Thermography is a sensitive and reliable method for diagnosis of radiculopathy. Skin temperature alterations of the involved dermatome named thermatone are diagnostic for sensory root involvement in radiculopathy which can be demonstrated by thermography. Digital infrared thermographic imaging system using computer is development and could measure thermal difference more exactly without hazards or discomforts to patient. Authors present 186 cases of thermographic evaluation in herniated lumbar disc disease by digital infrared thermographic imging system and the results are evaluated with literature review.

3.MRI Study about the Early Changes of Lumbar Disk Degeneration using Magnetization Transfer Contrast (MTC).

PURPOSE: To obtain magnetization transfer ratio(MTR) of the annulus fibrosus and nucleus pulposus and to assess the feasibility of utilizing the changes of these MTRs as an early indicator of disk degeneration. MATERIALS AND METHODS: MR images of lumbar spine with magnetization transfer(MT) technique in 42 patients were obtained. spin echo techniques (600/14) with same TR/TE with 1KHz off-resonance saturation were employed in 1.0T MR system. MTRs were calculated in two regions, anterior annulus fibrosus and nucleus pulposus, and the results were compared between the normal and degenerative disks, from grade I to IV, on T2-weighted images. RESULTS: MTRs of the nucleus pulposus were 17.6% in the normal disks, and 26.7%, 28.4%, 29.1%, 29.7% in degenerative disks, from grade I to IV, respectively, with a significant difference(P<0.05). On the other hand, MTRs in the annulus fibrosus were 30.2% in the normal disks and 31.5%, 33.2%, 32.1% and 35.6% in degenerative disks, from grade I to IV, respectively, without significant difference. CONCLUSION: Since MTRs are significantly higher in degenerative nucleus pulposus than those of the normal disks, increased MTRs in the nucleus pulposus can be used as an early sign of the degeneration of the nucleus pulposus.

4.Spinal Fusion with B.O.P.(Biocompatible Osteoconductive Polymer).

Spinal fusion has performed for instability and anatomical reconstruction since 1985 by Barthe. Bone grafts and synthetic materials has been used for spinal fusion, but they have several limitations and complications. Recently a new synthetic polymer B.O.P.(Biocompatible Osteoconductive Polymer) was developed and it overcome the limitations of other materials. The B.O.P. showed no foreign body reaction and gave scaffolding for the osteoconduction and osteointegration. Authors operated 35 cases of spinal fusion with B.O.P. and the results and literature reviews were discussed.

Anterior interbody fusion has used for instability and anatomical reconstruction in various cervical diseases since 1958 by cloward. Bone grafts such as autograft, allograft, xenograft and synthetic materials were utilized in fusion as a graft material. But conventional fusion materials have problems including postoperative morbidity, transmission of diseases, foreign body reaction, collapse, prolongation of operation time. A new synthetic material, Biocompatible Osteoconductive Polymer(B.O.P) is developed and it was useful for cervical anterior interbody fusion as a substitute for other fusion materials.

7.Intramedullary Spinal Hemangioblastoma Associated with Syringomyelia.

Intrameduallary spinal hemangioblastoma is frequently associated with syringomyelia. It grows slowly and can be removed totally. Syringomyelia can be subcided by total removal of tumor and opening of syringomyelia. Two cases of intramedullary spinal hemangioblastomas associated with syringomyelia are reported. Intramedullary tumor and syrinx was easily diagnosed by magnetic resonance imaging(MRI). They are successfully managed by total removal of tumor and opening of syrinx. Patients showed improved neurological status after operations.

8.Transpupillary Silicone Oil Removal through a Posterior Capsulorhexis and Intraocular Lens Implantation.

PURPOSE: To evaluate the safety and efficacy of transpupillary silicone oil removal using posterior capsulorhexis (PCCC) combined with phacoemulsification and intraocular lens (IOL) implantation. METHODS: We retrospectively reviewed 25 eyes of 25 patients who had undergone silicone oil removal through PCCC from December 1998 to August 2004. IOLs were placed in the bag or in the sulcus after the silicone oil was removed using PCCC. Preoperative and postoperative best corrected visual acuity, refractive error, intraocular pressure (IOP), silicone oil removal time, slit lamp examination, and specular microscopy were evaluated. RESULTS: After the surgery, visual acuity improved in 23 eyes (92%) and a refractive error deviation of 0.5 diopter or more compared with targeted refractive error was found in 13 eyes (52%). Re-detachment of the retina occurred in 3 eyes (12%), increased IOP was observed in 4 eyes (16%), and transient corneal edema in 2 eyes (8%). Endothelial cell count decreased significantly after the operation (P<0.01). However, this change was not statistically significant compared to that of the control group, where only the phacoemulsification was performed. CONCLUSIONS: Silicone oil removal through PCCC after phacoemulsification may be one of the best surgical options for patients who need cataract surgery along with silicone oil removal, provided that the retina is stable and no additional procedures such as membrane peeling, are necessary.

9.Invasive Ductal Carcinoma vs. Invasive Lobular Carcinoma: Mammographic Findings.

PURPOSE: The purpose of this study is to evaluate mammographic findings of invasive ductal carcinoma(IDC) and invasive lobular carcinoma(ILC)and to find differential points between the two. MATERIALS AND METHODS: 239 patients, who underwent mammography prior to surgery and were proved to have IDC(224 patients) or ILC(15 patients)pathologically, were analized retrospectively. On mammogram, presence of mass and micro calcification were analized. When there was a mass on mammogram, lesion opacity was classified into high, equal, or low opacity andborder of the mass was classified into spiculated, poorly marginated, and well-marginated. When there was nodefinite mass, mammographic findings were classified into asymmetric opacity and no mass. RESULTS: Masses were observed in 168 patients(75%) of IDC and 12 patients(80%) of ILC. Border of the masses were spiculated(n=50,22.3%), poorly marginated(n=112, 50%), or well-marginated(n=6, 2.7%) in patients with IDC. Spiculated and poorly marginated borders were observed in 8 patients(53.3%) and 4 patients(26.7%) respectively, in patients with ILC. Microcalcifications were seen in 88 patients(39.3%) of IDC and 2 patients(13.3%) of ILC. Equal or low opacities ofthe lesions were observed in 29 patients(17.3%) of IDC and 5 patients(33.3%) of ILC. CONCLUSION: Although equalor low opacities were observed more frequently in ILC and microcalcifications were noted more frequently in IDC,it was difficult to differentiate the two diseases based on mammographic findings.
