5.Dosimetry and MIRD for Re-188 Liquid Balloons.

Re-188 is suitable for endovascular liquid-balloon brachytherapy for the prevention of restenosis after angioplasty. Re-188 was concentrated to 3700 MBq/ml and labeled with DTPA. According to dosimetric calculation, it took 420 seconds using Re-188 solution with concentration of 3700 MBq/ml to irradiate 17.6 Gy to the target at 1 mm from the balloon surface. Software was made to estimate the irradiation time. MIRD calculation with dynamic bladder model yielded the whole body dose of Re-188-DTPA as 0.005mGy/MBq in case of balloon rupture and release of the whole amount into the blood.

7.A Clinical Observation on Tumors of the Genito-urinary Tract.

122 cases of the genitourinary tract tumors admitted to the Department of Urology, National Medical Center, during the period, Aug. 1968 to July 1975 and the followings are the results. 1. 122-cases(12.8%) were tumor patient among 954 (male 702, female 252) Total admitted patients during 7 years. 2. Age distribution was from 5 months to 83 years showing the highest incidence in the 7th decade (30.3%) and cases over 41 years of age were 99(81.1%) 3. Sex ratio of male and female was 5.4:1(103:19) 4. In 122 cases, 42 cases{34.4%) were benign, 77 cases(63. 1%) were malignant and remainders were uncertain pathologically. 5. Among 122 cases, 57 cases(46.7%) were vesical, 35 cases(28.7%) prostatic, 19 cases(15.6%) renal, 5 cases(4.1%) penile. 3 cases(2.5%) testicular and 3 cases(2.5%) were urethral tumors. 6. Operations were performed in 114 patients (93.5%).

8.Comparison of Epidural Anlagesia with Morphine-Bupivacaine Versus Fentanyl-Bupivacaine after Cesarean Section.

Background: This study was aimed to compare the analgesic effect and side effects of morphine- bupivacaine mixture with those of fentanyl-bupivacaine mixture after Cesarean section. Methods: Eighty patients who were taken continuous epidural catheterization after Cesarean section were divided into two groups. In group 1 (N=40) the mixture of 1% lidocaine 10 ml and morphine 1mg was firstly injected via epidural catheter, and then two day infusor (Baxter(R)) which contained the mixture of 0.15% bupivacaine 100 ml and morphine 6mg was connected to epidural catheter. In group 2 (N=40) the mixture of 1% lidocaine 10 ml and fentanyl 100 mcg was firstly injected via epidural catheter, and then two day infusor (Baxter(R)) which contained the mixture of 0.15% bupivacaine 100 ml and fentanyl 850 mcg was connected to epidural catheter. Mean arterial pressure (MAP) and heart rate (HR) were checked preoperatively, and at post-injection 10, 20, 30 and 60 minutes. The visual analogue scale (VAS) was checked at postoperative 1/2, 1, 6, 12, 24 and 48 hours. The side effects of epidural analgesia were evaluated. Results: In group 2 MAP was significantly decreased at post-injection 20 minute. VAS was significantly increased at post-injection 1/2 and 1 hour in group 1. The most frequent side effect was pruritus in both groups. Conclusions: The first bolus injection of the mixture of 1% lidocaine 10 ml and fentanyl 100 mcg has more rapid analgegic effect than the mixture of 1% lidocaine 10 ml and morphine 2 mg, but because of shorter duration of action of fentanyl it seems to be better to increase the dosage of fentanyl or replace fentanyl by morphine for more effective epidural analgesia after Cesarean section.

9.Posterior sagittal anorectoplasty, Secondary procedure, Anorectal malformation.

Eleven patients underwent posterior sagittal anorectoplasty(PSARP) as a secondary procedure. Two of them had rectovaginal fistula and another two had rectocutaneous fistula. Six of the rest complained of frequent fecal soiling and the last one had severe anorectal stricture after perineal anoplasty. Five patients had lived with colostomy until the second operations were carried out. The ages at the time of the secondary PSARP were between 7 months and 29 years. Distal colostogram and MRI were taken to evaluate distal colon, position of the rectum and voluntary muscle. All patients had normal sacrum except one who had anorectal stricture. Seven patients, six with fecal incontinence and the other one with rectovaginal fistula had mislocated anorectums. Three patients, two with rectocutaneous fistula, the other one with anorectal stricture, had abdominal approach to obtain enough length of colon for pull-through procedure. With the posterior midsagittal approach, we could manage all the problems, rectovaginal fistulas, rectocutaneous fistulas, strictures and malpositioned rectums, without difficulty. No patients had serious complications except wound infection in one. All patients were satisfied with the results after redo-PSARPs even though normal continence has been achieved in only one patient. Seven patients who had continuous soiling or rectocutaneous fistula, needed no more diapers even though four of them showed fecal staining under stressful condition and the other three showed intermittent fecal leaking less than once a day. The rest three of the patients maintained their continence with support of drugs and/or enemas because of constipation. The PSARP is a popular procedure as a primary operation; however, our results suggested that this procedure also gave us a good opportunity for management of serious complication developed after primary anoplasties.
