A 6-year old girl was diagnosed as atrophoderma vermiculatum on face, especially on cheek and preauricular area, and dorsum of both hands. She had suffered from congenital heart disease which was diagnosed as ventricular septal defect by cardiac: catheterization and cineangiography in final. Three months after patch repair of ventricular septal defect, no further new skin lesion developed. But reticulate skin atrophy was remained.

7.Efficacy and Safety of Bevantolol HCl in Treating Essential Hypertension.

BACKGROUND: Bevantolol HCl was developed as the first antihypertensive agent that has selective beta1 and alpha1 blocking effects with an additional calcium antagonistic activity. It's expected that antihypertensive effect is comparable to other beta-blockers without any significant adverse effect on lipid and glucose metabolism observed in other drugs, and It has less negative inotropic effect due to peripheral vasodilatation mediated through alpha1 and calcium channel blocking effects. To evaluate the antihypertensive effect and safety of bevantolol HCl, we investigated 73 patients with mild to moderately severe essential hypertension. METHODS: Patients who showed either systolic blood pressure 150-209 mmHg or diastolic pressure 95-119 mmHg, were enrolled in this study. Following placebo period of 2weeks, bevantolol HCl was administered in daily dose of 100-200 mg for 12 weeks. RESULTS: Of the 73 patients, 55 patients who were able to receive bevantolol HCl were observed for the safety and 45 patients who completed this study were evaluated for the antihypertensive effect of the drug. 1) Antihypertensive effect: The mean systolic and diastolic blood pressure significantly decreased from 156.7+/-11.7 mmHg to 144.0+/-16.7 mmHg and from 101.6+/-6.4 mmHg to 93.2+/-9.7 mmHg in two weeks of observation in 37/45 patients (82.2%) and was consistently effective for 12 weeks (p<0.01). Blood pressure under 139/89 mmHg was achieved in 20 out of 45 patients (44.4%). The heart rate also declined from 74.9+/-10.5/min to 69.1+/-14.2/min and the effect lasted for 12 weeks (p<0.01). 2) Safety: Mild adverse effects were observed in 27 out of 55 patients. Only one patient developed a significant bradycardia with heart rate of 40/min, which required withdrawal of the drug. No significant changes in the lipid profiles were observed. CONCLUSION: Bevantolol HCl is highly effective and generally well tolerated with an acceptable safety profile in patients with mild to moderately severe essential hypertension.

8.The Risk Factors of Renal Scar in Childhood Acute Pyelonephritis.

PURPOSE: To observe the risk factors of renal scar in children with first acute pyelonephritis (APN) who had photopenic defects on initial 99mTc-DMSA spect renal scan. METHODS: Forty-seven children with first APN(M : F=33 : 14) who were admitted to Ewha Womans University Hospital from September, 1993 to Febrary, 1998, were divided into renal scar group(N=17, Kidney unit(KU)=18) and non-scar group(N=30, KU=39) according to follow-up 99mTc-DMSA spect renal scan. The risk factors were compared between the two groups. RESULTS: There was no significant difference in age, gender, laboratory parameters, causative organism, a pattern of defect on initial 99mTc-DMSA spect renal scan, the presence of vesicoureteral reflux(VUR) and rate of febrile break-through infection between renal scar and non-scar group(P>0.05). The duration of fever before the treatement(3.4+/-2.94 vs. 2.1+/-2.29) and the rate of prolonged fever over 5 days[47.1%(8/17) vs 10.0%(3/30)] were significantly different between the two groups(P<0.05). The severity of VUR was also significantly higher in renal scar group than non-renal scar group(P<0.05, r=0.64). CONCLUSION: Duration of fever before the treatment, prolonged fever and high-grade VUR were risk factors of renal scar in childhood APN. Prompt treatment, especially in children with severe VUR, is important to prevent renal scars.

9.The effect of zonisamide in children with refractory epilepsies.

Zonisamide was administered to 20 patients with refractory epileptic seizures. The mean duration of the administration was 6 months, and the mean dosage was 7.2 mg/kg/day. The efficacy of zonisamide was rated remarkable in 15% of the cases, improvement in 40%, and no change in 45%. The response rates of zonisamide were 62.5% for myoclonic seizures, 50% for tonic-clonic seizures, 80% for atonic seizures and 33.3% for atypical absence seizures. There was no correlation between the clinical response and dose or serum concentration of the drug. The adverse effects were observed in 35% of the cases which were drowsiness, dizziness, ataxia, nausea, and vomiting. In all cases, however, the administration of zonisamide could be continued.

10.Hemorrhagic Chicken Pox.

Primary varicella infection usuelly runs a benign clinical course in the helthy population. However, hemorrhagic chicken pox presents with a very extensive eruption of hemorrhagic vesicles in patients with decreaed platelets or impaired immunity and is accompanied by severe constitutional symptoms. A 7-year-old male was admi due to abdompal pain for 1 day and peneralized vesicular eruption for 5 days. The eruption first appeared on the trunk and then spread to involved face, scalp and extrsmities. The skin rash was compatable with varicella but despite the administration of acclovir intravenously, the vesicular eruption became hemorrhagic. Ten days after admission, having experienced cardisc arrest twice and with his mental state in coms, he was diseharged as his situation was hopelesa. We have experienced a giatient diagnosed as having hemorrhagie, chicken pox complicated by encephalopathy.
