1.Relationship between Knowledge and Adherence to Self-management Guidelines, and Influencing Factors of Adherence for Hemodialysis Patients.

PURPOSE: This study was to examine the relationships between knowledge with same sub-areas of education needs and adherence to self-management guidelines, and to investigate the factors that influence adherence. METHOD: One hundred eleven patients with terminal stage of kidney failure were conveniently selected from a university hospital in Incheon. RESULTS: The level of knowledge was high, with a mean score of 15.25. The level of adherence to self-management guidelines was low, with a mean score of 64.53. The patients' education needs were high. The significant influencing factors on adherence were gender, educational level, and two sub-areas of knowledge, 'exercise and daily life' and 'follow-up care and treatment'. CONCLUSION: Relationships between knowledge, education needs, and adherence should be further examined before developing and implementing intervention programs for adherence to treatment guideline in hemodialysis patients.

2.Pulmonary Edema during General Anesthesia for Cesarean Seetion of Pre-eclampsia Patient .

Our hospital has been experienced pulmonary edema during general anesthesia for cesarean section of pre-eclampsia. But, the patient recovered rapidly without complication. Therefore, we think that before anesthesia, accurate laboratory data and careful attention are needed for general anesthesia of pre-eclamsia.

3.A Case of Thelazia callipaeda Isolated from Human Conjunctival Sac.

Thelazia callipaeda is a slender, long, and cylindrical nematode which parasitizes in the conjunctival sac of human and causes conjunctivitis. The animals such as the dog, rabbit, horse, deer, and cow were revealed as its reservoir and some species of the fly suspected as its vector. We experienced a case of T. callipaeda isolated from human conjunctival sac of a 41-year old man who lived in Wanju-gun, Chonbuk province and raised the dogs. He complained of an irritation, itching and foreign body sensation on his right eye and the two worms were picked out of his right eye by forceps from conjunctival sac. General features of the worms were ivory colored and slender. Two worms were 15.2mm and 15.8mm in length and both have less than 1.0mm in maximum width. Microscopically, both of the worms were female. The vulva opening of the worms located anterior to esophago-intestinal junction. The uterus filled with the eggs an6 larvae encysted with oval membrane. The buccal cavity in head portion was tetrazoid and connected with well-developed esophagus. At the tails of the worms, anus and papillae were observed. Characteristic compact cuticular transverse striations were identified on the whole body surface.

5.Sequence Analysis of the Gene Encoding gp55 Protein of Suri Strain, an Attenuated Classical Swine Fever (Hog Cholera) Virus.

An attenuated classical swine fever virus (CSFV), Suri strain, is a va.iant derived from a vaccine virus, LOM strain. This study was performed to elucidate the molecular biologcal properties of CSFV Suri strain, and to obtain the basic data for molecular epidemiological approaches for the disease. The truncated form of gp55 gene without the C-terminal transmembrane domain, in size of 1,023bp, was amplified by RT-PCR and sequenced by dye terminator cyclic sequencing method, and inserted into BamHI site of pAcGP67B baculovirus vector, establishing a cloned pAcHEG plasmid. By the nucleotide sequences determined, 341 amino acid sequences were predicted. As compared the nucleotide and amino acid sequences of gp55 of Suri with the various CSFV, Suri strain showed the high homology over 99.1% with ALD and LOM strains, but comparably the lower homology with Alfort and Brescia. In comparison of amino acid sequence in variable domain of gp55 protein, the similar tendency of homology was observed. In hydrophobicity analysis, all of four CSFV strains revealed the analogous patterns of hydrophobicity. The numbers and locations of N-glycosylation site and cysteine residues in gp55 were analyzed, those of Suri strain being coincident with ALD and LOM strains. The results suggest that gp55 in Suri strain has the high similarity to those in ALD and LOM strains in terms of the nucleotide and amino acid sequences and the functional properties of gp55 protein..

6.Management of High Grade Cervical Intraepithelial Neoplasia Using CO2 Laser Conization and LEEP.

Between June 1990 and May 1994, 350 laser conization and 200 LEEP were performed. Indications of conization were that directed biopsy specimen was proved CIN II, III or suggests possible microinvasion. In all the cases the procedures were carried out with the patients under local anesthesia. Excisional cone sections(6,600) were evaluated for lesion length,depth and margin status. Invasive cancer was found in 5(1.4%) women of laser group. Operative time was shorter LEEP group than laser group significantly. In laser group, 25(8.3%) women had bleeding that required treatment. One case(0.3%) of pelvic infection and 7 cases(2.3%) of cervical stenosis were observed. In LEEP group, 10(5.3%) women had bleeding, 3(1.6%) patients became cervical stenosis. The diameter of bumed tissue was 0.28mm in laser group and 0.25mm in LEEP group. Success rate were 97.4% in former and 96.3% in later. This study demontrated that CO2 laser conization and LEEP were effective methods for treating high grade cervical intraepithelial neoplasia and added benefit of preserving reproductive function and rule out invasive carcinoma.

7.Dysphagia and Decannulation Following Tracheostomy in Children

Dysphagia is a common problem in children with a tracheostomy tube. Such children are at risk of malnutrition, developmental problems, increased medical complications, and pose an increased burden on their caregivers. Therefore, proper evaluation and dysphagia rehabilitation are necessary for children with a tracheostomy tube.Also, as many such children experience serious complications, it is necessary to consider decannulation as early as possible, soon after the indication for tracheostomy placement is resolved. Decannulation should be performed safely as per the appropriate protocol.

BACKGROUND: We evaluated the performance of the Rapidlab 860 (Chiron Diagnostics, Medfield, MA, USA), the newly introduced automated blood gas&electrolytes analyzer, according to the guidelines of the National Committee for Clinical Laboratory Standards. METHODS: The tested analytes were pH (hydrogen ion activity), PCO(2) (carbon dioxide pressure or tension), PO(2) (oxygen pressure or tension), Na(+) (sodium), K(+) (potassium), Ca(++) (ionized calcium), Cl(-)(chloride), HCO(3)(-)(bicarbonate), ctCO(2) (concentration of total carbon dioxide). Within-day precision, between-day precision, and sample-related carryover were analyzed using quality control materials at three different concentration levels for each analyte. Correlation was compared with the routinely used NOVA-Stat Profile 5 (Nova Biomedical, Waltham, MA, USA) with patient's whole blood samples. RESULTS: The within-day coefficients of variations (CVs) for pH, PCO(2), Na(+), K(+), Ca(++), HCO(3)(-), and ctCO(2) were less than 5.0%. The within-day CVs for PO(2) and Cl(-) at low concentration levels were 7.128% and 5.065%, respectively. The between-day CVs for each group were less than 5.0% in all analytes. Sample-related carryover was less than 5.0% in all items. Coefficients of correlation exceeded 0.950 except for PCO(2) and HCO(3)(-). CONCLUSIONS: Rapidlab 860 showed good precision, close correlation with NOVA-Stat Profile 5 and sample-related carryover was barely present for pH, PCO(2), PO(2), Na(+), K(+), Ca(++), Cl(-), HCO(3)()-ctCO(2).

9.A case of fatal hyperinfective strongyloidiasis with discovery of autoinfective filariform larvae in sputum.

The autoinfective filariform larva of Strongyloides stercoralis causes hyperinfection in immunosuppressed hosts. Here we report on the case of a male patient who was admitted to the emergency room at Gwangju Veterans Hospital with a complaint of dyspnea, and who was receiving corticosteroid therapy for asthma. Many slender larvae of S. stercoralis with a notched tail were detected in Papanicolaou stained sputum. They measured 269 +/- 21.2 micrometer in length and 11 +/- 0.6 micrometer in width. The esophagus extended nearly half of the body length. The larvae were identified putatively as autoinfective third-stage filariform larvae, and their presence was fatal. The autoinfective filariform larva of S. stercoralis has not been previously reported in Korea.

10.Comparison of Mitomycin C Trabeculectomy and Ahmed Valve Implant Surgery for Neovascular Glaucoma.

PURPOSE: To compare the results of mitomycin C trabeculectomy and Ahmed valve implant surgery for neovascular glaucoma (NVG). METHODS: We reviewed the medical records of 58 patients (66 eyes) with NVG who had undergone mitomycin C trabeculectomy or Ahmed valve implant surgery in the previous 5 years. Trabeculectomy (A group) was performed in 27 eyes of 24 patients and valve implant surgery (B group) in 39 eyes of 34 patients. Intraocular pressure (IOP), visual acuity, complications and the rate of surgical success of the two groups were compared. IOP under 21 mmHg without medication was considered as complete success (CS), under 21 mmHg with medication as qualified success (QS), and IOP over 22 mmHg despite medication as failure (F). RESULTS: IOP at postoperative 2 months was lower in the trabeculectomy group than in the drainage implant surgery group (16.4 +/- 11.3 mmHg, 20.9 +/- 8.7 mmHg, P=.087), but at 12 months, there was no statistically significant difference (18.4 +/- 11.5 mmHg, 17.0 +/- 6.4 mmHg, P=.608). Visual acuity also showed no significant difference. The respective rate of success between A and B groups was 66.7% and 38.5% (CS), 25.9% and 33.3% (QS), and 7.4% and 28.2% (F), at postoperative 2 months, and 47.8% and 42.3% (CS), 30.4% and 46.2% (QS), and 21.7% and 11.5% (F), at 12 months. The most common postoperative complication was hyphema in both groups, while other complications were shallow anterior chamber, valve problems, etc. CONCLUSIONS: The type of surgery had no significant effect on the success rate of neovascular glaucoma treatment at postoperative 12 months.
