1.Histopathologic Observations on Behcet's Syndrome.

The present study was performed to evaluate clinical manifestations in 26 patients and histopathological findings in 19 patients with Behcets syndrome. 1. Clinical findings: A. Among the 26 patients, 9 were male and 17 were female (male to female ratio was 1: l.9). The age at onset of the disease varied from 10 to 62, with a mean of 26.9. B. The incidence of the major clinical manifestations: The oral ulcers were observed in all of the 26 patients, genital ulcers in 21 patients (81%), cutaneous manifestations in 25 patients (96%) and ocular lesions in 7 patients (27%). C. Saline intradermal test was positive in 62% and PPD intradermal test in 42% 2. Histopathological findings: Fourty-one specimens were taken frorn 19 patients with Behcets syndrome; Erythema nodosum like lesions (15), edematous papules (3), acneiform eruption (1), oral ulcers (4), genital ulcers (8), saline intradermal injection sites (5) and PPD intradermal test sites (5). As a control, 4 specirnens were taken from PPD test sites of the patients with erythem nodosum. The common histopathological findings were perivascular inflammatory cell infiltration, mainly composed of neutrophils and nuclear dusts, and vasculitis of varying degree. A. Cutaneous lesions; In erythema nodosum-like lesions main pathologic foci were in the deep dermis and/or the subcutis, and moderate to severe vasculitis were observed. In edematous papules, however, main foci were in the pa,pill- ary dermis and the upper dermis, papillary dermal edema was prorninent, and vasculitis was rather mild.

2.Intrathecal Phenol-glycerine Injection Therapy in Treatment of Intractable Cancer Pain .

To a patient suffering from intractable pain due to the terminal state of cervix cancer, 7% phenol-glycerine solution was injected through the L2-L3 interspace. We obtained a good result of pain relief, but motor paralysis developed.

3.Dermographism ( III ): Dermographism in Acute and Chronic Urticaria.

The prevalence of dermographism in 96 patients of acute or chronic urticaria was studied using dermographic tester. The results were summarized as follows : 1. The prevalence rate of dermographism in acute or chronic urticaria was 26.0% (25/96). 2. Dermographism was more common in acute or chronic urticaria than in the general population(p<0.005). 3. There was no significant difference in the prevalence rate of dermographiam between acute and chronic urticaria. 4. The prevalence rate of dermographism was not significantly correlated with the level of serum IgE.

4.Successful Control of Extended-spectrum Beta-lactamase-producing Klebsiella pneumoniae Outbreak in a Neonatal Intensive Care Unit.

BACKGROUND: Extended-spectrum beta-lactamase-producing (ESBL) Klebsiella pneumoniae is an important cause of nosocomial infections in neonatal intensive care units (NICUs). This study aimed to reduce ESBL-producing K. pneumoniae in the NICU by using infection control measures. METHODS: We performed prospective surveillance cultures, monitoring, and education for infection control in the NICU between May and August 2011. Specimens were collected from all infants and the environment including stethoscopes, thermometers, ventilators, incubators, etc. The anterior nares and hands of healthcare workers were also screened. We inspected infection control practices and provided feedback. The level of infection control awareness was measured using a questionnaire. RESULTS: The level of awareness and performance of hand washing increased significantly after intervention (both P<0.001). The environmental management of healthcare providers also improved significantly (P=0.001). The yield of ESBL-producing K. pneumoniae from clinical specimens decreased gradually throughout the study period (30.4% in May to 12.6% in August). Central catheter-related K. pneumoniae bacteremia decreased from 1.3/1000 to 0/1000 catheter-days. CONCLUSION: Infection control measures including education, monitoring, and surveillance can lower the incidence of ESBL-producing K. pneumoniae in the NICU.

5.Comparative Study on the Epithelial Regeneration of the Corneal Ulceration after Cryocautery and Iodine Cautery.

Recently cryogenic therapy is widely used for the treatment of herpes simplex keratitis. We had experimental studies on the effect of iodine cautery and cryocautery upon the regeneration of corneal ulceration. On 10 adult albino rabbits, one eye was treated by iodine, and the other by cryocautery in area of 0.6cm X 0.6cm. The epithelium in this area was completly removed by cotton applicator. There after, the rate of epithelial regeneration was observed every 12 hours. The results were as follows: (1) The average period of complete epithelial regeneration was 48 hours in animal treated with cryocautery and 78 hours in animals treated with iodine cautery. This difference is statistically highly significant. (2) Corneal opacity was not demonstrated in eyes treated with cryocautery. Six eyes treated with iodine, however, showed various degree of corneal opacities. These results support the view that the healing time in human herpes simplex keratitis is shortened in patients with cryocautery as compared with iodine.

7.A Study on Serum IgE Levels in Various Dermatoses.

The serum value of immunoglobulin E (IgE) in 47 cases of atopic dermatitis and 43 cases of urticaria were determined by Phadebas PRIST(R) method in department of dermatology at National Medical Center, from Jan., 1982 to Sep., 1982. The results were as follow; 1. The mea,n IgE value in atopic dermatitis was significantly higher (408+/-l43 u/ml) than that of normal subjects (p>0.01). 2. The mean value of IgE in urticaria (310+/-299 u/ml) was not significantly higher than tha,t of normal subjects (p>0.05). 3. The rnean value of IgE in severely expanded atopic dermatitis (664+/-404 u/ml) was significantly increased than that of normal subjects (p>0.01) 4. The mean value of IgE in 7 cases of atopic dermatitis with allergic respiratory diseases (817+/-233 u/ml) was significantly elevated (p>0.01). 5. No close correlation was seen between IgE value and eosinophilia. 6. IgE in 2 cases of the urticaria infested with PW, CS were extended over 1000u/ml.

9.Clinical and radiological observation of osteosarcoma

Osteosarcoma is a malignant tumor arising from the undifferentiated fibrous tissue of bone and one of the mostcommon pirmary malignant tumors of bone. The diagnosis of osteosarcoma is based one can be diagnosed with areasonable degree of confidence from the conbination of the lesion site, age and radiological findings. This is aclinical and radiological observation of 40 cases of osteosarcoma which were experienced at Chonnam NationalUniversity Hospital, Chosum University Hospital and Kwangju Christian Hospital from 1978 to 1981. The results wereas follows; 1. The peak incidence was in the age group from 10 to 19 years in 21 cases (52.5%). 2. In sexdistribution, male group showed slightly higer incidence than in female group with the ratio of 1.35;1. 3. Themost commonly affected site was proximal tibia(35%). The lesion was found around the knee in 65% of the studiedcases. 4. Increment of the level of serum alkaline phosphase was observed in 25 cases (62.5%), and the averagevalue of serum alkaline phosphature in these cases was 5.6 Bessay Lowry units. 5. Radiographically, osteoblastictype was 14 cases (35%), and soft tissue mass shadow with calcification was observed in 34 cases (85%). 6.Cortical destruction was observed in 30 cases (75%), and soft tissue mass shadow with calcification was observedin 34 cases (85%). 7. Solid periosteal reaction, sunburst periosteal reaction and Codman's triangle were observedin 35%, 57.5% and 22.5% respectively. 8. At the first visit, lung meatastasis was found in 5 cases (12.5%) andinguinal lymphnode metastasis in 2 cases (5%)

10.Locally Infiltrative Glomus Tumor: A case report.

Malignant glomus tumor is a rare neoplasm arising from the glomus body, comprising categories of locally infiltrative glomus tumor, sarcoma arising in benign glomus tumor and de novo glomangiosarcoma. We report one case of locally infiltrative glomus tumor arising in nail bed of the right thumb of a 50 year-old woman. The tumor showed the architectural features of glomus tumor but cytologically bizarre, pleomorphic features without mitosis and infiltrative growth pattern to adjacent connective tissue and nerve. Immunohistochemical results were consistent with glomus tumor. This category of malignant glomus tumor must be distinguished from other round cell tumors because of its good prognosis.
