1.A Study on the Clinical and Histopathologic Classification of Leprosy.

For comparison of clinical classification of leprosy to histopathologic classification, a detailed histopathologic study, using hematoxylin and eosin stain and Ziehl-Neelsen stain, was done on 72 fresh uncomplicated cases of leprosy. The clinical classification was done using the criteria of Ridley and Jopling (1966), and the microscopic features were classified according to Ridley's(1974) definition. Clinical classification revealed that 8 of total 72 patients had tuberculoid(TT), 9 had borderline tuberculoid (BT), 5 had borderline(BB), 10 had borderline lepromatous.(BL), and 31 had lepromatous leprosy(LL). Nine patients were claasified as indeterminate(I) group. Histopathologic classification showed that 3 cases presented tuberculoid(TT), 10 presented borderline tuberculoid(BT), 4 presented borderline(BB), 9 presented borderline lepromatous(BL), 20 presented subpolar leprornatous(LLs), and 10 presented polar lepromatous(LLp) histopathologic characteristics, Sixteen cases were classified as indeterminate(I) leprosy by histopathologic findings. On comparison of clinical classification to histopathologic classification, the two were in consonance with each other in 50 cases(69.4%) and the disparity between them was noticed in 22 cases(30.6%). Among the 22 cases which showed disparity, there was a shift of one step either towards the tuberculoid or lepromatous end of the spectrum in 15 cases, and ihe remaining 7 cases were classified as indeterminate group beaause of nonspecific histopathologic changes.

2.A Case of Toxic Epiderma Nocrolysis.

Toxic epidermal necrolysis(T.E.N.) is a toxic erythema of the skin that undergoes epidermal necrolysis and peding so that it appears to have been scalded. There seem to be two basic form of T.E.N. In children, the deavage of the epidermis that results from an acantholytic-dyskeratotic cytolytic process is in the malpighian or granular layer and is usually induced by a staphylococci toxin. In adult, it is characterized by a full-thicknees cytolytic cleavage of skin above the basal layer and is usually precipitated by a drug. The authors observed a 17 year-old male with T.E.N. He began to notify the erythematous skin lesion with burning sensation on the whole body surface at 12 hours after oral administration of some drugs for common cold and developed the characteristic flaccid bullous and denuded lesions with tenderness within next 20 hours.

3.A Case of Paraneoplastic Acrokeratosis (Bazex' Syndrome).

We report a case of paraneoplastic acrokeratosis(Bazex syndrome) occurring in a 70 year old man with small cell carcinoma of the lung. The skin lesions showed violaceous erythema and psoriasiform scaling of the face, aural helices, scalp, and palmoplantar regions with severe nail dystrophy, Histcipathological findings revealed hyperkeratosis, acanthosis and spongiosis in the epiderrnis, and lyrnphocytic infiltration in the upper dermis. He was treated with cis-diamine dichloroplanum(Cisplan), 4 o-demethy-l-o-epipodo- phyllotoxin (Etoposide ) and steroid onitrnent. But eight months later, he died of lung carcinoma.

4.A Case of Angioimmunoblastic Lymphadenopathy with Dysproteinemia with Skin Nodules.

We report here a case of angioimmunoblastic lyinphadenopathy with dysprote inemia(AILD) in a 65-year-old male associated with skin nodules on the face. Physical examina.tion showed generalized lymphadenopathy, hepatosplenomegaly, erythematous maculopapular eruptions on the trunk and extremities, and er the matous nodules on the face. Lahoratory findings showed hemolytic anernia, arid polyclonal gammopathy. Histopathological findings of lymph node revealed typical features of AILD. I-iistopathological findings of maculopapules revealed lymphohistiocytic infiltra tion and extravasated erythrocytes around the blocked vessels, whereas histological picture of nodules on the face mirrorred that of the lymph node. After taking systemic chemotherapy, his general condition and skin lesions were improved.

5.A Case of Kyrle's Disease.

We report a case of Kyrles disease occuring in a 19-year-old man with chronic renal failure. The patient had multiple hyperkeratotic papules with central plug on the upper extremities, lower extremities and buttocks. Histopathological findings showed epidermal invagination filled with parakeratotic column and basophilic debris. Many dyskeratotic and vacuolated cells were present below the keratotic plug, and penetration of the parakeratotic column into the dermis was seen.

6.A Case of Nevus Lipomatosus Cutaneous Superficialis.

Nevus Iipomatosus cutaneous superficialis (NLCS) is a very rare skin disease which usually is present at birth. The lesion consists of grouped papules and nodules which on histological examination show the presence of ectopic fatty tissue in the corium. The authors observed a 17year old female with NLCS. She noticed soft papules and nodules on the coccygeal area since 8 years old. We confirmed this case by clinically and histopathological findings which showed the strands or islands of mature fatty tissue within papillay, mid and lower dermis.

7.Correlation Between Tumor Microvessel Density and Prognosis in Breast Cancer.

It has been recognized that angiogenesis has a profound effect on growth of tumors in various animal models. Recently, it was suggested that tumor microvessel density may be an independent prognostic factor in breast cancer and other human tumors. This study was designed to define the correlation between tumor microvessel density and survival in breast cancer. Pathological parameters of 60 patients with breast cancer were analyzed: stage, nodal status, histologic grade, vascular invasion, and p53 expression. Tumor microvessels were stained by the immunohistochemical method in formalin-fixed, paraffin-embedded tissues by using the anti-CD31 monoclonal antibody (JC/70A, DAKO) and were counted with in a 10 objective field (about 0.74 mm2) in the area of the most intense neovascularization. The microvessel density (MVD) of the tumor was compared with other prognostic factors and 5-year survival rate. The mean MVD of the 60 breast cancers was 36.5 15.3 (range: 17~112). The MVD did not correlate with the stage, the node metastasis, the histologic grade, the vascular invasion, or the p53 expression. The MVD was graded by the number of microvessels as grade 1 (MVD 17~32), grade 2 (MVD 33~48), and grade 3 (MVD>49). The 5-year survival rate of patients with MVD grade 1 was 69.0%, and those of grades 2 and 3 were 58.2% and 29.6%, respectively, which was statistically significant (p=0.004). By univariate analysis, the stage (p<0.001), the axillary nodal status (p<0.001), the MVD (p=0.004), and the p53 expression (p=0.041) had a significant correlation with survival. In 25 patients with node-negative breast cancer, the MVD was the single significant prognostic factor (p=0.047). These results suggest that tumor microvessel density is a significant prognostic factor in invasive breast cancer and that it may be a prognostic factor in node-negative breast cancer.

8.Combination Treatment of Osmidrosis by Liposuction and Endoscopic Shaver.

Many surgical procedures have been introduced for treatment of axillary osmidrosis, but some complications, such as symptom recurrence, hematoma, seroma, delayed wound healing, skin flap necrosis and scarring, are not solved yet. Recently introduced liposuction method has several advantages over previous surgical methods, such as increased skin flap vascularity, minimal scarring and bleeding, etc, but the sympton recurrence rate has been reported 10-30%. Liposuction method could remove apocrine glands in subcutaneous tissue effectively, but the glands firmly seated in subdermis remain as the cause of sympton recurrence. For this reason, we employed combination treatment of liposuction and endoscopic subdermal shaving method. From March 1997 to February 2001, a total 71 patients (24 men and 47 women) had been treated with this procedure for bilateral axillary osmidrosis. In our procedure, apocrine glands in subcutaneous tissue were removed by liposuction method and the glands in subdermis were removed by using 4.5 mm incisor blade of endoscopic shaver. Follow-up evaluation period was from 3months to 24 months, and the results were satisfactory. No one suffered from sympton recurrence. Postoperative scar was negligible (below 0.5 cm) and other complications were minimal.

9.Bronchiolitis Obliterans Associated with Stevens-Johnson Syndrome: A Case Report.

We report a case of bronchiolitis obliterans associated with Stevens-Johnson syndrome. A 59-year-old man presented with respiratory distress that gradually worsened over 3 months. He had been diagnosed with Stevens-Johnson syndrome 3 months before admission. He had no history of previous airway disease. On physical examination, expiratory breathing sounds were not audible, and a chest X-ray revealed a hyperinflated lung. A pulmonary function test indicated a severe obstructive pattern. Computed tomography scans of inspiratory and expiratory phases of respiration showed oligemia and air trapping, and both were more prominent on expiration view than on inspiration view. The pathogenesis of bronchiolitis obliterans associated with Stevens-Johnson syndrome is largely unknown.

10.High Glucose Accelerates Autophagy in Adult Rat Intervertebral Disc Cells.

STUDY DESIGN: In vitro cell culture. PURPOSE: The purpose of this study was to investigate the effect of high glucose on autophagy in adult rat intervertebral disc cells. OVERVIEW OF LITERATURE: Diabetes mellitus is considered to be an important etiologic factor for intervertebral disc degeneration, resulting in degenerative disc diseases. A glucose-mediated increase of autophagy is a major causative factor for the development of diseases associated with diabetes mellitus. However, no information is available for the effect of high glucose on autophagy in adult intervertebral disc cells. METHODS: Nucleus pulposus and annulus fibrosus cells were isolated from 24-week-old adult rats, cultured and placed in either 10% fetal bovine serum (normal control) or 10% fetal bovine serum plus two different high glucose concentrations (0.1 M and 0.2 M) (experimental conditions) for one and three days, respectively. The expressions of autophagy markers, such as beclin-1, light chain 3-I (LC3-I) and LC3-II, autophagy-related gene (Atg) 3, 5, 7 and 12, were identified and quantified. RESULTS: Two high glucoses significantly increased the expressions of beclin-1, LC3-II, Atg3, 5, 7, and 12 in adult rat nucleus pulposus and annulus fibrosus cells in a dose- and time-dependent manner. The ratio of LC3-II/LC3-I expression was also increased in a dose-respectively time-dependent manner. CONCLUSIONS: The results suggest that autophagy of adult nucleus pulposus and annulus fibrosus cells might be a potential mechanism for the intervertebral disc degeneration in adult patients with diabetes mellitus. Thus, the prevention of autophagy in adult intervertebral disc cells might be considered as a novel therapeutic target to prevent or to delay the intervertebral disc degeneration in adult patients with diabetes mellitus.
