The transverse sacral fracture is rare; however, if it accompanies neurological injury or instability, difficult surgical treatment may be necessary. We performed surgical decompression and laminoplasty in a patient with neurological deficits and anterior displacement of S2 on S1. The patient showed a successful clinical outcome by neurological improvement.

Objective: To discuss the association between the length of stay at the intensive care unit (ICU) and sarcopenia among hemiplegic stroke patients. Methods: This study evaluated 66 hemiplegic stroke patients with history of ICU admission using handgrip strength and bioelectrical impedance analysis to obtain height-adjusted appendicular skeletal muscle mass. The diagnosis of sarcopenia was made according to the muscle mass based on the Asian Working Group for Sarcopenia. The patients were divided into sarcopenic and non-sarcopenic groups. The two groups were statistically analyzed, and the significant factors with differences were studied. A multivariate logistic regression analysis was performed to examine the association between length of stay in the ICU and sarcopenia, after adjusting for potential confounders. Results: Among 66 hemiplegic patients with an ICU admission history, 12 patients were diagnosed with sarcopenia. Sarcopenia patients showed lower scores on the Korean version of the Modified Barthel Index and the Korean version of the Mini-Mental State Examination. Additionally, patients with sarcopenia had a longer length of stay in the ICU, and univariate and multivariate analyses confirmed that the ICU length of stay was significantly related to sarcopenia (adjusted odds ratio=1.187; 95% confidence interval, 1.019–1.382; p=0.028). Conclusion The length of stay in the ICU was significantly associated with sarcopenia in hemiplegic stroke patients.

We report a case of cerebral lymphomatoid granulomatosis in a 50-year-old man. Lymphomatoid granulomatosis is an unusual condition which usually presents as a pulmonary manifestation, which occasionally affects the brain, and causes focal inflammatory lesions. Primary cerebral lymphomatoid granulomatosis is very rare. We describe a case of lymphomatoid granulomatosis in the left frontal lobe without pulmonary involvement.

Epithelial-myoepithelial carcinoma and basal cell adenocarcinoma are uncommon, low-grade malignant epithelial neoplasms of salivary gland. They occur predominantly in the parotid glands with frequent recurrences and occasional distant metastases. We report an unusual case of combined epithelial-myoepithelial carcinoma and basal cell adenocarcinoma within the same mass of the parotid gland in a 32-year-old woman. To the best of our knowledge, this is the first such a combined carcinoma case.

The purposes of the study was to examine changes in thickness and strength of the rat femur after administration of sodium fluoride in the drinking water. 48 female Sprague-Dawley rats were randomly divided into 4 groups, and they were supplied with 0(control), 1, 5 and 50 ppm of fluoride in the drinking water ad libitum for 6 weeks. Rats were killed and both sides of femur were dissected. Bone strength was measured as the stress of failure of femur at the middle point and femoral neck with Instron. Histologic slides were prepared from the femur shaft with routine processing of fixing, demineralizing, embedding and HE staining. Thickness and area of cortical bone and medullary cavity were measured by using Camera Lucida and Image Analyzer. All the collected data were analyzed with one-way ANOVA, Duncanos multiple range test for post-hoc tests and ANCOVA using the SAS 6.12 package at the level of 0.05. Bone strength increased significantly in the animals given 1 and 5 ppm of fluoride in the water, as compared to the control group. There were significant decreases of stress at fracture in 50 ppmF group compared to the 1 and 5 ppmF groups. The similar trends of bone strength at the femoral neck fracture test, but there were no statistical significances. Cortical bone thickness and area of the femur increased in the 1 and 5 ppmF groups compared to the control. However, the thickness of 50 ppmF group also decreased significantly as compared to 1 and 5 ppmF groups. On the other hand, medullary thickness and area increased in all fluoride groups than control group. All of the findings presented support the conclusion that, low fluoride dosage used in water fluoridation could increase the bone strength and might have preventive effect on femur fracture.
