1.Study on the Blood Picture of Various Dermatoses.

So far today the blood Picture of specific dermatoses such as leprosy, syphilis, skin tubercuIosis, pemphigus and eczema was studied a good many ways. Author has studied the blood picture especially red blood cells, hemoglobin, hematocrit, white blood cells and its differential counts in 217 cases of various skin diseases such as contact. dermatitis, urticaria, chronic eczema, atoPic dermatitis, seborrheic dermatitis, erythema multiforme, erythemanodosum, drug eruption, prurigo, lichen urticatus, exfoliative dermatitis and purpura. At the same time liver function tests were performed in 46 cases including 25 cases of contact dermatitis, 6 cases of chronic eczema, 5 cases of atopic dermatitis, 5 cases of urticaria and 5 cases of exfolistive dermatitis. Afterwards the correlations of those two groups were investigated. Among all mentioned above dermatose except purpura no changes of red blood cells, hemogiobin and hematocrit were observed according to the authors own control of normal values which were obtained in 62 normal healthy adults. Other findings are as follows; 1. In contact dermatitis leucocytes and eosinophils are increased while neutrophils slightIy decreased. 2. Moderate increase of lymphocytes in urticaria. 3. In chronic eczema leucocytes and lymphocytes slightly increased but neutrophils somewhat decreased. 4. In atopic dermatitis leucocytes and eosinophils increased and there was also moderate increase of lyrnphocytes. 5. In seborreheic dermatitis monocytes and basophils were increased but eosinophils were slightly increased. 6. In erythema multiforme monocytes were decreased and leacocytes increased slightly. 7. In erythema nodosum neitrophils increased and lymphocytes and monocytes were decreased. 8. In drug eruption monocytes were decreased. 9. ln Prurigo eosinophils were increased. 10.:ln lichen uiticatus neutrophils were decreased. 11. In exfoliative dermatitis there are marked increase of leucocytes and eosinophils and decrease of neutrophiIs, lymphocytes and basophils. 12. In purpura red b!ood cells, hemoglobin, hematocrit were decreased. The increase of white blood cells in contact dermatitis and exfoliative dermatitis almost paralleled with the extension of their lesions. Marked liver function disturbarces were found in a good many cases cf exfoliativedermatitis while slight degree of liver function disturbances were found in some cases of contact dermatitis and it was found that some cases of chronic eczema, urticaria and atopic dermatitis gave some influence to liver function. The fact of leucocytes increase in exfoliative dermatitis and contact dermatitis seems more or less go parallels with liver function disturbances.

2.A Case of Pellagra with Psychoneurotic Symptoms and Neurologic Disturbances.

The authors experienced a case of typical pellagra in 45-year old male. He has been a heavy alcoholic history about 30 years duratian. Well-marginated erythematous created scaly patches, hyperpigmentation and fissures developed on both forearms and both ear lobes, both periorbicular areas, and Casals and klace on the aa0erior neck repeatedly. The skin leaions were aggravated in summers, improving a little in winters from 3 years ago. The psychoneurotic symptoms, neurologic disturbances, and GI troubles accompanied the skin lesions. Diagnosis was made by the characteristic clinical findings. He was treated with nicotinamide and vitamin B complex, and most of symptoms improved much. Authors reported the case and reviewed the literatures.

3.Quantitative Changes of Immunoglobun Levels in the Serum of Allergic Dermatoses Patients.

With development of immunochemical methods for anlysis of serum proteins, quantitative determination of serum immunoglobulin levels and purification of them have been made possible in recent years. There are numerous reports about immunoglobulin levels in various diseases. A search of the literature reveals very few reports of quantitative immunoglobulin determinations in allergic dermatoses. The present study was conducted to find quantitative changes of serum immunoglobulins in a few allergic dermatoses. To quantify the IgG, IgA and IgM, one hundred and two sera were analysed form 30 normal control group, 30 contact dermatitis patients, 12 stopic dermatitis patients and 30 urticaria patients by modified Oudin's capillary tube method. The results are as follows. 1.In contact dermatitis the IgG was significantly increased to compare with control group(p<0.01) 2.In atopic dermatitis the IgA was significantly increased to compare with control group(p<0.01) 3.In urticaria the IgG was increased (p<0.05) 4.The IgM has no differences between allergic dermatoses and control group.

4.The Findings of Cerebrospinal Fluids in 338 parients with Syphilis.

We examined cerebrospinal fluids of 338 patients with syphilis who underwent the routine physical examinations or visited out-patient skin clinic at Kyunghee University hospital from January, 1978 to December, 1979. They were diagnosed as syphilis only with serological tests such as VDRL and TPHA tests. The patients did not have any neurologic symptoms associated with syphilis or any other clinical syphilitic syrnptoms. They did not have any history of receiving antisyphilitic treatment. The VDRL test, cell count, protein value, sugar and chloride levels in cerebr-ospinal fluid were tested in these patients. The results were as follows. 1) CSF VDRL reactive rate was 2. 1% among 388 patients with syphilis. 2) CSF celI count was not less than 5/mm in 2 patients (28. 6%) among the 7 patients with syphilis, who showed the positive reactivity of CSF VDRL, and in 13 patients (3. 9%) among the 331 patients with syphilis who did not show the positive reactivity of CSF VDRL. 3) CSF protein level was more thnn 45ml/dl in 3 patients (42. 9%) among the 7 patients with syphilis who showed the positive reactivity of CSF VDRL anct ig, 65 patients (19. 6%) among the 331 patients with syphilis who did not show the positive reactivity of CSF VDRL. 4) Both CSF sugar and chloride levels were within norrnal limits in all of the 7 patients who showed the positive reactivity of CSF VDRL.

5.A Clinical Study of 217 Cases of Vitiligo.

Vitiligo is an acquired, disfiguring skin disease characterized by the loss of melanin, which results in depigmented areas of the skin. It affects at least 1% of the world's population. In countries where people are dark skinned, vitiligo is a major health problem. The pathogenesis of vitiligo has not been fully elucidated, but three possible origins, autoimmune, neurohumoral and self destruction of melanocyte, have been described. The clinical manifestations of vitiligo are well described in many reports. However in Korea, a few studies on the clinical features of vitiligo have been performed. Authors studied 217 Korean cases of vitiligo who had been visited at the Department of Dermatology, Kyung Hee University Hospital during the last 1 year (1978.4.1~1979.3.31). (countinued..)

6.Syphilis Prevalence in Young Men who want Oversea Employment.

Serologic tests for syphilis, including the qualitative and quantitative VDRL test and TPHA test, were carried out on 18, 151 healthy young men, who took a medical check-up for the overseas employment at Kyunghee University Hospital from April, 1978 to February, 1979. The syphilitics who revealed TPHA reactive, were given a questionare that dealt with three items. The results are summarized as follows. 1) The reactive rate of VDRL test was 3. 1% among 18, 151 healthy young men, 2) The biologic false positive rate of VDRL test was 7. 7% among the 520 men in whom TPHA test was carried out, using TPHA as standard. 3) The reactive rate of TPHA test was 2. 7% among 18, 109 men. On 42 men the TPHA test was not carried out. 4) With regard to the VDRL quantitative test, the reactive rate of the group with a titration of 1: 4 or lower was 85,2% out of 480 syphilitics. 5) In 480 syphilitics, 45. 8% (220) had a history of venereal 72.3%(347) had no general knowledge of syphilis, and 86. 2% (414) was unaware of his syphilitic infection.

7.Two Cases of Leukemia Cutis.

Two cases of leukemia cutis associated with leukemia has been demonstrated. Case 1: 24 year old man had been fever, arthralgia and leproma like hard infiltrated nodular lesion with hemorrhagic macules was noted on face and upper arms and trunk, Monocytic Jeukemia has been presented. In peripheral blood and bone marrow finding, immature blood cells was counted in about 80%, Case 2: 64 yearold woman, had been erythematous papule with itching on lower extremities and coin ized ulcerated nodule was noted on lumbosacral region. Myeloid leukemia has been presented. In peripheral blood and bone marrow finding, immature blood cells was counted in about 7.2%. Diagnosis of cutaneous lesions was by clinical apperance and histopathological change. By laboratory examination, monocytic leukemia and myeloid leukemia was revealed in peripheral blood and bone marrow preparation.

8.Immunoelectrophoretic Analysis of Blister Fluids.

The proteins, especially the 19S globulin (IgM) and 7S globulin (IgA) in the CO2 snow-induced blister fluids of 10 male healty volunteers were analyzed immunoelectrophoretically. The fnllawing results were obtained. 1) Compared with autologous serum, all blister fluids tested lacked some protein fraction lines. 2) The 19S globulin (lgM) line was absent in all of the blister fluids except one which was contaminated with blood. 3) The 7S globulin (IgA) line was present in all of the blister fluids when undiluted. In 1 : 2 dilution, however, it was absent, or of trace amount in a few cases.

9.Effect of Some Immunosuppressive Agents on the Survival of Experimental Skin Homografts in Rats I. Effect of Total Body Irradiation, Imuran, Prednisolone and Splenectomy.

Several immunosuppressive agents were used to prolong the survival of full-thickries skin homografts in albino rats. The grafts were squares in size of 2.0cm. The results were as follows. 1. The mean survival time of skin homografts in control group was 6.0 days. 2. Total body X-irradiation (500r) to the recipients 24 hours before grafting resulted in prolongation of survival of the grafts. (Mean, 12.5 days) 3. Administration of Imuran(azathioprine) to the recipients in a does of 5mg/kg/day for 7 days. before grafting resulted in prolongation of survival of the grafts. (Mean, 9. 6 days) 4. Spleneetomized homografted rats showed no prolongation of survival of the grafts. (Mean, 6.1days) 5. Administration of prednisolone to the recipients in a does of 2mg/kg/day for 7 days before grafting resulted in no prolongation of survival of the grafts. (Mean, 6.3 days) 6. Above results were discussed and compared with those of others.

10.A Case of Lupus Vulgaris on the Patella Region of the Right Knee Joint.

A case of lupus vulgaris who has the typical skin lesion on the patella region of the right knee was presented. The patient is 9 years old, the generally healthy male, and is no special finding of chest X-ray but tuberculin test was strong positive. The skin lesion started about 2 years ago as baby's palm sized erythematous patch with apple jelly nodule on the right knee joint region. Diagnosis was made by characteristic of clinical appearance and histulagial finding including other laboratory data. The treatment was done with INAH, PAS, streptomycin and pyridoxine for 3 months. Now,The skin lesion is almost healed and still continue treatment and observation go on.
