BACKGROUND/PURPOSE: Phospholipase A (PLAq) is an rate-limiting enzyme hydrolyzing arachidonic acid from the sn-2 position of membrane phospholipids. In vitro studies demonstrated that the enzyme could be secreted into extracellular mileu by pro-inflammatory cytokines and endotoxin which were reported to have important roles in chronic liver diseases. This study was performed to know whether the enzyme is involved in the pathophysiology of the diseases. METHODS: The subjects were composed of 24 patients with chronic hepatitis B, 26 patients with liver cirrhosis and 14 healthy individuals. The PLAp activities wem measured in the sera of the subjects by detecting radioactivity of "C-fatty acid hydrolyzed from "C-labeled phosphotadylethanolamine by the enzyme. RESULTS: The activities of PLA were increased in the patients with chronic liver diseases, especially in the chronic hepatitis B patients with acute exacerbation and in the decompensated cirrhosis patients. Furthermore, their activities were closely related with the levels of transaminase in hepatitis group and with the levels of serum albumin in cirrhosis group, respectively. CONCLUSIONS: These results suggest that extracellular PLA might be involved in the exacerbution and progression of the chronic liver diseases.

Microsatellites are short nucleotide repeat sequences present throughout the human genome. Alterations of microsatellites, comprising extra or missing copies of these se quences, have been termed microsatellite instability(MSI, genetic instability, replication errors, RER(+) phenotype). To date, at least four genes involved in DNA mismatch repair, hMSH2, hMLH1, hPMS1 and hPMS2, are thought to account for the observation of microsatellite instability in tumor from Hereditary nonpolyposis colorectal cancer (HNPCC) patients. The genetic defect responsible for the MIN+ phenotype in sporadic colorectal cancer, however, has yet to be clearly delineated. The purpose of this study was to determine the presence of MSI in sporadic cancer and to correlate its occurrence with clinicopathological parameters, we have studied six microsatellite loci by use of polymerase chain reaction amplification and denaturing polyacrylamide gel electrophoresis. We found that 20%(9 of 46 cases) sporadic colorectal cancers showed RER at two or several loci(RER+). Microsatellite instability was associated with location of the tumor in the proximal colon 66%(6 of 9 cases) and with poorly differentiated tumor phenotype 56%(5 of 9 cases). In order to better understand the role of somatic alterations within hMSH2 in the process of colorectal tumorigenesis, we examined the most conserved regions(codon 598~789) of this gene in nine patients with MIN spotadic colorectal cancer. 6 patient of RER(+) colorectal ca. patients had a polymorphism which was a T to C base change in the intron sequence at -6 position of the splice acceptor site at the 5'end of exon 13. This particular sequence variation is a polymorphism rather than a mutation which increase cancer susceptability. These data suggest that the genetic instability is detect ed in some colorectal cancers and play an important role in the pathogenesis of sporadic colorectal cancer.

There were 18 woman with mild to moderate stress incontinence treated with the Sympathomimetic agents, with were combined with phenylpropanolamine hydrochloride, chlorpheniramine maleate, and isopropamide. The majority of these patients achieved significant improvement for 2 months to 2 and half years after beginning therapy. These patients were performed physical examination, voiding cystourethrogram, stress test lateral-chain cystourethrogram, cystoscopy in order to confirm stress incontinence. We thinked that these conservative therapy were advisable for stress incontinence especially when symptoms were mild.

De Lange Syndrome*

Solid and papillary cystic neoplasm of pancreas is an uncommon low grade malignant tumor found predominantly in young female in their second or third decade of life, and amenable cure by surgical treatment. The authors report two cases of solid and papillary neoplasm of pancreas pathologically verified at Kyung Hee university hospital. The first case was 11-years old male patient and the other case was 12-years old male patient. Symptoms of two patients were abdominal discomfort, nausea and vomiting and abdominal pain in the female patient. CT finding of solid and papillary neoplasm of pancreas depict a well-demarcated mass with solid and cystic necrosis component. In female patient, large hematoma was shown. Gross findings of tumor revealed apparent encapsulation, cystic degeneration and hemorrhagic necrosis. Microscopically the tumors were characterized by distinctive solid and papillary patterns of cellular arrangement without local invasion. All patients were discharged and follow up without any problem.
