2.Clinical Observation on Renal Cell Carcinoma.

Clinical observation was done on 40 cases of renal cell carcinoma, who have been admitted to the Department of Urology, Kyung Hee University Hospital from January, 1978 to January, 1988. The results were obtained as follows. 1. Among 40 patients, there were 22 men and 18 women, with peak incidence at the sixth decade. 2. Hematuria was the most frequent symptom at the presentation followed by flank pain and abdominal mass. 3. On laboratory examinations, 4 patients had anemia and 2 patients had erythrocytosis. Among 7 patients with hypercalcemia, 2 patients had bone metastasis. 6 patients showed abnormal liver function and liver function became normalized after nephrectomy in 5 of them. 4. Comparing to pathologic stage, accuracy of CT scan in determining of stage was 82.3 %, 5. 3 year-survival rate was 75% in patients with stage I, and 33% in stage II, 40% in stage III. 6. In 14 patients, there were metastasis to lung, lymph node and bone in order of frequency.

3.Clinical Observation on Renal and Perinephric Abscess.

Renal and perinephric abscess are uncommon diseases with a multitude of clinical presentations. In past morbidity and mortality were high rate because of delay in diagnosis. Diagnosis and treatment approaches were challenged recently by newer imaging modalities as ultrasonography and CT scanning and by interventional uroradiology. A clinical observation was made in 9 patients with renal abscess and 1O patients with perinephric abscess admitted in the Dept. of Urology, Kyung Hee University Hospital during the period from Jan. 1981 to Jan. 1986. The following results were observed: 1. Among 19 patients, 11 patients were women and their age was ranged from 22 to 68 yrs old with mean of 44.5 yrs. In 12 cases, Lt. kidneys were involved. 2. In 5 patients who had positive urine culture, E. coli was recovered in 3 cases, and klebsiella sp. and staph. epldermidis were recovered in one case, respectively. Blood culture was positive in one case. Among 14 cases performed pus culture, bacteria was recovered in 8 cases: E. coli in 4 cases staph. aureus in 2 cases, and staph. epldermidis and peptococcus in one case, respectively. 3. In 15 cases, diagnosis could be confirmed by ultrasonography, but CT scan was needed in 4 cases to confirm the nature of lesion. The common findings of ultrasonogram were mixed echogenic mass and echolucent mass found in 8 cases and 7 cases, respectively. 4. Among 19 cases, 1O patients underwent open drainage, 5 cases were treated by antibiotics only, and 3 cases were managed successfully by closed drainage.

4.The Assessment of Proliferating Cell Nuclear Antigen (PCNA) in Cervical Tumors.

Proliferating cell nuclear ntigen (FCNA) iis a nuclear protein that is syntheaimd in late Gl and S phases of cell cycle and is correlated with the cell proliferative stale. The recent study demonstrated that FCNA functions in 13NA replication. The present study evaluated proliferetive iindices (PI) for the assessment of tumor proliferation and for investigating prognostic significancx, in cervical tumors. lmmunohiatoehemical PCNA staining was perfurmed in formalin-fixed paraffin-embedded cervical tissues via the avidin-biotin-complex immunoperoxidase methad. Mean PI was 36.03+/-5.14% in normaI controls, as compared to 66.19+/-11.36% in cerviml intraepithelial neoplasia. and 63.19+/-10.94% in invasive cervical cancer. Our results showed no significant correlation between Pll and histological type. Among invasive cervical cancer (24 cases), PI waa 64.43+/-10.94% in squamoua cell carcinoma and 59.00+/-4.10% in adenocarcinoma. There was no eipiifiant relationship between Fl and clinical etage, and between PI and lesion size. This study auggeste that Pl may not serve as a new prognostie factor in cervical tumors.

Simulated three-dimensional (3D) imaging represents reformation of conventional sectional imaging data into a series of images that closely resemble the original studied structure. We tried to make 3D mucosal surface images of the sinonasal cavities, pharynx and larynx, and evaluated the feature of normal anatomy in 28 subjects. In the sinonasal cavities, 3D imaging was capable of demonstrating the inner wall of paranasal sinuses and its openings, and general status status of the nasal cavity. In the nasal cavity. In the nasopharynx, 3D imaging provided an easy concept of sectional images as 3D picture and displayed anatomic subsites and lesions comparable to that in fiberscope. In addition, 3D imaging had advantages in overcoming the technical limitations in fiberscope. In the larynx and hypopharynx, 3D imaging gave a 3D concept of the laryngeal structures and presented additive information not seen in axial iamges thus enabling access to regions beyond the scope of fiberscope. In conclusion, 3D imaging allows an easy conceptualization of transaxial CT images in complex anatomic areas and provides additional in formations undetectable in transaxial CT. We believe that the spaces of the sinonasal cavities, pharynx and larynx would be a new field of application of 3D image.

7.Analysis of p53 Protein Overexpression in Invasive Cervical Cancer.

The p53 gene is a tumor suppressor gene and mutations in this gene play an import-ant role in the development of many human malignancies. The purpose of this study was to investigate the overexpression of p53 protein as a prognostic factor in invasive cervical cancer. Forty-three formalin fixed, paraffin wax embedded blocks of invasive cervical can-cers were examined using immunohistochemical staining with a monoclonal antibody against p53. The result were as follows : 1. Immunostaining for p53 consistent with overexpression was seen in 23.8%(5 of 21) of stage I cancers and in 13.7%(4 of 22) of stage II cancers. 2. Immunostaining for p53 consistent with overexpression was seen in 21.6%(8 of 37) of squamous cell carciomas and in 0%(0 of 6) of adenocarcinomas. 3. The incidence of p53 overexpression was 25.0%(1 of 4) in cases with lymph node metastasis, compared with 17.9%(7 of 39) in cases without lymph node metastasis. 4.. The incidence of p53 overexpression was 20.0%(8 of 40) in cases with less than 4 cm, compared with 0%(0 of 3) in cases with equal to or larger than 4cm. In conclusion, p53 overexpression was not associated with stage, histologic type, and tumor size. However, there were trend for p53 overexpression to increase in patients with lymph node metastasis.

8.Analysis of Tumor Angiogenesis in Invasive Cervical Cancer.

OBJECTIVE: Tumor angiogenesis is believed to conelate with tumor growth, progression and metastasis. Studies of angiogenesis in breast, prostate and melanoma have shown that angiogenesis, the induction of new capillaries and venules, is associated with tumor metastases and recurrences. The purpose of this study was to investigate the angiogenesis as a prognostic factor in invasive cervical cancer. METHODS: Forty-three formalin fixed embedded blocks of invasive cervical cancers were examined using immunohistochemical staining with a monoclonal antibody against factor VIII-related antigen. RESULTS: The miaovessel counts were 53.50+/-20,07 in patients with lymph node metastasis, and 45.97+/-28.12 in those without such metastasis. There was a trend for the microvessel count to increase with lymph node metastasis. However, thae was no significant difference in microvessel counts regarding node status. There was no significant difference between microvessel counts in patients with stage I(47.90+/-25.89) and those with stage Il(45.50+/-29.27), The microvessel counts in squamous cell carcinoma(46.54+/-27.79) were not significantly different from those in adenocarcinoma(47,50+/-27.05), The microvessel count in patients with tumor size >-4 cm(53.00+/-21.17) was not significantly higher than in those with tumar size <4 cm(46.20+/-27.94). CONCLUSION: There was no significant correlation between microvessel counts and clinical stage of disease, pathological type, tumor size or lymph node metastasis in patients with invasive cervical cancer. There was a trend for the microvessel count to increase with lymph node etastasis.

9.Small cell carcinoma of pancrease.

Small cell cancer commonly occurs in the lungs, but it is rarely found in other organs. Small cell cancer that occurs in the pancreas is rare and its progress very rapid, and most patoents' tumor has already spread by metastasis when it is discovered. Although this cancer has a poor prognosis, we report here on treating case with combined curative resection and chemotherapy and showed a good prognosis.

10.Role of 3-D CT Reconstruction of Laryngeal IVlucosal Surface in Preoperative Staging of Laryngeal Cancer.

PURPOSE: CT or MRT is performed in preoperative staging of laryngeal cancer. These methods are used in assessment of the deep tissues and cartilage of the larynx, but cannot compete with laryngoscopy in the evaluation of the laryngeal surface. The purpose of this study is to evaluate feasibilty and clinical value of the 3-D reconstruction of the mucosal surface in laryngeal cancer. METHODS AND MATERIALS: Twenty two patients with laryngeal cancer proved by means of surgical.exploration (pathologic) or clinical examinations including laryngoscope, imaging studies and biopsy underwent preoperative staging with computed tomography(G-P);and three dimensional(3D) CT reconstruction. The TNM classification of the American Joint Committee on Cancer was used to compare the imaging findings with pathologic or clinical staging. RESULTS: When the extension of primary tumor(T staging) was evaluated, the findings at only transaxial CT and those at pathologic or clinical examination were concordant in 8 of 14 cases(57.1%) of supraglottic tumor, and 3 of 6 cases(50%) of glottic tumor. The overall accuracy of CT with additional 3D-reconstruction was 85.7% for assessment of supraglottic tumor, and 66.6% for glottic tumor. CONCLUSION: 3D CT reconstruction after transaxial CT may improve outcome in preoperative staging of laryngeal cancer and has a potential value in guiding management decisions.
