The tibia is the most commonly fractured bone of all the long bones. In this age of vehicular accidents, the tibia is frequently subjected to high energy trauma. Anatomically the tibia has poor soft tissue coverage and poor blood supply. Therefore severe injury on the tibia can lead to severe complication and major disability. 41 adult patients with comminuted fracture of the tibial shaft were treated by OR/IF with D.C.P. and bone graft at the Department of Orthopedic Surgery, Kyungpook University Hospital from January 1978 to June 1986. These were analysed according to mechanism of injury, degree of comminution, timing of operation, soft tissue injury and operation time. Bone union and end results of treatment were reviewed. The results were as follows Of 41 cases, causes of injury were vehicular accident in 32 cases. 34 cases had associated injury on other part of body and average injury per case was 1.7 injuries. Based on the classification of Johner and Wruhs, the B group with butterfly fragment was 27 cases and C group was 14 cases. Among the B group, B2 was most common as 14 cases. Average union time was 18 weeks and delayed union and nonunion were 6 cases. Degree of comminution of fracture might be important fractor to bone union. According to the end result of Johner and Wruhs, excellent and good were 31, fair and poor were 10. Of the 41 cases, 10 cases had permanent disability. 4 cases had angular and rotational deformity alone and 1 case had shortening of tibia more than 1 cm, 1 case had both of them. Limited R.O.M. of ankle joint (>50%) occured in 4 cases. The post-operative infection occured in 6 cases, 3 were superficial infection but 3 cases were osteomyelitis. OR/IF with D.C.P. on comminuted tibial shaft fracture was a risk operation. Therefore proper method of treatment should be selected according to degree of comminution, timing of operation, presence or abscence of open wound and associated injury.

Soft tissue defect on dorsum of hand requires immediate soft tissue cover to achieve early wound closure and to minimize wound infection and scarring. If the tendon or bone was exposed and the deep structures were absent, flap was required to provide adequate protection and allow proper function of the underlying structures. Small size in skin loss can easily be covered with local flaps but major defects involving the dorsal aspect of the hand should be coverd with distant flaps, free flaps or radial forearm flaps. Techniques in distant flaps require staged procedures and free flap transfer to obtain soft tissue cover in a single stage require microsurgical techniques. The reverse radial forearm flap is a fasciocutaneous flap based on the radial artery which together with its two venae comitantes lies invested in a condensation of the deep fascia known as the lateral intermuscular septum. The authors had treated 5 cases of distally based reverse forearm flap for soft tissue reconstruction on 2 patients of crushing injury with soft tissue defect on hand, a patient with Marjolin's ulcer on dorsum of hand due to previous burn scar and a patient with soft tissue necrosis on dorsum of both hands due to extravasation of adriamycin during chemotherapy. It has proved to be one of the saftest, simple and effective one-stage method of soft tissue reconstruction in hand without long term immobilization. The results of these operation in cover of soft tissue defect have been satisfactory.

This study includes 19 cases of complicated low extremity injury to whom underwent microvascular free-tissue transfer in cases of open comminuted fracture, osteomyelitis, and vascular compromise or extensive soft tissue defect from Nov. 1994 to July 1997. The average time after injury to coverage was 25 days. The nineteen patients underwent a total of nineteen free tissue transfers primarily, and eighteen (94.7 per cent) of the transfers survived. Major complications were encountered recurrent failure of the free tissue transfer in one patient and partial necrosis in the other flap, which were successfully treated by flow-through radial forearm free flap and distally based superficial sural artery flap respectively. Two patients were sustained below knee amputation, one above knee amputation, despite of survival of flaps. This reveals limb salvage in 84 percent. The microvascular reconstruction is a versatile and reliable methods in complicated cases after severe trauma. Free tissue transplantation should be considered as a primary treatment in such complicated cases.

Basal cell carcinoma is the most common skin cancer, especially on the eyelid and nose. As it rarely invades to a underlying bone or metastasizes an distant site, and is usually found at an early stage, it is regarded as a curative disease. However, basal cell carcinoma on the eyelid and nose may be resected incompletely due to efforts to preserve important structures or as a result of esthetic considerations. We experienced two cases of basal cell carcinoma with local invasion to underlying bone. One was a recurred case on the nasal area extending to the nasal bone arts medial wall of the ethmoid sinus. The lesion was widely resected and covered with a radial forearm free flap. The other was on the eyelid extending to the orbit. It was treated with orbital exenteration and resection of the involved eyelid. The defect was reconstructed with the temporalis muscle flap with split-thickness skin graft. These patients were followed up for 7 months with no evidence of recurrence. Since basal cell carcinoma can invade to the bone and metastasize to a distance site, it should be resected radically in the regions of the eyelid and nose.

This study was undertaken to grope the correlation of the maximal bite force and toothcraniofacial structure. The maximal bite force of 76 adult male, aged 18-28 (mean aged: 23.4 +/- 2.2) years, was estimated and cephalometric headplates were measured, tabulated and statistically analyzed. The results were as follows. 1. 59.61kg of bite force in first molar, 45.38kg in premolar and 17.10kg in central incisor were arranged. 2. The bite force was negatively correlated to gonial angle, mandibular plane angle, the angle between occlusal plane and mandibular plane, the angle between palatal plane and mandibular plane, and positively correlated to posterior height of face, length of mandibular body, length of ramus, facial depth in craniofacial structure. 3. The group with strong bite force showed small genial angle, rnandibular plane angle, the angle between occlusal plane and mandibular plane, the angle between palatal plane and mandibular plane, and long posterior height of face, length of mandibular body, length of ramus, facial depth. So they manifested the tendency to brachycephalic pattern, on the other hand, the group with weak bite force manifested the tendency to dolichocephalic pattern. 4. There is no correlationships between bite force and mesial inclination of premolar axis in this subject. 5. It is considered bite force have an effect upon craniofacial pattern, especially upon the lower face.

The aseptic loosening has emerged as one of the most common and serious long term complications, which requires the revision of total hip replacement arthroplasty. We studied the radiologic finding of the aseptic loosening in 79 patients of uncemented total hip replacement arthroplasty which had been performed in our hospital from March 1991 to December 1994. We evaluated the white reaction lines, osteolysis state, translation and migration in postoperative and last follow-up radiographies. The white reactive lines around the femoral stem and the acetabular cup were most frequent in zone IV and zone II. But it was not progressive and need more long term follow-up evaluations to confirm whether these lines are related to loosening or not. In assessment for stable state, the more stable at initial fixations, the better state of long term stability. Some patients have pain without loosening but all patients with loosening have pain. Press fit is the most important factor of the late stability. The osteolysis is not directly proportional to femoral loosening in this study, but it might be due to short follow up time.

Accidents are the leading cause of childhood mortality and rank second only to acute infections as the cause of morbidity. The hospital morbidity and long term sequelae of injuries in children have greater medical and social significance than in adults. In order to determine what role the skeletal injuries play in morbidity and residual impairment in the severely injured children, We studied 75 consecutive surviving children who had multiple injuries, including a major fracture of at least one extremity or of the pelvic girdle. The severity of injuries was classified according to the Modified Injury Severity Scale (MISS) and compared severity of injuries with residual impariment. The results were obtained as follows. 1. Of 75 injured children, boys were 47 and 33 of the children were 6 to 10 years old. 2. Cause of injury of 66 children was motor-Vehicle related accident. 3. Most frequently combined injury with muosulosgkeletal injury was neural injury. 4. There were 133 fractures in 75 children and femoral fracture was most frequent. 5. The mean score on the MISS of 75 children was 21. Fifty-three ehildren had a score of 25 points or less. 6. Twenty-eight children had residual impairment, and their mean on the MISS was 25. 7. Most of the musculoskeletal impairment were attributable to the nature of initial injury.

To assess the effectiveness of Magnetic Resonance Imaging (MRI) with endorectal surface coil in the ataging of carcinoma of the uterine cervix with emphasis on parametrial involvement. Thirty women with clinically and radiographically proven carcinoma of the uterine cervix were initially included for this study, but thirteen patients were excluded since the stages of tumors wore beyond stage IIa. CT and MR findings of the remaining seventeen patients were performed at Cheil General Hospital and compared a1ong with clinical findings with the special emphasis on the parametrial involvement by the tumor. Staging was assessed by CT and MRI, and the results were compared with the pathologic staging. Radiea1 abdominal hysterectomy with the pelvic and paraaortic lymphnode dissection was done to all sewenteen patients. The staging made primarily by CT and MRI was either stsge 1 or Ila, but MR images with endorectal surface coil was superior to CT in the visualization of depth of tumor infiltration, especially parametrial involvement. The determination of the depth of the tumor made by MR images showed statistically significant correlation with histologic evaluation(R =0.768, p<0.01). The accuracy rate for the evaluation of the parametrial invo1vement was 82.3% far CT and 94.1% for MRI with endorectal surface coil. The overall accuracy rate for tumor staging was 70.5% for clinical, 58.8% for CT and 82.3% for MR evaluation. The accurecy rate in evaluation of the pelvic and paraaortic lymphnode was 88.2% for CT, but the evaluation done by MRl was not adequate due to small FOV(field of view). In assessment of The steging of careinoma of the uterine cervlx, MR images with endorectal surface coil was superior to CT, especially in the evaluation of the parametrial involvement.
