1.Clinial Study of Nevocellular Nevus in Korea.

Nevocellular nevi are benign tumors composed of nevus cells, which are peculiar to man and almost everyone has a few nevi somewhere on his body. The dermatologist has a unique opportunity to study the most common neoplaam of man, the nevocellular nevus, becasue of its ready accessibility to visual observation and its simplicity obtaining for microscopic specimen. Much has been leamed last two decades about the nature, origin, and development of the nevocellular nevus. However, most of the these knowledge obtained by investigation of Caucasians. The investigation of colored subjects are scarcely reported. This paper is an attempt to clarify 1) the clinical features of nevocellular nevi developed on Korean, colored race. 2) the incidence of pigmented nevi developed on normal individuals and vitiligo patients. During the last l0 month(1975. 3. - 1975. 12. ) 525 normal individuals and 67 vitiligo patients were randomly selected and were investigated about incidence and clinical morphology(according to Levers clinical classification) of pigmented nevi developed on different body areas. The results were summarized as follows. 1) There was average 20 nevocellular nevi in each Korean individuals. Average 23 nevi was found in male and 15 nevi in female. 2) The number of nevus and its morphology were closely related to the age of host and body areas. 3) The flat type nevi was the most common type nevi and face was the most common site among the body areas. 4) In comparison with Caucasian, it seemed no racial difference in clinical features of the pigmented nevus. 5) There was no difference between the incidence of nevocellular nevus developed on normal individual and that of vitiligo patient.

2.Tibial Tunnel enlargement Following Arthroscopic ACL Reconstruction.

The roentgenograms following arthroscopic ACL reconstruction show the enlargement of bone tunnels. Many authors hypothesized the cause of the tunnel enlargement, either mechanical or biological causes. The purpose of this study was to find the factors which affected the enlargement of the tibial tunnel following arthroscopic ACL reconstruction with bone-patellar tendon-bone or hamstring tendon. Sixty patients were reviewed retrospectively for radiographic measurement of tibial tunnel at post-operative one year (27 patients received bone-patellar tendon-bone autograft, 12 patients received bone-patellar tendon-bone autograft and Kennedy LAD-ligament augmentation device, 21 patients received Semitendinosus and Gracilis tendons with Endobutton). Roentgenographic anteroposterior and lateral films were checked and the tunnel was measured by two independent observers using a digital caliper. Statistical analysis was performed using a one-way analysis of variance (ANOVA) and t-test. The tibial tunnel enlargement was only related to the position of the fixation of the tibial tunnel. We concluded that tibial tunnel enlargement following arthroscopic ACL reconstruction is attributed to the mechanical effect rather than the properties of grafts and the clinical results.

3.Arthroscopic ACL Reconstruction with Quadrupled Semitendinosus Tendon and endobutton.

In anterior cruciate ligament (ACL) deficient knee, several treatment methods were recommended. Recently, arthroscopic reconstruction is widely used as the best treatment method. As the graft materials, bone-patellar tendon-bone (BPB) unit is a gold standard material. Whereas, it has many problems. The purpose of this study was to introduce the new arthroscopic ACL reconstruction technique with quadrupled semitendinosus tendon and Endobutton and to evaluate its results. Twenty one patients were prospectively reviewed after ACL reconstruction with quadrupled semitendinosus tendon and Endobutton at one year post-operation. The surgery was performed in the series of graft tendon harvest, graft preparation, tunnel drilling and Endobutton fixation. The outcomes showed improved knee functions. Endobutton technique with quadrupled semitendinosus tendon is useful method for the arthroscopic ACL reconstruction. Stability and functional outcomes were similar to those with patellar tendon but it showed less donor site morbidity, less pitfall of fixation and simple procedure.

4.PCL Substituting Total Knee Replacement Arthroplasty.

Controversies about the posterior cruciate ligament continue with regards to total knee arthroplasty. There has been a long debate regarding PLC and its importance seems to be overrated in total knee arthroplasty. Twenty four patients had twenty six total knee replacements with insertion of the PCL substituting Press Fit Condylar modular total knee system with cement, and they were enrolled in a prospective study to assess the clinicoradiological results and measure the femoral roll-back postoperatively. Follow-up ranged from twenty four months to fifty months with an average of thirty two months. The mean HSS score was 92.1 points and the mean range of motion was 122.7 degrees. Twenty five cases(96.1%) showed excellent and good clinical results. The radiologic evaluation based on Knee Society roentgenographic system revealed proper alignment of all components and one significant radiolucent area. The mean femoral roll-back was 2.3mm posteriorly. In the present series, total knee arthroplasties with the Press Fit Condylar modular knee system resulted in excellent relief of pain, excellent range of motion and restoration of function.

5.Two Cases of Pseudocyst of Auricle.

We present two cases of pseudocyst of auricle which occured in 59-year-old man and 21-year-old man. Each showed bean sized non-inflammatory cystic swelling of upper part of right auricle with slight tenderness and the cystic content was serosanguinous fluid. Cystic wall was composed of fibrovascular tissue replacing degenerated cartilage without lining epithelium.

6.A Case of Mixed Tumor.

Mixed tumor can be defined as arising from the skin, often from sweat glands, and as being composed of epithelial elements intermingled with cartilage or myxoid tissue in such a way that epithelial cells appear to be continuous with these otber types of cells without separation by a basement membrane. RecentIy, we had opportunity to observe a patient, 34-year-old woman, who had typical mixed tumor of the skin in the central portion of the chin. The tumor developed as a solitary, firm, rice-sized subcutaneous nodule which had been present for 6 months with little increase in size and without any pain and tendemess. The covering skin was normal except mild erythema. Clinically, the lesion was simiIar to sebaceaus cyst, Under local anesthesia, tbe whole tumor mass was removed surgically for histologic study. The histopathologic finding reveals nests of cuboidal or polygonal cells, tubuloalveolar and ductal structures, and homogeneous, faintly bluish chondroid or foamy appearing matrix. 8.A Study on Plasma Levels of Testosterone and Dehydroepiandrosterone - sulfate ( DHEA - S ) in the Patients with Acne Vulgaris.

This study was performed to detect the hormonal abnormalities and to investigate the relationship between the levels of plasma testosterone and DHEA-S in 21 female patients with acne vulgaris using radioimmunoassay. The results were as follows : 1. The levels of plasma testosterone in patients group and control group(n=9) were 352+/-11.8 ng/dl, 223+/-9.7 ng/dl, respectively, and significantly elevated in patient group(p < 0.01), 2. The levels of plasma DHEA-S in patient group and control group were 286.6+/-126.8 pg/dl, 238.6+/-60.0 pg/dl, respectively, and did not show statistical significance. 3. The levels of plasma testosterones in patient subgroups classified by modified Pillsbury method were as follows minor group 32.6+5.3 ng/dl, mild group 33.4+/-15,5 ng/dl, moderate group 39.6+/-12.6 ng/dl. The testosterone levels in all the subgroups increased significantly than those in control group(352-t11.8 ng/dl) (p<0.05, p<0.05 p < 0.01) 4. The levels of plasrna DHEA-S in patient subgroups were follows minor group 242.5+/-412 pg/dl, mild group 263.9+/-166.1 pg/dl, moderate group 353.4+/-1273 pg/dl. The DE3EA-S levels in moderate group increased significantly than those in cotrol (238,6+/-60,0 pg/dl) and minor group(p<0.05). 5. There was no correlation between the plasma testosterone levels and the plasma DHEA-S levels in the acne patients.

9.Metastatic Adenocarcinoma Histologically Mimicking Fibrohistiocytic Tumors.

Cutaneous metastasis from visceral malignancies shows diverse manifestations. Very rarely, cutaneous metastasis mimicks benign mesenchymal tumors. We describe a case in which metastasis from gastric adenocarcinoma mimicked fibrohistiocytic tumors. The diagnosis of cutaneous metastasis was confirmed by the presence of atypical cells and signet-ring cells, positive staining for mucin and positive immunohistochemical stainings for keratins, carcinoembryonic antigen(CEA), and lysozyme within the tumor cells infiltrated in the irregular, intertwining bundles of collagen in the reticular dermis.
