BACKGROUND: HLA-B5102 is a newly approved antigen at the meeting of the WHO Nomenclature Committee held after the Eleventh International Histocompatibility Workshop. It had been called B5l.35 because it was defined by both B5l and B35 antisera. HLA-B5102 antigen cannot be accurately determined by current commercial HLA typing trays. This study was carried out to assess the frequency of HLA-B5102 antigen in Koreans and serological reaction patterns of HLA-B5102 on commercial HLA trays. METHODS: We performed HLA-A, B, C serological typing for 2,000 Koreans registered for KMDP (Korean Marrow Donor Program) donors using the Terasaki Oriental Tray (One Lambda, USA). Selected samples (17/2000) which showed atypical B5 reaction patterns were tested against Japan Central Block HLA Workshop tray to detect the presence of HLA-B5102. RESULTS: HLA-B5102 showed a phenotype (antigen) frequency of 0.45% (9/2000) and an allele frequency of 0.23%. Two locus HLA haplotype and linkage analysis showed that HLA-B5102 was in linkage disequilibrium with HLA-A3l (p<0.01). The serological patterns of HLA-B5102 on Terasaki Oriental Tray were 1) Lot 14, 15 : B5(+), 2) Lot 15 B : B5(+), B35+53(+), and 3) Lot 16 : B5(+), B5l(+), B35+53(+), and therefore could be identified as HLA-B5, B5l, B52, B35 or B53. CONCLUSIONS: The frequency of HLA-B5102 in the Korean population (antigen frequency 0.45%, allele frequency 0.23%) is similar to that of Japanese. The presence of HLA-B5102 can be suspected when atypical BS reaction patterns are encountered in commercial HLA typing trays, and B5 or BSI had better been assigned to these cases when additional confirmatory typing is not available.

The author has observed the effects of collagenase on the relapse phenomenon and the histochemical changes during the relapse period. 50 rats were used. : 3rats as a normal group, 15rats as control groups, and 32rats as experimental groups. Rat's teeth were moved for 10days with helical spring applied, followed by injection of "collagenase in Hank's sol." to the experimental groups and the "Hank's sol." to the control group in the interdental gingiva on the 10th day, and the spring was removed on the 11th day. After injection, the experimental animals were sacrificed on the 11th, 13th, 15th, 17th, 20th, and 24th day and perpared histochemically for the Hematoylin-Eosin, Van-Gieson, and Methyl Green-pyronin staining. The results are as follows: 1. Group I (11th day): In the control group the supracrestal fibers were stretched and the metabolic rate was high. Experimental group showed that supracrestal fibers were resor, bed, disarrayed, and the metabolic rate was low. 2. Group II (13th day): In the control group, the supracrestal fibers began to vhange from the vertical direction to tooth-axis to the parallel. Experimental group showed that supracrestal fibers were completely resorbed. 3. Group IV (17th day): The control group showed almost normal structure. Form this group the metabolic rates were low. Experimental group showed the most destructive pattern. 4. Group VI (24th day): Experimental group showed almost normal structure. It follows that experimental groups were relapsed less than the control groups, and collagenase was effective in the prebention of relapse after rat's experimental tooth movement.

The purpose of this prospective study was to evaluate the diagnostic value of color Doppler scoring system in characterization of ovarian masses. We performed transabdominal or transvaginal color Doppler sonopaphy on 82 women with ovarian masses and analyzed the sonographic findings. Ovarian lesions were assessed by means of morphological and color Doppler scoring system by Kurjak. Tumors were characterized by ultrasonographic findings as benign or suspected of being malignant. Then the results of each scoring systems were correlated with histopathological findings. The results were as follows; Of 82 ovarian masses, 64 were benign lesions(13 mucinous cystadenomas, 16 endometriomas, 20 teratomas, 8 serous cystadenomas, and 13 other abnormalities), and 18 were malignant(12 cancers and 6 borderline tumors). The color Doppler scoring system was useful in distinguishing malignant from benign masses, with a sensitivity of 88.2%, compared with the morphological scoring system of 86.7%. The specificity of color Doppler and morphological scoring system were 95.4% and 92.5%, respectively. The sensitivity of combination of both scoring systems was 88.9%, with a specificity of 96.9%. In conclusion, color Doppler scoring system is a useful tool in predicting the malignancy of ovarian lesions especially combined with morphological scoring system.

Lot-to-lot reproducibility is an important issue in immunoassays and reagent lot-to-lot comparability test comparing the results of new reagent lot with those of used lot using patients' samples or controls is usually performed to detect the difference between lots. However, there are no universally used acceptability criteria regarding reagent lot-to-lot comparability test. We performed reagent comparability test between different lots of alpha-fetoprotein (AFP) reagents and tried to determine its comparability by several criteria. Both the commercialized controls and in-house controls for AFP made from pooled patients' sera were measured 10 times using the old and new lots of reagents, whenever a reagent lot was changed. The differences in the mean control values, the percent difference (% difference), and the difference to between-run standard deviation ratio (D:SD ratio) between successive lots were calculated. We compared the results of reagent comparability test to arbitrarily determined acceptability criteria suggested by CLSI C54-A. Although comparability between reagent lots was determined according to how strictly we set the criteria, some lot-to-lot differences between certain pairs of lots exceeded the criteria. We hope that the results of this study might be helpful to perform reagent lot-to-lot comparability test and set the criteria for reagent comparability test between lots in other laboratories.

BACKGROUND: Anti-hepatitis C virus antibody (anti-HCV) assays are recommended for screening HCV-infected persons. The VIDAS Anti-HCV Assay (bioMérieux, France), based on the enzyme-linked fluorescence test principle, was recently introduced in Korea. We evaluated the clinical performance of the VIDAS assay. METHODS: One hundred HCV-positive and 1,002 HCV-negative blood samples confirmed by Architect anti-HCV (Abbott Laboratories, USA) and COBAS TaqMan HCV real-time PCR (Roche Diagnostics, USA) or the Procleix Ultrio Plus Assay (Gen-Probe Incorporated, USA) were obtained from the Human Serum Bank (HSB) and tested by VIDAS. In case of discrepant results, we conducted a recombinant immunoblot assay (RIBA). RESULTS: The agreement rates for known HCV-positive and HCV-negative samples between the VIDAS assay and the HSB testing were 100% (95% confidence interval [CI]: 96.4-100%) and 99.5% (95% CI: 98.8-99.8%), respectively. One of the five discrepant samples was positive for Core 2+ and NS3-2 2+ reactivity, two samples were negative, and the other two were indeterminate regarding NS4 2+ reactivity in RIBA. We observed a significant but weak positive correlation between the titers of VIDAS and Architect assays (r=0.315, P<0.001). CONCLUSIONS: The VIDAS anti-HCV assay, developed on the VIDAS automated immunoassay platform based on the ready-to-use, single-sample test concept may be useful in small-to-medium-sized laboratories. It showed good agreement with Architect anti-HCV and COBAS PCR assays and is therefore useful for detection of HCV infection. Weakly test-positive (ambiguous) samples require additional testing by another anti-HCV, RIBA, or HCV RNA assay.

A 44-year-old woman who comlpained of dizziness and generalized weakness was admitted. The hemoglobin was 6.6g/dL, hematocrit 25.5%, and serum ferritin 2.14 ng/mL Stool occult blood was positive and microcytic hypochromic anemia was found on periyheral blood smear. Gastroscopic examination showed about 2 x 1 cm sized hemispherical sebmucosal tumor on antrum. The patient underwent operatioh for confirmatory diagnosis and treatment. The final pathologic diagnosis of the resected lesion was hemangioma of stomach. Cavernous hemangioma of stomach is a rare disease.Mostly, it has a benign course clinically, but early diagnosis is important because massive hemorrhage and anemia by chronic blood loss can occur. We report a case of hemangioma of stomach with review of literature.

PURPOSE: We wished to compare the ability of ultrasonography and radiography performed on the same day to detect rib fractures in minor chest injuries. MATERIALS AND METHODS: Two hundred and fifteen patients with minor chest injuries were selected. Radiography and ultrasonography were performed on the same day with these patients. Chest wall pain was the only presenting symptom. Two radiologists performed ultrasonography. Fractures were identified by a disruption of the anterior margin of the rib and costal cartilage. The incidence and location of fractures and complications revealed by radiography and ultrasonography were compared. RESULTS: Radiographs revealed the presence of 70 rib fractures in 50 (23%) of 215 patients and ultrasonography revealed the presence of 203 rib fractures in 133 (62%) of 215 patients. Ultrasonography uniquely identified 133 rib fractures in 83 patients. Ultrasonography identified a 2.9 fold increase in the number of fractures in a 2.6 fold number of subjects as compared to radiography. Of the 203 sonographically detected fractures, 201 were located in the rib, one was located at the costochondral junction, and one in the costal cartilage. There were no complications seen by either radiography or ultrasonography. CONCLUSION: Ultrasonography reveals more fractures than those that may be overlooked on radiography for minor chest injuries.
