1.PCR-RFLP patterns of four isolates of Trichinella for rDNA ITS1 region.

We have studied the genetic differences among four isolates of Trichinella including a new strain of Trichinella spiralis (ISS 623) recently found from a human case who took a badger in Korea. Because they have a different host origin and came from geographically separated regions, we supposed the genetic pattern of the isolates might be different as had been previously reported. It was analysed by PCR-RFLP analysis of the rDNA repeat that can readily distinguish a species or strain from others. Isolated genomic DNA of each isolate of Trichinella larvae was amplified with ITS1 specific primers and digested with restriction endonucleases. The PCR product of ITS1 was confirmed using Southern blot analysis to be a 910 bp fragment. The restriction fragments of each isolate had variable patterns when it was digested with Rsa 1 only. According to the RFLP patterns, the estimated genetic divergence between each isolate was different. In conclusion, four isolates of Trichinella including a new strain of T. spiralis obtained from a Korean patient may have genetic differences in the ITS1 region and the Shanghai isolate was genetically more similar to the Japanese unknown isolate than others in the ITS1 region.

2.Information-Stressors and Cancer Patients’ Quality of Life: Responses to Deviant Information-Stressors Due to Pre-Postoperative Stage Discordance

This study assessed preoperative quality of life (QoL) of gastric cancer patients exposed to inconsistent illness information by pre-post-operative stage discordance. The impact of information-stressors on patients’ QoL was investigated to determine information processing as a potential target for QoL management. Early-stage gastric cancer (EsGC) and late-stage gastric cancer (LsGC) groups based on their final stage were categorized by the consistency of preoperative staging information that was being shared. Those with consistent preoperative staging information were rated as EsGC (n=1,420) and LsGC (n=153) controls. EsGC and LsGC patients with misdirected information about their LsGC and EsGC were categorized as EsGC/iLsGC (n=32) and LsGC/iEsGC (n=55), respectively. Preoperative QoL data was obtained using EORTC QLQ-C30 and -STO22. QoL outcomes of EsGC/iLsGC and LsGC/iEsGC were compared with those of the EsGC and LsGC controls. QoL outcomes of the EsGC/iLsGC group matched that of EsGC control, but were significantly better than those of LsGC control on multiple scales including global health status/QoL, physical/role/social-functioning, and ten symptom scales/items. On the other hand, QoL outcomes of LsGC/iEsGC group were significantly better than those of LsGC control on multiple scales (global health status/ QoL, physical/role-functioning, and nine symptom scales/items) while they roughly matched with those of EsGC control. Intensified information-stressors did not exacerbate QoL beyond the influence of the patients’ medical condition, while de-intensified information-stressor improved QoL. Fear of negatively impacting QoL should not prevent the sharing of stressful illness information. As the de-intensified information- stressor improves QoL, information processing is recommended as a potential target for QoL management in cancer patients.
3.Comparison of Nutritional Status According to Serum C-reactive Protein Concentration in Hemodialysis Patients.

Serum C-reactive protein(sCRP) is an acute-phase reactant that exhibiting negative correlation with serum albumin concentration. It was reported that sCRP is an independent predictor of survival in both hemodialysis and peritoneal dialysis patients, and an acute phase inflammation could be preceded by protein catabolism, hypoalbuminemia, anorexia and even atherosclerotic cardiovascular disease. We have evaluated serum biochemical parameters including albumin and prealbumin, Kt/Vurea, nPCR, SGA score, anthropometric parameters and diet history in 30 ESRD patients maintained on chronic hemodialysis subdivided by sCRP concentration. Upon comparing the two subgroups[high CRP group(sCRP >or= 0.4mg/dL), n=15 vs. normal CRP group (sCRP<0.4mg/dL), n=15], high CRP group showed significantly lower levels of hemoglobin(9.3+/- 0.7 vs. 9.8+/-0.6g/L, p<0.05), hematocrit(28.3+/-2.3 vs 29.8+/-1.696, p<0.05), creatinine(9.6+/-3.1 vs. 12.2+/-2.5mg/dL, p<0.05), prealbumin(20.9+/-5.0 vs. 25.8+/-6.4mg/dL, p< 0.05), SGA score(5.0+/-1.2 vs. 5.9+/-0.7, p<0.05), and percent of patients who have higher nPCR than protein intake(85.7 vs. 28.6%, p<0.05). Ferritin was significantly higher in high CRP group(503.1+/-205.7 vs. 323.3+/-186.6, p<0.05). There were no differences in age, sex, duration of hemodialysis, prevalence of diabetic nephropathy and cardiovascular disease, Kt/Vurea, nPCR, residual renal function, amount of protein intake and other nutritional parameters. In conclusion, there was higher probability of malnutrition, anemia and protein catabolism in hemo-dialysis patients with elevated sCRP concentration.

4.Acute Respiratory Failure after Embolectomy in Patient with Chronic Pulmonary Embolism: A case report.

Thrombolytic therapy is usually reserved for patients with clinically serious or massive pulmonary embolism. In desperated cases, however, pulmonary embolectomy is recommended despite its high mortality rate. We experienced acute respiraory failure after embolectomy performed under cardiopulmonary bypass in patient with chronic massive pulmonary embolism. The patient recovered sucessfully with postoperative management in the intensive care unit.

5.Age, hypertension, and genetic polymorphisms and their relative associations with white matter hyperintensities in Korean patients with Alzheimer’s disease

Objectives: White matter hyperintensities are known to influence dementia in Alzheimer’s disease.
Genetic components are suggested as putative risk factors for vascular pathology and cognitive
decline. This study aimed to determine whether there is an association between candidate genetic
polymorphisms and the severity of white matter hyperintensities in patients with Alzheimer’s disease.
Methods: Seventy-five patients diagnosed with Alzheimer’s disease underwent genetic tests for specific
alleles of apolipoprotein E, angiotensin-converting enzyme, and methylenetetrahydrofolate reductase.
All patients underwent brain magnetic resonance imaging scans and neuropsychological tests. The
severity of white matter hyperintensities was semiquantified using the CREDOS rating scale, and
patients were divided into three groups according to their rating. Results:The severity of white matter
hyperintensities was related to age and hypertension. However, none of the gene polymorphisms we
tested was found to be associated with the severity of white matter hyperintensities.
Conclusion:The genetic polymorphisms found in apolipoprotein E, angiotensin-converting enzyme
and methylenetetrahydrofolate reductase did not contribute to white matter hyperintensities in
Alzheimer’s disease.Only age and hypertension factors were found to be contributory to white matter
hyperintensities.

6.A Case of Juvenile Nasopharyngeal Angiofibroma-Transnasal Endoscopic Surgery-.

Juvenile nasopharyngeal angiofibroma (JNA) is a highly vascular and locally invasive tumor that occurs almost exclusively in adolescent boys. JNA accounts for 0.05-0.5% of all head and neck tumors. Pre-operative embolization is useful for reducing intraoperative blood loss and the risk of incomplete excision of the tumor. A case of angiofibroma limited to the left posterior and superior nasal cavity and nasopharynx was excised transnasally under endoscopic control after pre-operative embolization with Gelfoam(r). The tumor was completely removed without complications, and there was no evidence of any residual tumor or recurrence observed during one year of endoscopic follow-up. It can be suggested that for the limited lesions of angiofibroma, the option of a transnasal endoscopic approach could be cautiously applied by experienced surgeons.

7.Necrotizing Conjunctival Ulceration following Subconjunctival Atropine Depot Injection.

PURPOSE: To report a case of severe conjunctival necrosis associated with scleral melting following subconjunctival atropine depot injection in treating posterior synechiae associated with uveitis. METHODS: A 31-year-old male patient visited our clinic after subconjunctival injection of 1% atropine, 0.4 ml, in treating circular posterior synechiae associated with uveitis in his left eye. The marked conjunctival injection and chemosis around the limbus were noted. Three days after first visit to our clinic, remarkable necrosis had developed around the whole limbus, and newly developed whitish necrotic tissue was noted over sclera and episclera. We debrided the necrotic conjunctiva and episclera, and the exposed sclera was grafted with amniotic membrane. REUSLTS: After 4 weeks, the complete re-epithelization of necrotized defect was shown and there were no evidence of inflammation, symblepharon, or scleral melting. CONCLUSIONS: In usage of atropine, especially to eyes locally, subconjunctival injection should be avoided due to direct toxicity to the conjunctiva and sclera to form necrosis and melting. The damaged lesions of conjunctiva and sclera due to usage of this drug generally cause chronic inflammation and persistent surface defect. In order to restore and re-epithelize the necrotic defect, more aggressive treatment such as an amniotic membrane transplantation would be needed.

8.A Case of Acute Renal Failure due to Bilateral Acute Pyelonephritis.

Although urinary tract infections are common in adults, pyelonephritis is rarely considered in the differential diagnosis of acute renal failure. Acute pyelonephritis without urinary tract obstruction, previous renal diseases or septic shock is a rare cause of acute renal failure. Despite appropriate antibiotic therapy, recovery of renal function could be slow and incomplete. We experienced a 45 year-old woman with diabetes who developed bilateral acute pyelonephritis followed by acute renal failure. The renal biopsy revealed diffuse edematous and focal fibrotic inters- titium with infiltration of lymphocytes compatible with interstitial nephritis. Although her renal function improved gradually with antimicrobial treatment, the process was incomplete and renal dysfunction persisted at about 10-month follow-up, suggesting permanent renal damage. Therefore, we report this case with brief review of related literature.

9.A Study of the Personality of Adult Patients with Atopic Dermatitis.

BACKGROUND: Atopic dermatitis will persist into adult life and chronic severe atopic dermatitis may cause significant interference in social interaction. Despite the controversy concerning the existence of specific personality exhibited by patients with atopic dermatitis, psychological and social factors have long been considered important factors that have influence on the course of the disease. OBJECTIVE: The purpose of this study was to investigate the charateristics of the personalities of adult patients with atopic dermatitis and their relationship to psychological factors and clinical characteristics. METHODS: Eysenck Personahty Questionnaire(EPQ) was carried out on 54 adult patients with atopic dermatitis. Using the EPQ scores, a cluster analysis was done to identify the subgroup of psychologically disabled patients. The psychological and social aspects of the identified patients were then investigated using a designed questionnaire. RESULTS: 1. A cluster analysis identified two patient subgroups. Psychoticism(P), extroversion-introversion(E), and neuroticism(N) scores were significantly different between the subgroups. The neuroticism scores were above the upper limit of the normal range in the higher EPQ scored subgroups. However, the total clinical severity score which was evaluated in 25 patients showed no significant difference between these subgroups. 2. The higher psychoticism, extroversion-introversion, and neuroticism scored patients tended to express severe disability due to atopic dermatitis and a high level of stress which was evaluated by the designed questionnaire. CONCLUSION: Eysenck personality questionnaire can be helpful in identifying a subgroup of patients who may need psychiatric intervention.

10.A Study on the ROI Optimizing Technique for Accurate Breath Measurements in Sleep Apnea.

OBJECTIVE: A sleep apnea is the most frequent symptom among sleep disorders. As the number of aged people are increasing, research activities are also increasing to monitor sleeping disorders of the elderly who lives alone. We proposed a new processing algorithm to measure the quantity of breaths accurately. METHODS: We improved the conventional center-of-mass method and further apply the projection profile method. RESULTS: In this study a system that can monitors respiration non-invasively and automatically, by identifying the movement of the chest and abdomen using image processing technique during sleep, was implemented. We can see breathing status in real time based on the acquired breathing waves. To verify the designed system, the values from the polysomnography were compared and analyzed to validate the system accuracy. As a result, the mean accuracy is 96%. CONCLUSION: We can simply monitor the sleep apnea with no sleep interferences.
