Morquio's syndrome is a very rare disease, which is characterized by dwarfism, flattening of the vertebral body, marked spinal kyphosis, widespread affection of the epiphyses and normal intelligence. The authors experienced a case of Morquio's syndrome with typical clinical and radiological findings. The patient underwent bilateral supracondylar varus osteotomies for severe genu valgum deformities.

We reviewed thrity-one cases of unstable tibial fractures treated with Ilizarov method from January 1991 to December 1992. We attained satisfactory bony union in all cases without bone graft(Average time 19.2 weeks). There were numbers of complications, but its were soluble and careful management & numbers of minor surgery were needed to prevent & solve such complications. Post-op immediate weight bearing and ROM exercise were possible and showed no difficulty in getting mainteance of reduction & fracture healing, and serious joint ankylosis was not developed. In conclusions, Ilizarov method is an excellent treatment in getting reduction, maintenance of reduction, early ambulation and fracture healing in the cases of unstable tibia fractures.

The management of LcP disease is dependent on the prognosis yet. This may be difficult to predict for the individual child, particularly at the onset of the disease when radiological sign may be minimal. But some form of conservative treatment is generally commenced before the prognosis become apparent. The purpose of this study was to determine the validity of Catterall's grouping and value of other radiological and clinical factors in predictiong prognisis and also the correlation between the clinical and radiological results at the conclusion of the pathological precess in LCP disease. Between June 1986 and July 1995, 64 childrens with LCP disease were tearted in Kyungpook University Hospital, 44 hips of 40 childrens who had adequate clinical and radiological data were reviewed. The average follow-up period was 5.4 years. The results obtained were followings: 1.The mean age at first visit or diagnosis was 6.7 years. 2. Of 40 childrens, 4 childrens had bilateral involvement. Boys were more predominat. 3. The degree of uncovering of femoral head and medial joint space at the presentation seemed to be prognostic factor for the end results. 4. Catterall & lateral pillar classification seemed to be difficult at presentation of the disease but also an important guide to determine the prognosis.

BACKGROUND: Video-assisted thoracic surgical procedure via thoracoscopy has recently gained popularity, as it avoids a thoracotomy, reducing intraoperative blood loss, postoperative pain, respiratory dysfunction and hospital stay. However, to visualize adequately the intrathoracic structures, creation of artificial pneumothorax by carbon dioxide insufflation during thoracoscopy would cause significant hemodynamic compromise. The aim of this study was to evaluate the effect of CO2 insufflation into the pleural cavity on the hemodynamics and the arterial blood gas tension under general anesthesia. METHODS: Twenty-five patients, after intubation with single lumen endotracheal tube, underwent enflurane (1~2%) and N2O-O2 (1:1) general anesthesia. Before placement of a thoracoscope, the baseline mean arterial pressure and heart rate were obtained. Measurements were taken at 5, 10, and 20 min. after the beginning of carbon dioxide insufflation (3~5 mmHg) and 10 min. after gas evacuation. Blood gas analyses were done before, during CO2 insufflation and after CO2 evacuation. Data were analyzed using Student t-test. RESULTS: Positive-pressure CO2 insufflation (3~5 mmHg) caused a decrease of mean arterial pressure (5~7%) and an increase of airway pressure (1.5 times) and heart rate (13~20%) throughout the gas insufflation period (p<0.05). Blood gas analyses revealed no significant change. CONCLUSIONS: These results suggest that low CO2 insufflation pressures (3~5 mmHg) may cause cardiovascular depression during thoracoscopy. Therefore careful monitorings should be done during this procedure.

We observed clinically 27 cases of MCLS at this pediatric department from Jan. 1980 to Jun. 1984 and following result were obtained. 1) The peak incidence of these cases was from 1 year to 4 year of age(74%) and male children were affected more frequently than female children at a ratio of 2.4:1. 2) Clinical manifestation of MCLS were high fever, conjunctioval injection, changes of oral cavity and lips, erythematous rash, cervical lymphadenopathy, changes of the extremities. 3) The abnormal laboratory findings include leukocytosis, slight anemia, raised ESR, positive CRP, mild proteinuria or pyeuria, and slight increase of serum transaminase and LDH. 4) The abnormal cardiovascular findings were as follow : heart murmur in 15% cardiomegaly in 19% by chest roentgenogram, electrocardiographic abnormalities in 59%, suspicious aneurysmal dilatation and coronary aneurysm by 2-D echocardiogram in 11%. 5) Aspirin was given in acute febrile stage as antiinflammatory dose(100mg/kg/day), in non-febrile convalescent stage to maintain low dose of aspirin(20-30mg/kg/day) as antiplatelet effect, and completely recovered in all cases without andy complication.
