases with Hypertrophic Cardiomyopathy, whose ages were above 20 years old. 2. LVDeD 4.29+/-0.60cm, LVSeD 2.85+/-0.66cm, LVDeV 82.06+/-24.66ml and LVSeV 26.91+/-9.21ml in the Hypertrophic Cardiomyopathy were significantly changed with those in the normal. 3. E.F. 68.63+/-10.46% and F.S. 34.32+/-6.43% in the Hypertrophic Cardiomopathy were significantly increased with those in the normal. 4. VSTh 1.66+/-0.37cm, LVPWTh 1.04+/-0.29cm in the Hypertrophic Cardiomyopathy were significantly increased with those in the normal.

2.A Case of A cute Fevrile Neutrophilic Dermatosis.

A 36-year-old man has suffered from multiple, well defined, variable sized, and elevated papulonodular patches with oozing, scale, and crust on the face. He also presented multiple folliculitis-like eruptions on upper chest, back, and both limbs. He had no leukocytosis, but elevated ESR and fever during development of skin lesions. Histologically a dense iaflammatory cell infiltra,tion composed predominantly of polymorphonuclear neutrophils and few lymphoid cells were seen in the dermis and subcutis. The symptoms and signs did not spond to dapsone and tetracycline, but disappeared on orally administered predniselone.

4.Anterior Fixation with Kaneda Device for Unstable Fractures of the Thoracolumbar Spine

Recently posterior stabilization with various instrumentation and fusion has been used for fractures of the thoracolumbar spine involving anterior and middle columns. However, these methods are sometimes inadequate to gain anatomical reduction and complete decompression especially in burst fractures. So anterior decompression and interbody fusion were frequently added as a second operation. Biomechanically a rigid anterior spinal instrument such as kaneda device can provide enough anterior decompression through partial or total corpectomy and adequate correction of kyphosis as an one stage operation. We have experienced 27 cases of Kaneda instrumentation via anterior approach for thoracolumbar fracture, most of them were burst fractures, from Jan. 1989 to June 1990. Of there 27 cases, 12 were followed up at least 1 year and reviewed. The results were as followed: l. Anterior spinal approach using Kaneda instrumentation provided sufficient anterior neural decompression and adequate correction of kyphotic deformity. It eliminates the second posterior procedure in most cases and enhances early solid union. Simultaneously, it allows early mobilization of patient with an application of brace. 2. Post-operative courses were eventful in most cases except a case of pathological fracture. 3. This procedure seemed to be one of the most suitable method for the treatment of burst fractures of the thoracolumber region. 4. Complications by Kaneda device itself were not developed in the early evaluation stage. However, on longer follow-up study over 1 year revealed some complications including 2 cases of screw breakage and one lateral wedging deformity.

7.A Case of Zosteriform Cutaneous Metastasis.

We report a case of zosteriform cutaneous metastasis from lung cancer in a 72- year-old male patient who have asymptomatic confluent or discrete hard papulonodules over the right upper arm and right side of chest. Histopathologic examinations of the cutaneous lesion revealed neoplastic cell, which showing glandular structure, infiltrating in the drmis and subcutaneous layer. Also histopathologic findings of metastatic lesion of skull and olecranon are similar to them. Fie died after three months from the appearance of cutaneous metastasis

8.Sonographic findings of biliary tract disease

Fourtyone patients fo gallbladder and bile duct diseases were studied clinically and sonographically.Tweentynine (Seventyone percent) patients were distributed between age fourty to fiftynine and male to femaleratio was 1:1.4. The order of frequency of biliary tract disease was cholelithiasis, acalculous cholecystitis, CBDstone and CBD cancer. Sonographic findings of cholelithiasis were strong echo with posterior shadowing, faintinternal echoes without shadowing, gallbladder wall thickneing and anechoicity of the gallbladder wall. Instead ofsmall proportion of gallbladder distension and wall anechoicity, faint internal echoes without shadowing were seenin ten of nineteen cases of cholelithiasis. On choledocholithiasis, meniscus sign at the junction of the stone andgallbladder wall was identified in most cases and was helpful to differentiation stone from malignancy. The degreeof CBD dilatation was more severe in malignancy than in CBD stones and ascaris in CBD. Sonographic examination wasuseful in detection of gallbladder and bilicary tree pathology and the cause of biliary tract obstruction could beidentified.
