Isolated situs in versus of the stomach with otherwise normal position of the thoracic and abdominal vescera isan extremely rare anomaly occurring in two distinct forms. Majority of cases are associated with eventration of the diaphragm and are reported as being confused with spontaneous pneumothorax of pyopneumothorax at base of the right lung. The right sided stomach may produce interesting and confusing changes in liver scan. We have experienced 2 cases of the isolated dextrogastria.

4.First Report of Two Colletotrichum Species Associated with Bitter Rot on Apple Fruit in Korea – C. fructicola and C. siamense.

Bitter rot caused by the fungal genus Colletotrichum is a well-known, common disease of apple and causes significant yield loss. In 2013, six fungal strains were isolated from Fuji apple fruits exhibiting symptoms of bitter rot from Andong, Korea. These strains were identified as Colletotrichum fructicola and C. siamense based on morphological characteristics and multilocus sequence analysis of the internal transcribed spacer rDNA, actin, calmodulin, chitin synthase, and glyceraldehyde-3-phosphate dehydrogenase Pathogenicity tests confirmed the involvement of C. fructicola and C. siamense in the development of disease symptoms on apple fruits. This is the first report of C. fructicola and C. siamense causing bitter rot on apple fruit in Korea.

Holoprosencephaly is a rare CNS developmental defect with midline cleavage of embryonal forebrain during the fourth week of fetal development characterized with s single ventricle. Holoprosencephaly is divided into three types by the degree of the brain cleavage : alobar, semilobar, and lobar. Depending on the degree of defected regions, there exist variable clinical symptoms and signs such as stillbirth, hypotonia, apnea, convulsion, delayed development and visual disturbance. It was frequently associated with the deformity of the brain as well as the midline facial structures. Advances in neuroimaging over the past decades have led us to a better understanding of the pathogenesis and the variability of holoprosencephaly. As a result, a lot of cases of holoprosencephaly with various clinical and pathologic findings have been reported. We experienced a case of isolated interhemisphere fusion of frontal lobes with intact septum pellucidum in a child with febrile status epilepticus. Is it a variant of holoprosencephaly?

Audiant bone conductor(ABC) can be beneficial to patients with conductive hearing loss and sensorineural hearing loss. We have implanted ABC for 3 patients with bilateral conductive hearing loss and 2 patients with unilateral profound sensorineural hearing loss. And we evaluated the hearing gain in these patients except 1 case who did not return for follow up. The preoperative average air conduction threshoulds at the three speech frequencies 500, 1000 and 2000Hz, SRT, SD scores were 48.3dB HL, 47.5dB HL, 95% respectively and 19.1dB HL, 20dB HL, 95% postoperatively in 2 patients with bilateral conductive hearing loss. The preoperative average air conduction threshoulds at the three speech frequencies 500, 1000 and 2000Hz, SRT, SD scores were no response, no response, 0% respectively and 42.5dB HL, 51.3dB HL, 60% postoperatively in 2 patients with sensorineural hearing loss.

8.Dizziness in Patients With Obstructive Sleep Apnea

Obstructive sleep apnea (OSA) and dizziness are common conditions observed in the general population, and several epidemiological studies have reported an association between OSA and dizziness. Vestibular dysfunction, autonomic instability, and cerebellar degeneration secondary to recurrent hypoxia are implicated as mechanisms underlying dizziness in patients with OSA. Moreover, OSA is a risk factor for many diseases associated with dizziness, including Meniere disease, stroke, and psychiatric conditions. A dizziness questionnaire, vestibular function tests, and tests for autonomic function are useful for evaluation of OSA and concomitant dizziness. A growing body of evidence has shown that effective treatment of OSA including continuous positive airway pressure therapy reduces dizziness in these patients. Greater attention to dizziness is warranted in patients with OSA.

Blueberry muffin rashes occur in various diseases including TORCH syndrome, transfusion reactions, leukemia, hereditary spherocytosis and neonatal sepsis. We report a case of congenital CMV(cytomegalovirus) infection showing blueberry muffin skin lesions which revealed dermal erythropoiesis. Even though these cutaneous findings were nonspecific, they could provide a valuable clue in approach the congenital viral infection in the perinatal period.

10.Evaluation of the Augmentation Cystoplasty in the Pediatric Neurogenic Bladder.

From 1985 to 1995, 13 children with neurogenic bladder underwent augmentation cystoplasty. Diagnosis in these 13 children included Meningomyelocele in 11 and unknown origin in 2. Indications for augmentation cystoplasty were persists incontinence in 6 patients and progressive upper urinary tract deterioration in Bowel segments were used for augmentation included sigmoid colon in 1 patient, ileocecal segments incontinence in 6 patients stomach in 3. Dilated ureter also was used in 1 patient. Postoperatively, all the patients have stable upper tracts radiographically and stable or improved renal function. Of 13 patients 12 require clean intermittent catheterization to empty bladder and 10 are completely continent. After operation, bladder capacity increased from 155 ml to 305 ml and there were significant increases in bladder compliance. Hypereflexia was present in 5 patients before operation and in 3 patients after operation. There has been no surgical mortality. Postoperative complications occurred in 4 patients included mild ileus in 2 patients, mucus obstruction in 1, mild hematuria in 1, metabolic alkalosis in 1 and superficial wound infection in All complications were transient and managed medically. the kind of bowel segments did not seem to influence results. We think that augmentation cystoplasty is a therapeutic modality that should be considered as a viable treatment option in selected patients with neurogenic bladder dysfunction.
