1.Clinical Studies on the Giant Cell Tumor of Bone

The giant cell tumor is an rather common primary bone tumor of young adult 20 to 40 years of age, most frequently found around the knee joint and the distal end of the radius. So the normal life expectancy and the active life style of patient necessitate a reliable and durable surgical remedy for eradicating the tumor and preserving joint function. Although almost are benign, sometimes these tumors are locally invasive and tend to have a high rate of recurrence, and malignant transformation is well known. Thus, a satisfactory method of treatment has not yet be found. Clinical observation were carried out on 44 cases of giant cell tumor of bone in the department of Orthopedic Surgery, Yonsei University College of Medicine from January 1981 through November 1991. Mean follow-up period was 2 years and 9 months. The age incidence was most frequent in third decade (50.0%), with male predominence as a whole. Usual location of the lesion was around the knee(70.5%). The recurrence rate for total cases was 25%: 46.2% after curettage and bone graft ; 25% after curettage and bone cement filling : no recurrence after segmental resection and replantation or amputation. We analyzed the recurrence rate according to the Jaffe s pathologic grades. It looked like higher incidence of local recurrence with higher grade. But due to the small number of patients and short follow-up, we need more cases and follow-up. The local recurrence was noted even after 2 years postoperatively, long term fol low-up is need to find it.

2.Segmental Resection and Rotationplaty for Malignant and Aggresive Benign Bone Tumors about the Knee

A segmental resection and rotationplasty was performed in 23 patients with malignant and aggressive benign tumor of the distal femur or proximal tibia between February 1988 and September 1992 at the Severance Hospital. There were 13 male and 10 female patients. The yongest was 5 years old and the oldest was 37 years old. Mean age at operation was 26.2 years. Of the 23 cases, 14 were osteosarcoma, 7 were giant cell tumor and 2 were synovial sarcoma. After an average observation period of 32.5 months, there was no evidence of local recurrence but 6 distant metastases were developed. According to Shriner's rating scale, the functional result were excellent or good in all cases. All patient were convinced that they had chosen the appropriate operative procedure and the ability to achieve high level of activity was considered as the main benefit.

3.Range of Active Motion and Axial Angles of the Wrist Joint in Normal Adult Korean

With a ordinary goniometer, we measured the range of active motion of the wrist flexion, extension, ulnar and radial deviation in 120 normal adult subjects ranging in age from 20 to 49 years old. Normal limits were determined for the male and female groups, for the age groups, and for the right and left sides, respectively. The data were compared with the axial angles in roentgenograms that described by T.E. Keats in 1966. And we have undertaken a study of the correlation between the range of motion and the axial angles. The results were as following: 1. Range of active motion (degrees±standard deviation) Rexion; 73.9 ±6.386. extension; 65.9 ±5.164 unlar deviation; 28.3 ±5.124 radial deviation; 18.6±2.556 2. Axial angles (degrees±standard deviation) P-A view;73.8±3.985 lateral view;79.7±4.777 3. Males had a smaller range of motion than female in wrist joint. 4. The right side showed a smaller range of motion than the left. 5. There were no correlations between the range of motion and the axial angles.

4.A Clinical Study of the Malgaigne Fractures

The incidence of pelvic fractures with or without dislocation of the sacro-iliac or symphysis pubis, is continually increasing. Such injuries not only are serious but can result in disabling complications and deaths. The author reviewed 57 cases of Malgaigne fractures treated at the orthopedic department of Kyungpook National University Hospital during the period from January 1978 to December 1977, and analized as to the cause of injury by mechanical forces, type of fracture, average duration for treatment and complications. We have adopted the classification of unstable fractures of Dunn and Morris. The fractures were classified as followes: vertical shear, 11: bucket handle, 8: lateral compression, 6: straddle, 5: total pelvic disruption, 4: dislocation, 3. Twenty-four patients were male thirteen, female. Traffic accident caused the injury in twenty-seven of the patients, accident of mine and factory, each three. Two fell from heights. Frequent associated soft tissue injuries were rupture of bladder (6 patients) and rupture of urethra (10 patients) The principle of treatment consisted of skeletal traction through the lower limb (supracondyle of femur) on the involved side and balance skin traction on the normal side. Pelvic sling were used. In lateral compression type, reduction was obtained by application of lateral skeletal traction with pelvic screw. Traction was continued for eight weeks. After discarding the traction physiotherapy start in bed and continued for further four weeks. Complete reduction of the displaced fragment and an adequate period of immobilization in recumbency is necessary to obtain the best results in treatment of Malgaigne fractures.

7.Clinical Study on Ipsilateral Fracture of The Femur and Tibia

Twenty-six cases of fracture of the femur and tibia on the same leg in twenty-five patients were treated at the Dept. of Orthopedic Surgery, Kyungpook National University Hospital during the period of 1973 to 1979. Twenty-two patients were sustained by the motor vehicle accident. Open fractures of the femur and tibia were ten cases, and closed femur fracture and open tibia fracture were twelve cases. Concomitant Injuries were brain Injury in eight patients and hemorrhagic shock In seven patients. Eight patients were treated by internal fixation (Kuntschernall or Compression plate) on femur and by conservative treatment on tibia. Conservative treatment was done in eleven patients on both femur and tibia fracture. Five patients were amputated. Average healing time of fracture was: twenty-two weeks in femur and twenty-seven weeks in tibia. Functional end results were assessed and rated with satisfactory results In six patients and fair and poor in elght patients respectively.

8.A Case of glomor on the Thumb

Authors have treated a case of glomus tumor which was occurred in the tip of the right thumb, 34 years old female. The patient has complained of pain, tenderness, and cold sensitivity on the lateral side of the tip of the right thumb for 10 years. Reddish purple discolorarion was noticed under the nail bed. On X-ray, erosion of the lateral cortex of the distal phalanx and increased soft tissue density around the erosion was noted. Grossly, the tumor was purple in color, more deeply colored than the surrounding tissue and tumor tissue will shell out of the surrounding bone and soft tissue. Glomus tumor was confirmed by the excisional biopsy.

9.Clinical Study on the Femoral Shaft Fracture in Adult

The steadily increasing frequency of car accidents and industrial injuries have contributed to increasing numbers of injuries to the femoral shaft. Fractures of the femoral shaft have always created camplicated problems associated with long periods of immobilization, resultant functional disability and economic loss. The methods of treatment are variable, therefore it is important to choose the correct form of treatment. The authors have reviewed 67 fractures of the femoral shaft in 66 patients for the 5 years period from Jan., 1972 to Dec., 1976, who were treated in the department of Orthopedic Surgery, Kyoungpook National University Hospital. The results were as follows; 1. Most of fractures occured in person who are in their third and fourth decades. Male are predominant. 2, Eliciting causes were mainly traumatic and most of them were car accidents. 3. Middle third of the femur was most prevalent location of the fracture site, and comminuted type was most frequent in the fracture type. 4. The period of immobilization for femoral shaft fracture was 7 weeks for those treated with compression plate and Kuntscher nailing; 11. 7 weeks with conventional plate and screw fixation and 17. 8 weeks with skeletal traction. 5. Postoperative infection rate was 3% and infection was treated by conservative methods.

10.Subungual Melanoma of Left Thumb: A Case Report

The following case report concerns a rare subungual melanoma of left thumb which metastasized into the ipsilateral axilla. Subungual melanoma, or melanotic whitlow, is a malignant lesion requiring early diagnosis and urgent treatment. Unlike melanomas located elsewhere on the skin of the body, subungual melanomas are shielded by the nails, which handicaps the securing of a biopsy by the clinician. We report a case of subungual melanoma in a 61 year-old man who had a 6 months history of painful axillary mass.
