1.CT Findings of Focal Organizing Pneumonia: Correlation with Pathologic Findings.

PURPOSE: To evaluate the CT findings of focal organizing pneumonia and to correlate them with pathologic findings to help differentiating from lung cancer. MATERIALS AND METHODS: We evaluated radiologic and pathologic findings of five patients with solitary pulmonary nodule which were confirmed as focal organizing pneumonia pathologically. RESULTS: On CT scan, focal organizing pneumonia had irregular margin contacting the pleura in all five cases. The shape of the nodules were spherical to wedge or elliptical and the size from 3.5cm to 5.5cm (average 4.2cm) in largest diameter. On postcontrast CT scan, all nodules showed enhancement and four cases showed central low density components. Two nodules contained air within the nodule. In four cases, pleural changes such as effusion and/or focal thickening were noted. No lymphadenopathy was found in all cases. Pathologically, the enhancing portion on CT showed findings of organizing pneumonia such as granulation tissue with fibroblast proliferation in alveolar space and interstitial thickening. The central low density areas on CT were due to ischemic necrosis, abscess and exudate, transudate and infiltration of foamy histiocyte. CONCLUSION: The possibility of focal organizing pneumonia should be considered when peripherally located solitary pulmonary nodule had enhancing component with no combined lymphadenopathy on CT scan.

4.Clinical Study of Mucocutaneous Lymph Node Syndrome.

We observed clinically 27 cases of MCLS at this pediatric department from Jan. 1980 to Jun. 1984 and following result were obtained. 1) The peak incidence of these cases was from 1 year to 4 year of age(74%) and male children were affected more frequently than female children at a ratio of 2.4:1. 2) Clinical manifestation of MCLS were high fever, conjunctioval injection, changes of oral cavity and lips, erythematous rash, cervical lymphadenopathy, changes of the extremities. 3) The abnormal laboratory findings include leukocytosis, slight anemia, raised ESR, positive CRP, mild proteinuria or pyeuria, and slight increase of serum transaminase and LDH. 4) The abnormal cardiovascular findings were as follow : heart murmur in 15% cardiomegaly in 19% by chest roentgenogram, electrocardiographic abnormalities in 59%, suspicious aneurysmal dilatation and coronary aneurysm by 2-D echocardiogram in 11%. 5) Aspirin was given in acute febrile stage as antiinflammatory dose(100mg/kg/day), in non-febrile convalescent stage to maintain low dose of aspirin(20-30mg/kg/day) as antiplatelet effect, and completely recovered in all cases without andy complication.

The leiomyoma of the vagina is relatively rare benign solid tumor, and majority of them were not recognized until after operation. Variable opinion exists as to the exact origin of this lesion. The most frequent location is the anterior vaginal wall. Recently we experienced a case of vaginal leiomyoma arising in the anterior vaginal wall and now we present it with a brief review of the literatures.

8.Interleukin-10 Gene Polymorphism in patients with Schizophrenia.

OBJECT: This study was aimed at comparing the Interleukin-10 gene polymorphic variants of patients with schizophrenia with those of normal controls, to investigate its contribution to the development of schizophrenia. METHOD: Two hundred and thirty-three patients with schizophrenia in accordance with DSM-IV criteria and 181 healthy individuals participated in this study. DNA was extracted from whole blood using proteinase K, and the interleukin(IL)-10 gene promoter region was amplified by polymerase chain reaction(PCR). Gene typing was analyzed by restriction fragment length polymorphism(RFLP) and single strand conformation polymorphism(SSCP). RESULTS: Distributions of the alleles and haplotypes of IL-10 gene in patients with schizophrenia were not significantly different from those of controls. There was no difference in the frequencies of alleles and haplotypes between patients with paranoid subtype and with non-paranoid subtype. CONCLUSION: In this study, we found no relataionship between IL-10 gene polymorphic variants and schizophrenia. Further systemic studies including adjacent genes and diverse clinical variables may reveal the effects of IL-10 gene on the susceptibility to schizophrenia.

9.Ozon-induced airway hyperresponsiveness of rat tracheal smooth muscle.

BACKGROUND: Ozone (03) induces airway inflammation and hyperresponsiveness which are characteristic features of asthma. There have been few studies observing O3-induced increase in responsiveness of rat airway muscle. Objectives: The aims of this study were to develop an O3-induced nonallergic asthma model using rat tracheal smooth muscle (TSM) and to evaluate the role of airway epithelium on the modulation of muscle responsiveness. METHOD: Five groups of 20 male Sprague-Dawley(SD) rats were exposed to filtered air including 0.12, 0.5, 1.0, or 2.0 ppm 03 for 1 hour. Thirty minutes after the exposure, bronchoalveolar lavage (BAL) and isometric contractile responses of the isolated tracheal ring segments to KCI, acetylcholine (ACh), and electrical field stimulation (EFS) were measured. RESULTS: The percent age of neutrophils was significantly higher and that of alveolar macro-phages in BAL fluid was significantly lower in 2.0 ppm O3-exposed rats than in the control. There were no significant differences in the maximal contractile responses of TSM to KC1, ACh, EFS and in the sensitivity to ACh (ACh-EC50) and EFS (EFS-EC50) between the control group and the ozone exposed group. ACh-EC50 and EFS-EC50 were correlated positively with the percent age of neutrophils and inversely with that of macrophages. Removal of epithelium significantly increased the sensitivity to ACh in O3-exposed group, but not in the control group. CONCLUSION: These findings indicate that O3 induces neutrophilic airway inflammation, but not an increased sensitivity of TSM to ACh or EFS in SD rats. However, O3-induced epithelial damage may be associated with increased muscle response.

10.Polymerase Chain Reaction and Heteroduplex Analysis Based Detection of Clonal T Cell Receptor Gamma Gene Rearrangements in Paraffin-embedded Tissues of Cutaneous T Cell Proliferative Diseases.

BACKGROUND: Recently, the molecular pathologic investigation for clonality in lymphomas has been introduced and has gained a role in the diagnosis of lymphomas. In fact, the clonality test using TCRGR phenomenon has been done by Southern blot analysis (SBA) and polymerase chain reaction (PCR) for molecular pathologic diagnosis of T cell lymphomas. However, it is difficult to perform SBA with paraffin embedded specimens or with samples of small skin biopsies. OBJECTIVE: We investigated the efficacy of PCR amplification of TCR gene in paraffin em-bedded cutaneous T cell lymphomas. METHODS: Iii this study, the clonality was assessed by polymerase chain reaction (PCR) analysis of T cell receptor gamma (TCR) gene from the DNA extracts obtained from paraffin em-bedded tissues (PET) of malignant T cells, B cell lymphomas, and benign cutaneous T cell proliferative disorders. Heteroduple-x-analyses were also performed to rule out the false positives. RESULTS: Among the total of 62 cases analyzed, monoclonality was observed in 4 out of 10 mycosis fungoides, 7 out of 9 cutaneous T cell lymphomas excluding mycosis fungoides, 1 out of 3 angiocentric lymphomas, 2 out of 2 lymphomatosis papulosis, 1 out of 7 large plaque parapsoriasis, and 1 out of 2 T cell lymphomas in other organs. No monoclonality was observed in 9 inflammatory cutaneous diseases, 5 small plaque parapsoriasis, 4 cutaneous B cell lymphomas, and 11 B cell lymphomas in lymph nodes. CONCLUSION: The results suggest that the PCR method and heteroduplex analysis used in this study were not only practical but also efficacious for the diagnosis of cutaneous T cell lymphomas using tissues embedded in paraffins.
