1.Juvenile Posterior Bony Spur of the Lumbar Spine.

The bony spur formation without degenerative change in the lumbar spines in the young age group is quite interesting and not uncommon. This bony spur is different from the degenerative spur in the old age group in shape and mechanism of formation. The authors analyzed 111 patients with such posterior lumbar bony spur for recent 3 years and 9 months. The authors concluded that posterioly located Schmorl's node is a main cause of the bony spur without degenerative change at lumbar region. The authors advocated 'Juvenile spur' as a new disease entity.

2.Digital Infrared Thermographic Imaging(D.I.T.I.) in Herniated Lumbar Disc Patients.

Thermography is a sensitive and reliable method for diagnosis of radiculopathy. Skin temperature alterations of the involved dermatome named thermatone are diagnostic for sensory root involvement in radiculopathy which can be demonstrated by thermography. Digital infrared thermographic imaging system using computer is development and could measure thermal difference more exactly without hazards or discomforts to patient. Authors present 186 cases of thermographic evaluation in herniated lumbar disc disease by digital infrared thermographic imging system and the results are evaluated with literature review.

4.Screening of the Presence of Hepatitis B and C Virus Infections in Terminally Failing Human Hearts.

In order to evaluate the prevalence of hepatitis B virus (HBV) and hepatitis C virus (HCV) infections in terminally failing hearts, we screened the explanted hearts of transplantation recipients for the presence of HBV DNA and HCV RNA. DNA and RNA extractions were taken from explanted failing hearts (N=7) and normal hearts (N=). Polymerase chain reaction (PCR) and in situ PCR of HBV or reverse transcription-polymerase chain reaction (RT-PCR) and in situ RT-PCR of HCV were performed. The positivity of HBV in failing hearts was 63% (17/27) and it was 50% (3/6) in normal hearts in PCR. There was no significant difference in the positivity of HBV DNA between failing and normal hearts. The positivity of HCV in failing hearts was 18.5% (5/27) and it was 16.7% (1/6) in normal hearts in nested RT-PCR. There was no significant difference in the positivity of HCV RNA between failing and normal hearts. HCV was very rarely observed in explanted terminally failing hearts and HBV was frequently found in both explanted failing hearts and normal hearts. We concluded that these viruses have little direct causal relationship with the development of heart failure.

5.A Case of Subcorneal Pustular Dermatosis.

Subcorneal pustular dermatosis is a chronic, benign, relapsing pustular eruptions characterized histologically by subcorneal pustules which contain abundant neutrophils. We report a case of subcorneal pustular dermatosis occured in a 71-year-old male with typical climcal lesions. Biopsy of a pustule showed a subcorneal cavity with numerous neutrophils, which were also scattered throughout the Malpighian layer below. The result of imrnunofluorescent studies was negative. His eruption is controlled by DDS (25-100mg daily) but the eruptions recur quickly on sessation of the drug.

8.A case of asynchromous bilateral nonfunctioning adrenal cortical carcinoma.

Bilateral nonfunctioning adrenal cortical carcinoma is an extremely rare disease with poor prognosis. We report a case of asynchronous bilateral nonfunctioning adrenal cotical carcinoma. The patient was a 55 year-old man who had complained of discomfort in the right upper quadrant and flank area. There was no endocrinological abnomaliries. Abdominal CT revealed about 9.5 x 7.0 cm sized right adrenal mass. Under the impression of nonfunctioning adrenal cortical carcinorna, right radical adrenalectomy was performed. After 1 year from operation, 5.0 x 7.0 x 6.0 cm sized new mass was detected in the left adrenal gland with metastasis to the peritoneum.
