1.A Case of Toxic Epiderma Nocrolysis.

Toxic epidermal necrolysis(T.E.N.) is a toxic erythema of the skin that undergoes epidermal necrolysis and peding so that it appears to have been scalded. There seem to be two basic form of T.E.N. In children, the deavage of the epidermis that results from an acantholytic-dyskeratotic cytolytic process is in the malpighian or granular layer and is usually induced by a staphylococci toxin. In adult, it is characterized by a full-thicknees cytolytic cleavage of skin above the basal layer and is usually precipitated by a drug. The authors observed a 17 year-old male with T.E.N. He began to notify the erythematous skin lesion with burning sensation on the whole body surface at 12 hours after oral administration of some drugs for common cold and developed the characteristic flaccid bullous and denuded lesions with tenderness within next 20 hours.

3.A Case of Dilated Pore of Winer.

We report a case of dilated pare of Winer occuring in a 33-year-oid female patient. Dilated pore belongs to adenoma or organoid group of benign hair differentiation tumors. The patient has had a giant comedone on the right submandibular area for 1 month. Histopathologic features revealed rnarkedly dilated pilar infundibulum lined by epidermis that is atrophic near the ostium but hypertrophic deeper in the cystic cavity. Three months after excisional biopsy, there was no recurrence.

7.Clinical and Histopathologic Study of the patients with Lichen Striatus.

Nineteen patients with lichen striatus were investigated by clinical data, clinical photographs and histopathologic study by the pattern analysis method of Ackermsn. Results were as follows '. 1. The male to female ratio was 1: 1.1, and average age was 7.8 years. Seven- teen patients (about 90%) visited hospitals during summer season. 2. Nine cases showed superficial perivascular dermatitis, 9 cases interface dermatitis (vacuolar type : 7, lichenoid type, 2), 1 case nodular dermatitis and 2 rases interface dermatitis with nodular dermatitis. 3. In the epidermis, 9 cases showed variable nurnber of dyskeratotic cells. One case showed finding of perforating lichen striatus. 4. Two cases showed inflammation around acrosyringium, 4 cases around hair follicle, 6 cases around eccrine sweat gland and 1 case showed perineural inflamrnation. Summarizing above results, histopathology of lichen striatus characteristically shows superficial perivascular dermatitis, interface dermatitis, and infiltration of inflammatory cells around the sweat gland, which helps to diagnose the disease.

8.Risk factors of ischemic brain stroke in Korean diabetic patients: a retrospective study.

In order to investigate the risk factors of ischemic brain stroke (IBS) in diabetic patients, we analyzed 416 cases selected from 6239 diabetic patients admitted to Severance Hospital from Jan. 1983 to Dec 1987. Two hundred and eight cases had IBS. The other 208 cases without IBS were selected as a control group by a stratified random sampling method. The two groups were compared using various clinical characteristics of diabetes mellitus (DM) and known risk factors of IBS. In diabetics with IBS, the duration of DM was longer (9.2 +/- 7.1 years), hypertension was more frequently associated (68.2%) and the serum cholesterol level was higher (213.0 +/- 55.2 mg/dl) than in diabetics without IBS (7.6 +/- 7.1 years, 32.1% and 192.1 +/- 44.8 mg/dl, respectively). By means of Stepwise Logistic Regression Analysis (SLRA), it was found that the strong risk factors were hypertension and serum cholesterol level. The serum triglyceride level, type of DM and response to diabetes treatment were also thought to be risk factors of IBS by the retrial of SLRA of residuals after exclusion of hypertension and serum cholesterol level. IBS was not significantly related to the duration of DM, fasting blood glucose level, body weight, glycosylated hemoglobin value, and serum high-density lipoprotein-cholesterol level.
