Isolated situs in versus of the stomach with otherwise normal position of the thoracic and abdominal vescera isan extremely rare anomaly occurring in two distinct forms. Majority of cases are associated with eventration of the diaphragm and are reported as being confused with spontaneous pneumothorax of pyopneumothorax at base of the right lung. The right sided stomach may produce interesting and confusing changes in liver scan. We have experienced 2 cases of the isolated dextrogastria.

Urinary tract infection (UTI) is common in patients with diabetes mellitus. Furthermore, UTI is more severe, more often caused by resistant pathogens, and produces worse outcomes in those with diabetes mellitus. Although some patients may have altered clinical signs, symptoms of UTI are similar in patients with or without diabetes mellitus. Treatment depends on severity of systemic symptoms, results of urine culture, and underlying diseases of patients. There are no definite indications to treat asymptomatic bacteriuria in patients with diabetes mellitus.

Nonalcoholic fatty liver disease (NAFLD) is the most common liver disorders and is associated with various metabolic diseases, including type 2 diabetes mellitus (T2DM). There are no approved drugs for NAFLD, and the only approved treatment option is weight reduction. However, it is not easy to maintain weight loss by lifestyle modification alone; pharmacological treatments are helpful in this regard. As insulin resistance plays an important role in the development of NAFLD, many antidiabetic drugs have been evaluated for treatment of NAFLD. Pioglitazone could be a first-line option to improve nonalcoholic steatohepatitis (NASH) in patients with T2DM to produce some improvement in fibrosis. Glucagon-like peptide-1 receptor agonists show evidence of improving NAFLD/NASH with fibrosis. Metformin and dipeptidyl peptidase-4 inhibitors are not recommended for treating NAFLD in patients with T2DM. Evidence that sodium-glucose cotransporter 2 inhibitors improve NAFLD/NASH with fibrosis in patients with T2DM is emerging.

Gestational diabetes mellitus (GDM) not only increases perinatal complications, but also increases lifetime risk of type 2 diabetes. Recently, many studies have shown that women with GDM also have an increased risk of cardiovascular disease (CVD). The risk of preeclampsia, hypertension, dyslipidemia, metabolic syndrome, atherosclerosis, and CVDs were elevated in women with GDM compared with in those without GDM. Although it was not confirmed whether the management of GDM could reduce the development of CVDs, it is important to identify and manage CVDs in women with GDM.
