This study was performed to investigate the distribution of endotoxin in various organs after intraperitoneal injection of E. coli homogenator(0111:B4, 3X10(9)cells/200g of body weight). Sprague-Dawley rats were intraperitoneally injected with E. coli homogenator and sacrificed 1 and 3 hours after injection. The lung, liver, and kidney were immunohistochemically stained with avidin-biotin complex method and observed by light and electron microscopy. On the light microscopy, granular deposits of reaction products of immunohistochemical stain were found on the cytoplasmic membrane of endothelial cells and some of parenchymal cells of all organs observed. Electron microscopic study revealed finely granular reaction products on the surface of endothelial cells and some of parenchymal cells. The pinocytotic vesicles of endothelial cells demonstrated reaction products in the early phase of experiment. The distribution of reaction products were prominent in the liver among three organs. The Kupffer cells showed the most sensitive and strongest positive reaction. The hepatocytes and endothelial cells revealed weak positive reaction 3 hours later. The alveolar macrophages of the lung were also positive from the early phase of endotoxemia, while the pneumocytes and alveolar septa demonstrated weakly positive reaction in the later phase. The capillary endothelium of the kidney revealed positive reaction from the early phase. According to above results, it is concluded that the endotoxin entered into the systemic circulation was captured in the liver and lung. And both mononuclear phagocytic system and endothelial cells could be activated or damaged by endotoxin.

BACKGROUND: The purpose of this study was to measure and compare the optimum depth of the internal jugular venous catheterization between the right and left side. METHODS: Forty-four patients were enrolled for this study and divided into two groups (22 patients each). The optimum depth of the catheterization was calculated using the sum of two component (A and B); the advanced length of the catheter from the level of the cricoid cartilage (A) and the distance from the catheter tip to the junction of the superier vena cava and right atrium (B). RESULT: The optimum depths of the internal jugular venous catheterization were 16.0 1.0 cm (right) and 18.4 1.5 cm (left) respectively. Left side was significantly longer than right side (p<0.05). In this study, we experienced some complications; arterial punctures (5 cases) and migration of the catheter to the opposite subclavian vein (1 case). Five complications were associated with left internal jugular venous cannulation and one was associated with the right side cannulation. CONCLUSION: We concluded that the optimum depth of the internal jugular venous catheterization was longer in the left side than in the right side.

OBJECTIVES: The purpose of this study is to evaluate the effect of geriatric depression in activities of daily living for self maintenance (ADL) and instrumental activities of daily living (IADL) under control for effect of cognitive impairment which can be originated by other reasons. METHODS: The author had interviews with 100 old peoples using various scales (KGDS, MMSE-K, Karnofsky Performance Status Scale, ADL, IADL, QOL questionnaire, HDRS). This study also attempts to compare three groups (depression group, recovered group, normal control group) in their functional status. RESULTS: 1) Self-maintenance skills are more affected by physical states than by depressive symptoms. But the effect of depression is more prominent in instrumental activities of daily living. 2) Korean Geriatric Depression Scale (KGDS) correlated well with activities of daily living for self maintenance (ADL) and instrumental activities of daily living (IADL), but Hamilton Depression Rating Scale (HDRS) did not show significant correlation. This reflects that KGDS is a more specific measure for geriatric depression patients. When the effect of physical illness state was eliminated, partial correlation between KGDS and ADL/IADL showed no significant association. This means that the patient's physical functioning status is very important in functional level of geriatric patient. 3) When the patient is recovered from geriatric depression through appropriate treatment, patient's functional level showed no significant differences with normal control. CONCLUSION: This article emphasizes the importance of appropriate treatment for geriatric depression. After the effect of cognitive function was controlled, geriatric depression itself showed significant functional impairment. This functional deficit can threaten the patient's independence and self-respect. So appropriate treatment for depression in geriatrics is indispensable for enhancing quality of life in old people.

BACKGROUND: In spite of relative high risk of extrapyramidal side effect and tardive dyskinesia, it is common clinical practice to use antipyschotics in treatment of bipolar I disorder. But in Korea, there has been few study about clinical factors related with antipsychotics treatment in bipolar disorder patients. So the author studied about the clinical factors related with antipsychotics treatment in bipolar inpatients focusing on the comparison of risperidone and classical antipsychotics. METHOD: By reviewing medical record retrospectively, datas about patient-related, illness-related, and treatment-related clinical factors on antipsychotics use were collected. Association between antipsychotics dose and duration and clinical factors were analysed by statistical method. RESULTS: Aggressive behavior was only statistically significant factor associated with antipsychotics use. And in the case of aggression, maintenance dose of risperidone was not changed(p=0.84), but dose of classical antipsychotics was increased significantly(p=0.005). Total hospital days and antipsychotics treatment duration showed no difference between risperidone and classical antipsychotics treatment groups. CONCLUSION: Clinical factors associated with antipsychotics use was aggressive behavior. In the case of aggression, risperidone required lesser dose increment compared with classical antipsychotics.

BACKGROUND: Coronary artery bypass grafting without cardiopulmonary bypass (Off-Pump Coronary Artery Bypass Grafting, OPCAB) causes significant hemodynamic derangement by displacement of the beating heart. The purpose of this study was to analyze the hemodynamic changes caused in relation to grafted arteries by displacing the heart and stabilizing the coronary arteries in patients undergoing OPCAB. METHODS: Nineteen patients underwent OPCAB using two deep pericardial sutures and tissue stabilizers (Octopus Tissue Stabilization Syetem, Medtronic, USA). The hemodynamic variables were obtained after induction of anesthesia, after deep pericardial sutures, before and after anastomosis of each coronary artery during epicardial stabilizing, after sternal closure, and after postoperative 6 hours and 12 hours in the intensive care unit. RESULTS: The hemodynamic variables were maintained with the Trendelenburg position, volume loading and low dose vasopressors after deep pericardial stay sutures. Displacement of the heart and placement of the stabilizer on all coronary territories except the obtuse marginal artery before anastomosis showed no significant difference in hemodynamics compared with baseline. Positioning for the graft to the obtuse marginal artery decreased cardiac index (1.6+/-0.4 L/min/m2) and stroke index (27.6+/-9.9 L/beat/m2) and increased systemic vascular resistance (2318.9+/-673.7 dyne sec cm(-5)), resulting in hemodynamic compromise (P<0.01). There were no significant hemodynamic and electrocardiographic changes before or after grafting of other coronary arteries but there was a significant increase in cardiac index after postoperative 6 and 12 hours compared with baseline values (P<0.05). CONCLUSIONS: Although the complete revascularization of most coronary arteries is feasible on the beating heart without significant hemodynamic compromise with minimal vasopressor support, the positioning for the graft to the obtuse marginal artery needs special attention because two deep pericardial stay sutures and Octopus tissue stabilizers on the obtuse marginal artery territory induce significant hemodynamic disturbances.

A 42-year-old male was hospitalized with abdominal pain, dyspnea, and turbid peritoneal fluid. He was diagnosed with hypertension, diabetes and started continuous ambulatory peritoneal dialysis (CAPD) 11 months ago. He was treated with intraperitoneal cefazolin and ceftazidime, and then white blood cell counts of dialysate decreased. Incidentally, liver abscess was found in chest CT performed for the evaluation of dyspnea, and patient was febrile persistently. So percutaneous abscess drainage was done by pigtail catheter. We changed the antibiotics to ceftriaxone and metronidazole, and hemodialysis was started. Klebsiella pneumoniae was cultured from peritoneal fluid and blood simultaneously. We concluded that liver abscess is a primary cause of CAPD peritonitis.

In order to examine elderly depression in the elderly that too weakened cognitively, and physically to check depressive symptoms reliably, the authors deviced collateral source version of elderly depression scale and tested it's validity. Sixty elderly depression patients and forty-eight old person in normal group completed the KGDS (Korean Form of Geriatric Depression Scale), and their collateral source (CS) completed a CS version of the KGDS (CS-KGDS). The differences of CS-KGDS means was signified at alpha=.001 level (patients group mean=18.33 (4.71), control group mean=7.60 (5.25), t=11.18). The values of Cronbach's alpha and Split-half reliability were .88 and .81 respectively, and correlational coefficent with KGDS was .68. On factor analysis, 5 factors were extracted. They were labeled 'Physical weakening and decreased vitality' (factor 1), 'Emotional discomfort' (factor 2), 'Negative thinking and Unhappiness feeling' (factor 3), 'Cognitive dysfunction' (factor 4), 'Decreased social interest and activity' (factor 5), which congruous with internal structure of KGDS. The result of discrimination analysis showed 87.09% of hit ratio, and suggested the score of 13 as optimal cut-off score. This cut-off score was identical to that of KGDS. Conclusively, CS-KGDS could be a useful tool for evaluaion of elderly depression instead of KGDS in such case that KGDS can not be administered reliably.
