1.Clinical Effect of Nicardipine(Perdipine(R)) on Cerebrovascular Diseases.

Twenty-five patients with cerebrovascular diseases were administered Nicardipine, a calcium antagonist, in daily dose of 60 mg per orally for 8-12 weeks during the year of 1983, to evaluate the efficacy of the drugs clinically. The following results were obtained. 1) General improvement rating was 80%. 2) Marked improvement(88%) were noted in subjective symptoms, especially of tinnitus, paresthesia and headache. significant improvement were noted in psychiatric(74%) and neurologic(71%) symptoms e.g disturbance of sleep, emotion, memory, speech, swallowing and muscle power. 3) In 13 hypertensive patients, average blood pressure lowered from 163/96 to 138/83 mmHg in 4 weeks and maintained thereafter. 4) major adverse effect was facial flushing, noted in 4 patients(16%), which were transient and mild and allowed to contiume medication.

5.Congenital Bronchoesophageal Fistula of Adult in Korea.

BACKGROUND: Congenital bronchoesophageal fistula(BEF) presented in adult life is a rare disorder and has characteristic clinical findings such as paroxysmal cough after water ingestion and recurrent respiratory infections. It usually manifested recurrent pneumonia and chronic cough with purulent phlegmon which was mis-or under-diagnosed as chronic bronchitis, bronchiectesis or lung abscess so forth. METHODS: We reviewed retrospectively 13 cases of congenital BEE in adult of Paik Hospital, College of Medicine, Inje University including 22 cases of congenital BEE previously reported in literature of Korea from 1979 through 1995. RESULTS: The mean age at diagnosis was 40.2 +/- 14.3. There was no difference in sex ratio(Male: Female 18 : 17). The mast common symptom was cough(91.4%), follwed by chronic sputum(74.3), hemoptysis(25.7), and paroxysmal nocturnal cough at specific position(20%). Twenty one of 31 patients who were able to review have the most specific sign, Ono's sign presented as paroxysmal cough after liquid ingestion. By classification of Braimbridge-Keith, Fourteen(45.1%) of 31 patients were group I (associated with esophageal diverticulum), 15(48.4%) were group II (simple fistula), and group III arid IV was one case in each. The opening of fistula confined to right lower lobe in 26(76.5%), left lower lobe in 6(176%), arid left main bronchus in 2(5.9%) cases. CONCLUSION: Congenital bronchoesophageal fistula is uncommon disorder which has characteristic histories and specific symptoms such as chronic and recurrent lower respiratory infections, and paroxysmal cough after liquid ingestion. Medical attention and careful history should be done in patients who have localized recurrent lower respiratory infections in right lower lobe.

7.Acute Lymphoblastic Leukemia with Philadelphia Chromosome and Monosomy 7.

BACKGROUND: The combination of Philadelphia chromosome (Ph) and monosomy 7(-7) was rarely observed in acute lymphoblastic leukemia (ALL). With the results from immunophenotyplc and molecular analysis, Philadelphia chromosome positive ALL with monosomy 7[Ph(+)/-7] has been considered that it may be derived from neoplastic transformation at the pluripotent stem cell level. We compared the clini-cal, laboratory, and hematological findings between 5 cases of Ph(+)/-7 and 5 cases of Ph(+) without monosomy 7 [Ph (+) /N7]. METHODS: During the period from January, 1995 to December, 1996, total 72 cases of ALL were confirmed among 259 cases of hematologic malignancy with bone marrow cytogenetic analysis. Among 72 ALL cases, 5 cases of Ph(+)/-7(monosomy 7 or 7q abnormalities) were compared with Ph only or Ph without monosomy 7(ph(+)/N7] on the hematological, immunophenotypic, other laboratory, clinical findings and event ree survival (EFS) The karyotyping of the bone marrow specimens was analysed byshort-term unsynchronized culture methods such as overnight colcemid treatment and 24 hours incubation following ethidium bromide treatment. RESULTS: The mean age of Ph(+)/-7 was 30.6+/-12.8 years, and it was significantly different from that of Ph(+)/N7 (p=0.009), Four cases of Ph(+)/-7 were classified as ALL L2 subtype, and 2 cases revealed CNS involvements. Immunophenotyping was positive in CD10, CDl9, CD2O, CD22 and HLA-DR. But one case revealed e-B-lymphoid lineage with positivity in CD34, CDl3, and CD33. The response to chemotherapy and EFS was very poor in Ph(+)/-7 group, and the mean EFS was 3.2+/-1.9 months(p=0.014). All of cases showed induction on failure in chemotherapy, relapsed with bone marrow, CNS and extramedullary involvements, and expired due to sepsis. CONCLUSIONS: Ph(+)/-7 ALL had very Poor clinical course with being resistant to chemotherapy and unfavorable prognosis, revealed L2 subtype by FAB classification, and was slightly older in ages compared with Ph(+)/N7 ALL.

8.The Usefulness of Endo-rectal Coil MRI in the Staging of Clinically Localized Prostate Cancer.

PURPOSE: We evaluated the ability of endo-rectal coil MRI (ER-MRI) to predict the local pathological stage of prostate cancer prior to radical prostatectomy and compared the results with those of transrectal ultrasonography (TRUS). MATERIALS AND METHODS: ER-MRI using high field magnets (1.5 Tesla) were performed in 22 patients (mean age 62.8 years, range 51-73) with clinically localized prostate cancer before radical prostatectomy. Of the 22 patients, 17 patients were also assessed by TRUS. The results of the imaging techniques were compared with the post-operative histopathological findings. As one patient with pelvic lymph node metastasis, which was detected on frozen-section examination during surgery, was spared radical prostatectomy, the final evaluation included 21 patients. RESULTS: DSeven of the 21 patients (33%) were found to have extraprostatic extension (EPE), and 5 had seminal vesicle invasion (SVI). The sensitivity and specificity for diagnosing EPE using ER-MRI were 62.5% and 84.6%, respectively, and 16.7% and 100% with TRUS. The sensitivity and specificity for diagnosing SVI were 80.0% and 93.8%, respectively with ER-MRI, and 0% and 92.3% with TRUS. The accuracy of predicting SVI was 90.5% with ER-MRI compared to 70.6% with TRUS. CONCLUSIONS: ER-MRI was significantly better than TRUS for determining the local extent of prostatic cancer and for prediction of SVI in the preoperative staging of clinically localized prostate cancer.

9.A Study of Production of Interleukin-1alpha by Peripheral Lymphocytes in Patients with Coronary Artery Disease.

BACKGROUND: Interleukin-1alpha is interesting lymphokine to cardiologists because it has been implicated as a regulatory protein in the development and clinical sequale of atherosclerosis, including the modulation of low density lipoprotein metabolism, the regulation of vascular smooth muscle cell mitogenesis, the stimulation of leukocyte adherence to endothelium, and procoagulant activity. But most interleukin-1alpha remains in the cytosol of cells in its precursor form, and is transported to cell surface. and associated with cell membrane. Therefore considerable amount of interleukin-1alpha, contrast to interleukin-1beta, is not released by cells into the extracellular space and the circulation. Despite of increased production of interleukin-1alpha, circulating level may not be elevated because of autocrine and paracrine action of that. In order to elucidate whether interleukin-1alpha is systematically elevated or not in patients with coronary artery disease who are complaining of chest pain, we undertook this study. METHODS: We isolated lymphocytes from peripheral blood in patients and control group. After the peripheral lymphocytes were cultured in the presence or absence of phytohemmagglutinin in RPMI-1640 media for 24 hours, we measured the content of interleukin-1alpha in supernatant by radioimmunoassay. RESULTS: 1) In the absence of phytohemagglutinin, the mean value of Interleukin-1alpha in the supernatant was 29.13+/-17.42 pmol/ml in control group and 27.28+/-18.80 pmol/ml in patients group(p=NS). 2) In the presence of phytohemagglutinin, the mean value of Interleukin-1alpha in the supernantant was 36.53+/-20.72 pmol/ml in control group and 152.13+/-91.85 pmol/ml in patient group(p<0.0001). CONCLUSION: Significant increase of interleukin-1alpha in the presence of phytohemagglutinin in the patient group means that the peripheral lymphocytes in patients with coronary artery disease are activated to produce interleukin-1alpha.

10.Treatment of Tracheobronchial Stenosis with a Self-Expandable Metallic Stents.

PURPOSE: We analysed the role of modified Gianturco self-expandable stents in the treatment of tracheobronchial stenosis in 13 patients. MATERIALS AND METHODS: We inserted modified Gianturco self-expandable stents under the fluoroscopic and bronchoscopic guidance. There were stenosis in the trachea(n--2), the right main bronchus(n=2), and the left main bronchus(n=9). The causes of the stenosis were endobronchial tuberculosis(n=10), intubation granuloma (n=l), restenosis after surgical reconstruction(n=2). RESULTS: Dyspnea or wheezing was improved within 1 or 2 days following the procedure. There were 32% and 22% respective increase in average FEV1 and FVC. Lung perfusion scan showed 9.6% increase of perfusion in the involved lung. No complications related to the procedure were encountered. During follow-up period of up to 31 months, 2 patients showed tracheal or bronchial restenosis, at 3 and 6 months, retrospectively. There was a distal migration of the stents in one case. CONCLUSION: During the follow up period after stent insertion, improvement of the obstructive changes and dyspnea persisted in 10 out of 13 patients. The modified Gianturco self-expandable metallic stents may be a good choice for the treatment of tracheobronchial stenosis, either as a primary treatment, or when the reconstruction failed.
