OBJECTIVES: This study was to evaluate the cognitive and noncognitive effects of Psychodrama on elderly dementia patients. METHODS: The psychodrama treatment group was composed of 16 elderly dementia patients and control group was matched the psychodrama group. The psychodrama group received 8 session of psychodrama therapy every week. Cognitive function and non-cognitive function of two groups were evaluated by MMSE-KC, K-BNT, Geriatric Depression Scale, Barthel Activities of Daily Living, Self-Esteem Scale before and after Psychodrama. RESULTS: Improvement of Self esteem was observed in psychodrama group. Worsening of depression was observed in control group, but psychodrama group was not observed. CONCLUSION: Psychodrama with elderly dementia can be useful to enhance Self esteem and to prevent worsening of depression

The diagnosis of ovarian tumor has been mainly dependent on manual pelvic examination and ultrasonography. Butin case of malignant ovarian tumor, CT has more advantages over ultrasonography in assessing anatomic details,relationships to bowel loops, precise extents of tumors and follow-up examinations after surgery. Authors analyzedCT features of 46 cases of patholgocially proven ovarian tumors for recent 4 years at keimyung University DongsanHospital. The results were as follows: 1. The msot common tumor was serous cystadenocarcinoma(9 cases:20%),followed by metastases(8 cases: 17%), mucinous cystadenocarcinoma(7 ases:15%), mucinous cystadenocarcinoma(5cases:11%), teratoma(5 cases:11%), lymphoma(3 cases: 7%) and dysgerminoma(2 cases:4%). 2. The ovarian tumors werevariable in size from 2.5cm to 33cm in diameter. Most of the solid tumors were smaller than 10cm in diameter andmost of the cystic tumors were larger than 10cm in diameter. Usually mucinous tumors were much larger than seroustumors. Mucinous cystadenomas were the largest tumors. 3. Unilateral tumors(left 19,right 13 cases) were morecommon than bilateral tumors(12 cases). Bilateral tumors were seen in serous and mucinous cystadenocarcinoma,metastases and lymphoma. 4. CT features of mucinous cystadenomas were smooth margins and thin wall of the tumormasses and multiloculated cysts with internal septa in all 7 cases. 5. In contrast, CT demonstration ofbilaterality, irregular margin, thick wall, enhancing solid lesions, septal irregularity, adhesion to adjacentstructures, peritoneal/omental implantation, ascites and hydronephrosis were signs suggesting malignancy. CTfeaturs of the serous cystadenocarcinoma were mostly solid to mixed nature(83%), irregular margin(75%), enhancingsolid lesion(67%), papillary growth (75%), internal septa(58%), multilocularity (58%) and calcification (25%) indescending order of frequency. 6. On CT, mucinous cystadenocarcinoma were irregular-marginated, thick-walled,cystic tumors with enhancing solid lesion, septal irregularity and signs of metastasis, although there were somecases having similar features of benign. 7. Among the extrapelvic CT findings of malignant epithelial ovariantumor, peritoneal/omental implants(11 cases:79%) and ascites(10 cases:71%) were the most common, and indistinctuterus(6 cases:43%), bowel adehsion(5 cases:36%) and pseudomyxoma peritonei (2 cases) were descending order offrequency. 8. CT features of teratoma were diagnostic having at least three more of different tissue densitiesamong fat, water, soft tissue and calcific densities. Also there were thick wall (4 cases) and fat-fluid level(1case). 9. In 8 cases of ovarian metastases, there were solid type tumor smaller than 10cm in 4 cases and hugecystic in 2 cases. The site of primary cancer were stomach in 4 cases, colon , cervix, endometrium and unknown ineach one case. 10. In 3 cases of malignant lymphoma, the CT featurs were solid in appearance, smaller than 10cmand accompanied by lymphadenopathy in all cases.

We selected a patient with physiological bad breath from among the outpatients with bad breath that have visited the halitosis control clinic in KUMC. We identified the factors that were associated with the development of bad breath and assessed the occurrence of discomfort in the mouth; thereafter, we were able to prescribe a treatment method for the patient. We determined the daily routine of the patient through a questionnaire and a diary of dietary life. From these analyses, we deduced the factors associated with her physiologic halitosis. We combined this information with the results of analysis of her oral and exhaled gas measurements, nasal gas measurements by oral malodor measuring devices, and salivary measurements to develop a treatment method that we prescribed to the patient. We conclude that oral health education, including correction of the patient's mismanaged oral-care methods and routines, could act as an alternative treatment method for patients with physiologic halitosis in Korea.

Interdigitating dendritic cell sarcoma (IDCS) is a very rare and aggressive neoplasm that arises from antigen presenting cells. IDCS usually involves lymph nodes; however, extra-nodal involvement has also been reported. Because a consistent standard therapy for IDCS has not been established to date, we report a case of the successful treatment of disseminated IDCS using ABVD chemotherapy (doxorubicin, bleomycin, vinblastine, and dacarbazine). A 64-year-old man was diagnosed with IDCS on the basis of immunohistochemical findings of a biopsy specimen of the inferior nasal concha. Immunohistochemical staining showed a positive reaction for CD68, leukocyte common antigen, and S-100 protein, but a negative reaction for CD34, CD1a, and CD21. Imaging studies showed cervical and axillary lymphadenopathies, subcutaneous nodules, and a soft tissue lesion in the nasal cavity. Treatment with the ABVD regimen resulted in complete remission after 8 cycles of chemotherapy.

BACKGROUND: High-dose melphalan (200 mg/m2) with autologous stem cell transplantation (ASCT) is the standard treatment for young patients with multiple myeloma (MM). However, the response rates after ASCT are often unsatisfactory. We performed a pilot study by using bortezomib-melphalan as conditioning regimen for ASCT in Korean patients with MM. METHODS: The conditioning regimen consisted of administration of intravenous infusion of bortezomib 1.0 mg/m2 on days -4 and -1 and melphalan 50 mg/m2 (day -4) and 150 mg/m2 (day -1). In this study, we enrolled 6 newly diagnosed patients and 2 patients with relapse. RESULTS: The disease status of the 6 newly diagnosed patients at ASCT was as follows: 1 complete remission (CR), 1 very good partial remission (VGPR), and 4 partial remissions (PRs). The disease status of the 2 relapsed patients at ASCT was PR. All patients except 1 showed adequate hematologic recovery after ASCT. The median time for the absolute neutrophil counts to increase over 500/mm3 was 13 days (range, 10-19 days). Six patients with VGPR or PR at the time of transplantation showed an improvement in response to CR after ASCT. The patients were followed up without any maintenance treatment after ASCT except 1 patient who died during ASCT. During the follow-up period, CR was maintained in 3 newly diagnosed patients, but the other 4 patients, including 2 newly diagnosed patients, relapsed. CONCLUSION: Conditioning regimen consisting of bortezomib and melphalan may be effective for ASCT in MM; however, the feasibility of this regimen should be further evaluated in large study populations.

Dental caries and periodontal disease are considered to be chronic, but can be prevented through an incremental oral health program covering all ages. The National Oral Health Program for adults provides oral health exam and scaling, and is covered by national health insurance for those over 20 years of age in Korea. The aim of this study was to collect basic data for developing an oral health program for adults by identifying factors related to awareness and need. The data were obtained by convenience sampling of 303 subjects. The use of dental plaque disclosing agents affected tooth brushing frequency, toothbrushing time and use of oral auxiliary devices. Education on toothbrushing methods affected toothbrushing time and use of oral auxiliary devices. Of those surveyed, 93.1% replied that an incremental oral health program for adults was needed, and 68.0% intended to participate. In a regression model, the factors that had an effect on the perceived need for an oral health program were education level, use of oral hygiene auxiliary devices, and toothbrushing time, and the factors affecting intent to participate were education for prevention of periodontal disease and the use of oral hygiene auxiliary devices. The subjects stated that the following oral health programs were needed: an oral bacteria exam (74.3%), toothbrushing education (71.6%), a bad breath exam (69.3%), education on use of oral hygiene auxiliary devices (46.9%), a dental plaque exam (42.9%) and a saliva exam (37.6%). Oral health education appears to be an important factor for participation in an incremental oral health program.

Atypical chronic myeloid leukemia (aCML) is a rare leukemic disorder that shows myelodysplastic and myeloproliferative features simultaneously. The Janus kinase 2 gene V617F mutation (JAK2V617F) in aCML has been the source of much controversy. Some JAK2V617F positive cases have been reported but others observed no JAK2V617F mutation in aCML as defined by WHO classification. Recently, we experienced a case of aCML with JAK2V617F mutation with typical myelodysplastic/myeloproliferative features in peripheral blood and bone marrow aspirates. The karyotype was normal and no BCR/ABL1, PDGFRA or PDGFRB gene rearrangement was noted with FISH analysis. JAK2V617F mutation of the case was identified with amplification refractory mutation system PCR and direct sequencing. We also studied JAK2V617F mutation status in 3 additional cases of previously diagnosed aCML in our institution, but no mutation was identified.

Malignant melanoma is a highly metastatic tumor, resistant to chemotherapy and radiotherapy. Recent studies have suggested that many therapeutic agents used against cancer mediate their effects by induction of apoptosis of the cancer cells. Eugenol enhances the generation of tissue-damaging free radicals and inflammation or allergic reactions. In particular, it is more cytotoxic against cancer cells compared with normal fibroblasts. This study was performed to investigate whether the cytotoxic effect of eugenol is associated with the induction of apoptosis and involves activation of caspase in the human melanoma G361 cells. Eugenol-induced apoptosis was confirmed by MTT assay, Hemacolor stain, Hoechst stain, DNA electrophoresis, and Western blot analysis. Eugenol had a significant dose- and time-dependent inhibitory effect on the viability of G361 cells. Eugenol treatment induced caspase-3 and -6 cleavage, and activation. The caspase-3 substrates PARP and DFF45 are cleaved during eugenol-induced apoptosis. It was found that the casapase-6 substrate lamin A was cleaved, whose cleavage has been reported to be necessary for complete condesation of DNA during apoptosis. These results suggest that eugenol may constitute a potential antitumor compound against melanoma occurring in the skin and oral mucosa.
