4.A Predictive Index of Left Ventricular Performance after Mitral Valve Replacement in Pure Mitral Regurgitation.

The index on myocardial contractility of pure mitral regurgitation(MR) after mitral valve replacement is believed to be useful in determining proper operation time before irreversible myocardial damage by volume overload. Thus the authors examined pre and post-operative echocardiographic results of 20 cases of pure MR patients who had been admitted to Pusan National University hospital and compared the usefulness of each index. Pre and post-operative echocardiographic results were as follows respectively ; 1) Left ventricular internal dimension in diastole(LVIDd) were 6.49+/-0.19cm and 5.51+/-0.17cm. 2) Left ventricular internal dimension in systole(LVIDs) were 4.26+/-0.12 and 3.79+/-0.18cm. 3) Ejection fraction were 71.28+/-1.57% and 59.24+/-3.05%. 4) Fractional shortening(FS) were 34.09+/-1.18% and 27.21+/-1.84%. 5) Mean Vcf were 1.53+/-0.08cm/sec and 1.18+/-0.09cm/sec. 6) Left ventricular end-diastolic volume index(EDVI) were 196.35+/-18.33cc/m2 and 98.46+/-9.96cc/m2. 7) Left ventricular end-systolic volume index(ESVI) were 55.28+/-5.12cc/m2 and 41.88+/-6.07cc/m2. 8) Left ventricular end-systolic wall stress/ESVI(ESS/ESVI) were 2.3+/-0.21 and 3.91+/-0.83. ESS/ESVI showed significantly(p<0.05) elevated postoperative change, LVIDd, LVIDs, EF, FS, EDVI, ESVI revealed significantly(p<0.05) reduced postoperative change whereas Mean Vcf had no postoperative change. After examining correlation between preoperative and postoperative results of each index, it is believed that LVIDd, LVIDs, and ESS/ESVI were useful prognostic indices but were unrelated to postoperative ejection phase indices such as EF, FS, and MEan Vcf. Especially in case of LVIDd>6cm, LVIDs>4cm, and ESS/ESVI<2, the prognosis was poor due to myocardial damage by mitral regurgitation.

7.Clinical Study of Vitiligo.

BACKGROUND: The clinical behavior of vitiligo has not been clearly understood and hypothesis concerning the pathogenesis of the disease has been confusing and contradictory though autoimmune mechanisms have been considered important by many authors. OBJECTIVE: The purpose of this study was to develop a better understanding of the clinical features and pathogenesis of vitiligo. METHODS: We investigated clinical features of vitiligo in 1315 patients, and also compared the clinical course and features of non-segmental type(type A) and segmental type(type B) vitiligo patients to see whether the two types of vitiligo have a different pathogenic mechanism. RESULTS: Previously reported clinical patterns of the disease were reviewed and compared with our data, and the different clinical findings between the two types which supported the hypothesis of Koga et al. that type A and type B vitiligo had a different pathogenesis and autoimmune mechanisms played a role only in type A were shown. CONCLUSION: We investigated the clinical characteristics of vitiligo in Korea and showed that the type A vitiligo might have a different pathogenic mechanism with type B.

8.Endoscopic Variceal Ligation for Treatment of Esophageal Varices.

Endoscopic injection sclerotherapy(EIS) has been widely used in treating and eradicating acutely bleeding esophageal varies, but may be associated with some undesirable local and systemic complications. Endoscopic variceal ligation(EVL), which consists of mechanicai ligation and thrombosis of varices using elastic o-band, has been recently developed as a non operative alternative to EIS. We performed EVL in 65 patients who had bled from esophageal varices between November 1991 and September 1993. Total 274 sessions were performed and 774 o-bands were used. Six patients were actively bleeding and all of them were successfully controlled by emergency EVL. During the follow-up period, five patients who had combined hepatoma died. Varices were eradicated or reduced grade I in 43(71.6%) of the 60 survivals by 8-36 ligations(mean 15.6 ligation) in 2-13 EVL sessions(mean 5.6 sessions). During follow up period, five patients had recurred from grade 0 to grade 2 or 3 in 106-260 days(mean 182.6 days), and then eradicated by repeated EVL. During or after EVL, there were no complications, except mild substernal distress and mild dysphagia in 17 and 7 patients respectively. These results showed that EVL is a safe and effective method for eradication of bleeding esophageal varices.

9.HLA Antigens, T and B Lymphocytes and other Associated Laboratory Findings in Patients with Vitiligo.

Although vitiligo is a common disorder with well-known clinical features, its etiology is still vague. It has been suggested that immunologic factors are important in the pathophysiology of vitiligo. And the association of HLA antigens with vitiligo has been reported. Authors examined HLA phenotypes in thirty-nine patients, T lymphocytes in thirty- three patients, B lymphocytes in thirty-one patients, and quantitation of IgG, IgA, IgM and IEP(immunoelectrophoresis) in twenty-five patients with vitiligo in this study. T3 and T4 were investigated in twelve patients, CBC in thirty-nine and urinalysis in thirty-five patients with vitiligo for screening of associated diseases ; thyroid disease, anemia and diabetes mellitus. The results are as follows; The frequency of HLA A9, Bw15 and Bw22 was significantly decreased in vitiligo patients compared to control group(p=0. 003, 0. 02, 0. OR) 2. There were no significant differences in the results of total and active lymphocytes and B lymphocytes in vitiligo patients compared to the control group. The results of quantitation of IgG, IgA, IgM and IEP showed within normal lirnits except one, an increased IgG, 2,200rng/100ml (normal: 500 1,500) and polyclonal gammopathy. 4. Among twelve vitiligo patients, one showed increased TR, 355ng/100ml (normal: 80 220) and T4, 17. 3pg/100ml (normal: 5 13). 5. CBC and urinalysis were within normal limits in all patients.

10.Three cases of subcutaneous sarcoidosis.

We report herein three cases of subcutaneous sarcoidosis without apparent internal lesion. The patients had multiple subcutaneous nodules on the extremities and abdomen. Hyper-gammaglobulinemia, a relative increase of T suppressor cells, and increased serum angiotensin converting enzyme level and skin anergy were observed. Histopathologic findings revealed sarcoidal granulomas involving the subcutaneous fat tissue. The lesions of two cases resolved spontaneously and the other one case required systemic corticosteroid therapy.
