2.Study on the Development of Nursing Information System about Drugs.

The purpose of this study was to develop an information system about drugs. The Joint Application Design Method which is popular in developing information systems was used. This system will be useful for the health care personnel and nursing students with easy access to information by using computer. The user requirements for this system were collected and analyzed through a survey of 50 nurses in a hospital and 30 students at a school of nursing. Based on this analysis, the specific contents were designed. The characteristics of this system as follows. Firstly it is the fast searching function which uses not only names of drugs(generic name, functional name, commercial name), but also names of diseases. Secondly, it is the function which maintains an updated database by the way of adding, updating and deleting the data. Thirdly, this system is useful for the education. In teaching and learning process, Computer-Assisted Instruction(CAI) is considered as one of the outstanding method bacause the individual differences in learners are respected and thus educational effect in pharmacology is expected.

5.Correlation between Expression of c-erbB-2 Oncogene and Various Prognostic Factors in the Colorectal Carcinoma.

The c-erbB-2 oncogene, which is a new human proto-oncogene similar to EGFR structurally, generates a glycoprotein of tyrosine kinase family with a molecular weight of 185,000 To evaluate the prognostic significance of c-erbB-2 oncogene expression in colorectal carcinoma, We analysed 73 colorectal carcinomas in paraffin sections immunohistochemically, using the monoclonal antibody specific for the c-erbB-2 oncogene product and correlated with clinicopathological data. The results were as follows 1) The immunoreactivity for c-erbB-2 oncogene was localized to cell membrane of the tumor cells and occasionally observed within the cytoplasm. 2) The positivity of c-erbB-2 oncogene expression was 71.2%(52/73) of the colorectal carcinomas overall. According to the histological types, the positivity of c-erbB-2 oncogene in adenocarcinoma(77.4%) was higher than that in mucinous carcinoma(36.4%)(p<0.05). 3) Expression of c-erbB-2 oncogene was significantly correlated with lymph node metastasis or distant metastasis(p=0.0117), Dukes stage(p=0.0432), and TNM classification(p=0.0102). These results suggest that c-erbB-2 oncogene expression may be used as a prognostic factor of colorectal carcinoma because of its correlation with other clinicopathological prognostic factors.

6.Comparison between a Pediatric Health Promotion Center and a Pediatric Obesity Clinic in Detecting Metabolic Syndrome and Non-Alcoholic Fatty Liver Disease in Children.

This study was done to evaluate the efficacy of health check-ups in children in detecting metabolic syndrome and non-alcoholic fatty liver disease (NAFLD) by comparing the pediatric health promotion center with the pediatric obesity clinic. Children who visited a pediatric health promotion center (n=218) or a pediatric obesity clinic (n=178) were included. Anthropometric data, blood pressure, laboratory tests, and abdominal ultrasonography were evaluated. Two different criteria were applied to diagnose metabolic syndrome. The prevalence of metabolic syndrome in the 2 units was 3.2%-3.7% in a pediatric health promotion center and 23%-33.2% in a pediatric obesity clinic. Significant differences were observed in the prevalence of each component of metabolic syndrome between the 2 units including abdominal adiposity, blood pressure, serum triglycerides, and fasting blood glucose (P<0.05). The prevalence of NAFLD was 8.7% and 71.9% in the 2 units according to liver enzymes and 5.9% and 61.8% according to ultrasonography (P<0.05). The prevalence of metabolic syndrome and NAFLD was higher among patients visiting the obesity clinic targeting obese children than that among patients visiting the health promotion center offering routine check-ups. An obesity-oriented approach is required to prevent obesity-related health problems in children.

7.Antiphospholipid Syndrome Presented with Pulmonary Embolism and Deep Venous Thrombosis in Child.

Antiphospholipid syndrome is a thrombotic disorder and the serologic marker of the syndrome is antiphospholipid antibody(lupus anticoagulant, anticardiolpin antibody, or both). In a 13-year-old girl who presented with dyspnea, pulmonary embolism and femoral vein thrombosis were demonstrated by lung scan and abdominal ultrasonography. She had 3 out of 11 criteria for the diagnosis of SLE, such as thrombocytopenia, positive antinuclear antibody, low C3 and high anti-ds-DNA Ab, but did not meet whole criteria. And lupus anticoagulant was positive. She was treated with anticoagulants(heparin and wafarin) and prednisolone. Antiphospholipid antibody(lupus anticoagulant and anticardiolipin antibody) should be checked in children with thrombosis occurring without a known predisposing cause.

8.Periodontal response following application of intrusive forces to the incisors with bone loss and periodontal disease in adult dogs.

While periodontal disease results in the extrusion of upper incisors with interdental spacing in adult patient, the intrusive movement should be executed for the rehabilitation in terms of esthetics and function. The present study was performed to investigate the periodontal response following orthodontic intrusion of teeth with infrabony pocket. Bone defect and periodontal disease were produced adjacent to the both maxillary second lateral incisors in four adult dogs. Four weeks later, a flap operation was performed to eliminate inflammation and a reference notch was made on the root surface at the level of bottom of each defect. Two weeks after periodontal surgery, 4 weeks of intrusion and subsequent another 4 weeks of retention was executed on the right side of incisors while left incisors served as the control. Through the histologic analysis, following results were obtained. 1. Histologic section of control side showed the reduction of pocket depth after flap operation. However, the status of long junctional epithelium was observed while new connective tissue attachment occurred in a small area adjacent newly formed cementum. 2. Epithelial attachment was less around the intruded incisor while new connective tissue attachment was much more compared to the control side. 3. Plenty of cells were observed in the connective tissue around the reference notch of the intruded incisor and the degree of the new cementum fonnation was higher in the intruded incisor than the control. The results of the present study suggested that an orthodontic intrusion of periodontally involved and extruded incisors could be carried out with a desirable periodontal response, provided that inflammation is under control.

9.The Value of Transvaginal Ultrasonographic Measurement of Cervical Length for Prediction of Preterm Delivery in Patients with Preterm Labor.

OBJECTIVE: The aim of this study is to evaluate the value of vaginal ultrasonographic cervical length in prediction of preterm delivery in patients with preterm labor. METHODS: One-hundred twenty-nine women in preterm labor at 24-34 weeks of gestation were studied retrospectively. Transvaginal ultrasonography for measurement of cervical length was performed at the time of admission. Receiver-operating characteristic (ROC) curve analysis, partial correlation coefficient, and multiple logistic regression analysis were used for statistical analysis. RESULTS: The mean gestational age at admission was 30.2 +/- 2.5 weeks and the mean gestational age at delivery was 35.7 +/- 3.9 weeks. The median admission-to-delivery interval was 864 hours (24-2616). ROC curve analysis showed a significant relationship between cervical length and preterm delivery (area under the curve=0.82, p<0.001). The diagnostic indices of cervical length (cut-off value 2.5 cm) by ROC curve were sensitivity of 86%, specificity of 64%, and positive and negative predictive value of 54% and 90%. Multiple logistic regression analysis indicated that cervical length was an independent predictor of preterm delivery after adjustment of confounding factors (x2=0.326, p<0.01). Admission-to-delivery interval in patients with cervical length >or=2.5 cm was significantly shorter than that in patients with cervical length >2.5 cm. CONCLUSION: Transvaginal ultrasonographic measurement of cervical length is a useful predictor of preterm delivery in patients with preterm labor and intact membranes.

10.Protective Effects of Curcumin on Renal Oxidative Stress and Lipid Metabolism in a Rat Model of Type 2 Diabetic Nephropathy.

PURPOSE: Diabetic nephropathy is a serious complication of type 2 diabetes mellitus, and delaying the development of diabetic nephropathy in patients with diabetes mellitus is very important. In this study, we investigated inflammation, oxidative stress, and lipid metabolism to assess whether curcumin ameliorates diabetic nephropathy. MATERIALS AND METHODS: Animals were divided into three groups: Long-Evans-Tokushima-Otsuka rats for normal controls, Otsuka-Long-Evans-Tokushima Fatty (OLETF) rats for the diabetic group, and curcumin-treated (100 mg/kg/day) OLETF rats. We measured body and epididymal fat weights, and examined plasma glucose, adiponectin, and lipid profiles at 45 weeks. To confirm renal damage, we measured albumin-creatinine ratio, superoxide dismutase (SOD), and malondialdehyde (MDA) in urine samples. Glomerular basement membrane thickness and slit pore density were evaluated in the renal cortex tissue of rats. Furthermore, we conducted adenosine monophosphate-activated protein kinase (AMPK) signaling and oxidative stress-related nuclear factor (erythroid-derived 2)-like 2 (Nrf2) signaling to investigate mechanisms of lipotoxicity in kidneys. RESULTS: Curcumin ameliorated albuminuria, pathophysiologic changes on the glomerulus, urinary MDA, and urinary SOD related with elevated Nrf2 signaling, as well as serum lipid-related index and ectopic lipid accumulation through activation of AMPK signaling. CONCLUSION: Collectively, these findings indicate that curcumin exerts renoprotective effects by inhibiting renal lipid accumulation and oxidative stress through AMPK and Nrf2 signaling pathway.
