1.Clinical Study of Operative Treatment of Medical Collateral Ligamentous Injuries of the Knee Joint

Rotatory Instability of the permitting abnormal external totation of the tibia on the femur is the result af forced abduction of the flexed knee and external rotation of the tibia. The basic lesion is a tear in the medlal capsular ligament. The authors have reported clinical study in 40 patients with medial collateral ligament injuries who were treated by operative methods in the Orthopedic Department at the Taegu Presbyterian Hospital from January 1973 to Octdber 1979. of 40 patients, pes anserinus transplantation was performed on 15 patients and direct repair on 25 patients. The results are as followings: 1. All the patients were seen with a complete rupture of the medial collateral ligament and of these 45% had rupture of the femoral attachement. 2. Exellent to good result were more often obtained in recent injuries of less than 2 weeks rather than those in older Injuries. 3. Exellent to good result were more often obtained in the groups of Pes Anserinus tranplantation than the groups of direct repair. 4. In all cases of groups of pes anserinus transplantation, permanent sensory palsy af the patellar branches of the saphenous nerve was present due to the location of the surgical incision.

3.Granulosa Cell Tumor Arising in the Paratesticular Area: A case report.

Granulosa cell tumor of the testis is extremly rare in adult males and granulosa cell tumor occurring in the paratesticular area has not been reported. We report a paratesticular granulosa cell tumor in a 34-year-old man who presented with a 5.5 cm sized scrotal mass. The tumor was present in the paratesticular area near the head of epididymis. It was located in the tunica and completely separated from the testis by thick fibrous tissue. Microscopically, the tumor exhibited solid, microfollicular, and trabecular patterns. The tumor cells had ovoid to elongated nuclei with longitudinal intranuclear grooves and one or two nucleoli and scanty cytoplasm. Mitoses were relatively frequent with an average number of 9/10 HPFs. The tumor extended into the visceral tunica vaginalis and showed multiple lymphatic tumor emboli. Immunohisto chemical stains revealed diffuse strong positivity for inhibin, vimentin, and keratin and diffuse weak positivity for estrogen and progesteron receptor. Placental alkaline phosphatase (PLAP) and epithelial membrane antigen (EMA) were negative. On electron microscopic examination, tumor cells were polygonal and had large indented nuclei. The cytoplasm contained a moderate number of small round mitochondria, abundant rough and smooth endoplasmic reticula, and a few lipid droplets. Small aggregates of intermediate filaments and intercellular junctions were observed. The patient was alive and well 5 months after orchiectomy. This is the first case of adult granulosa cell tumor arising in the paratesticular area.

4.Activity and Sleep Apnea Monitoring of Aged-Person using Image Processing.

OBJECTIVE: As the rapid progress of aged society, there must be solutions of preparation against unpredicted accidents for aged solitary people. The most important thing that we must consider is the unconstraint of daily life. So, we are to develop a system and algorithm which meet our objectives. METHODS: We have monitored the degree of activity of the aged solitary person. The CCD camera was used not to disturb the daily activity and we evaluated the degree of activity using image processing on personal computer. The activity monitoring during night was assumed by sleeping on the bed, so the major method was breath monitoring during sleeping. On the other hand, daily activity was monitored by wide viewing camera in the living room. To prevent the privacy trouble, the acquired image was converted to binary form and the degree of breath and moving factor was estimated. RESULTS: In this paper we propose a new processing algorithm to accurately measure breathing characteristics in sleep apnea sufferers. We improved the conventional center-of-mass method and further applied the projection-profile method. As a result, we have improved breath measurement accuracy. In a comparison with conventional polysomnography, our method was 92% effective in detecting apnea cases. CONCLUSION: As a result of this study, we can monitor sleep apnea more simply and with no sleep interference. In measuring the activity of daily life, these improved algorithms were applied. So, we established a monitoring method of no-constrained, quantitative measurement for the aged solitary people during the whole day.

5.Web-based Monitoring of Infant Incubators.

Infant Incubator is the most important equipment in a infant ICU. This maintain adequate temperature and humidity for survival of infants. The final object of this research is Web-based monitoring of several infant incubators in ICU. To realize these objects, we developed a temperature/humidity sensing module which is interfaced to Web server board via RS485 port. We used a small micro controller in each sensing module. So the size of this module is small enough to be attached to incubator. We can access each sensing module with RS485 port on Web server board via network. As a first trial, we networked 4 infant incubators. The operator can send alarms to each incubator and these alarms are indicated with sounds and lights on measurement module. Our system can increase the convenience by a intensive central monitoring, so instant handling to some kinds of accident is possible.

7.A Case of Severe Hypertension associated with Growth Hormone Therapy.

The use of recombinant DNA technology to produce human growth hormone has resulted in a marked increase in availability of Growth Hormone(GH) to treat short stature due to GH deficiency and other conditions, such as Turner syndrome, familial short stature, chronic renal insufficiency and intrauterine growth retardation (IUGR). But, the GH therapy may result in the adverse events such as sodium and water retention, pseudotumor cerebri, slipped capital femoral epiphysis, growth of nevi, recurrence of tumor. We experienced a case of severe hypertension associated with GH therapy in a 14-year-old male who presented high blood pressure up to 190/100 mmHg and normalized at 2-3 weeks after discontinuation of GH. Therefore, we think that the blood pressure should be carefully monitored during GH therapy.

8.Study on the radiographic measurement of the urinary tract in the normal Korean adults

The review of normal intravenous pyelography can give us important criteria of the diseases of the urinary system and the adjacent organs. Authors analyzed 200 cases of intravenous pyelography of normal Korean adults and measured the following points; the length of kidneys, the distance from midline to both poles, the slope degree of longitudinal axis, the ureteral length and width, the shortest distance from midline to abdominal ureters, the longest distance form midline to pelvic ureters, and configuration of urinary bladder. The results were obtained as follows; 1. The average length of kidneys in adult male was 12.53±0.78 cm on right, 12.83±0.82cm on left; in adult female, 12.08±0.72cm on right, 12.46±0.73cm on left. In the age distribution, the length gradually became shorter by getting old. In 20% of cases, right kidney was longer than left. 2. The average distance from both the renal poles to midline was longer in male compared to female. The distance from upper pole to the midline was longer in left side than right in male, but in the lower pole, there was no difference in both sexes. 3. The slope degree of the renal longitudinal axis was larger in male than in female, and lager in right kidney compared to left. 4. The length of ureter was longer on left, but there was no remarkable difference between both sexes. 5.The short set distance from midline to abdominal ureter, there was no remarkable difference between both sexes and also between right and left side. But concerning the longest distance from midline to pelvic ureter, it was longer in female; and especially in female, right side was shorter than left. 6. The longitudinal diameter of urinary bladder was longer in male, but horizontal diameter was longer in female.

9.Q-Switched Nd:YAG laser Posterior Capsulotomy of the Aphakia and Pseudophakia Inserted with Posterior Chamber IOL.

To get the pure clinical results of Yag Laser Posterior Capsulotomy, excluding all the possible factors that might affect the results of that, we performed posterior capsulotomies on after cataracts(118 eyes) by using Q-switched Nd:Yag laser from February 1987 to August 1989 and reviewed the results. Group A is composed of 68 surgical aphakic eyes and group B is composed of 50 Posterior chamber pseudophakic eyes. The results were as follows: 1. One eye in groupA and one eye in group B were congenital cataracts and the others were all adult and senile cataracts. 2. As a result of posterior capsulotomy, 59 eyes(86.8%) in group A and 42(84%) in group B were above 0.5 in corrected visual acuity. 3. On performing, the power setting was under 1.5mJ in 59 eyes(86.8%) in groupA and 40 eyes(80%) in group B. 4. Ocular complications developed in 35 eyes(51.5%) in group A and 15 eyes(30%) in group B. 5. Good preoperative ocular condition may predict good postoperative clinical results. 6. Less energy and pulses in operation combined with less occurrences of vitrous prolapse in group B might be associated with less development of postoperative complications in group B than in group A.

10.Clinical Study of Controlled Release Form (Orfil Retard(R)) of Sodium Valproate.

A cross-over study was conducted to compare the pharmacokinetic properties of controled release form of Sodium Valproate (Orfil Retard(R)) with that of conventional forrnulation (Orfil(R) in healthy adult volunteers. The clinical efficacy of Orfil Retard(R) was also investigated in 15 epileptic patients under Orfil(R) monotherapy by replacing an equivalent bid dosage of Orfil Retard(R) for 2 months. Orfil Retard(R) achieved more stable blood level than Orfil(R), which was statistically significant(P;0.0051). Orfil Retard(R) was associated with less side effect. Especially of gastrointestinal system. None of epileptic patients showed worsening of seizure control after switch to the equivalent b.I.d. dosage of Orfil Retard(R). Our results indicate that Orfil Retard(R) offers more advantage than Orfil(R) in terms of more stable blood level and less side effect. Clinically. Orfil Retard(R) in equivalent b I d dose is as effective as Orfil(R) in b.I.d. or t.I.d. dosing schedule.
