2.The Study of Changes in Arterial Blood Gas Finding with Time Interval.

BACKGROUND: Arterial blood gas analysis are highly susceptible to preanalytic error due to improper method of obtaining or handling the blood sample before analysis. The error in measurement of blood gas analysis are loss of CO2 by exposure to atmospheric air, effect of anticoagulant itself, temperature difference between the experimental subject and the measuring electrode and metabolic change which occur between blood sampling and measurement. METHOD: To study the effect of the delay in estimation of blood gas and drawn blood on values of blood gas partial pressure and pH. Blood sample were divided into 2 groups according to the method of storage, group I stored at 24~25degrees C(room temperature) under anaerobic condition. ;group II stored at 0~4degrees C(refrigerator) under anaerobic condition. The samples were analyzed by time interval through 180 minutes in each group. RESULTS: The result were as follows: 1) PaO2 decreased significantly after 10 mins in group I, whereas it decreased significantly after 20 mins in group II. 2) PaO2 increased significantly after 20 mins in group I, whereas it increased significantly after 120 mins in group II. 3) pH decreased significantly after 60 mins in group I, whereas it decreased significantly after 120 mins in group II. 4) No significant changes of bicarbonate and SaO2 were noted in each group CONCLUSION: From above results, it would be advisable to analyze the sample in a short period of time or to store in a refrigerator when the measuring will be delayed. So we highly recommend that blood gas analysis should be performed as soon as possible after sampling, especially within 10 minutes.

3.A Case of Guttate Morphea with Secondary Cutaneous Mucinosis.

The cutaneous mucinose are a group of connective disorders characterized by the deposition of mucin in the dermis. Thc disease may be a primary (metabolic) or secondary (catabolic). Secondary mucinosis is common in lupus erythematosus and dermatomyositis but rare in morphea. We report a case of guttate morphea with secondary cutaneous mucinosis.

4.Solitary Keratoacanthoma Developing on the Abnormally Sun - exposed Labial Mucosa.

The case of a 56-year-old woman with a keratoacarithoma(KA) of the labial mucosa is herein reported, The tumor was excised and the pathological features were characteristic of a KA, This case is reported because the oral mucosa is such an unexpected location for solitary KA and the labial mucosa of this patient received chronic actinic damage, which suggested an etiological relationship. Follow-up examination of the patient after four months showed no recurrence.

5.Multiple Satellite Pyogenic Granuloma.

Multiple satellite pyogenic granuloma developed on the left upper back of a 14-year-old boy after excision of a hemangioma-like lesion. He has had nevus flammeus on the left upper arm since birth with fine linear telangietasiae around the main lesions. Histopathology showed the capillary proliferation in the dermis, and transmission electron microscopy revealed multiple Weibel-Palade bodies, considered an early endothelial cell marker, in the cytoplasm of the endothelial cells. We believe this patient may have the propensity to develop different angiomatous lesions.

6.X linked ichthyosis.

We report herein a case of multiple glomus tumors on the scrotum in a 14-year-old boy. The lesions were present at birth and had gradually increased in size and number. Pea to thumb tip sized, dark colored nodules without pain sensation were confined to the scrotum. Histopathologically, there were many dilated and irregular vascular spaces lined by a single layer of flattened endothelial cells in the dermis. In the periphery of the endothelial cells, there are a few layers of glomus cells. No capsule was seen. Total excision of the tumors was performed. No recurrence was observed for an year after surgical excision.

7.Pleomorphic Xanthoastrocytoma: A Case Report.

PURPOSE: To draw attention to the radiological findings of a benign variant of cerebral astrocytoma in a young patient. MATERIALS AND METHODS: A 24-year-old man with generalized tonic-clonic seizure of 7 years' duration and normal neurological examination was examined with plain skull series, brain CT and MRI, and cerebral angiography. MR imaging was performed with a 0.5 Tesla Toshiba MRT-50A scanner (TIWI, PDWI, T2WI, 0.1 mmol/kg of Gd- DTPA, SE). RESULTS: 1 ) Plain skull series: A radiolucent lesion with a partial radiopaque rim of about 2.5 x 3 cm size in the right anterior parietal bone.2) B rain CT scan: A cystic mass in the right frontoparietal cortex of midconvexity with pressure erosion on the adjacent skull and partial enhancement at outer and anterior portion.3) Brain MRI:A hypointense mass containing a small, intensely enhancing isointensity anterolaterally on Tl-weighted images, which was hyperintense with better delineation of bulging cortical-based appearance on T2-weithted images. No peritumoral edema.4) Cerebral anglography: An avascular mass. CONCLUSION: The diagnosis of pleomorphic xanthoastrocytoma(PXA) should be entertained in patients in whom a superticially placed enhancing intracerebral tumor containing cystic portion that seems to be in contact with the meninges develops during juvenile years.

8.Clincal Features of Methemoglobinemia in Newborn Diarrhea Infant.

PURPOSE: Newborn infants with diarrhea, metabolic acidosis and dehydration may develop methemoglobinemia without exposure to oxidizing agents. This study was undertaken to investigate clinical features in the development of methemoglobinemia in newborn infants with diarrhea. METHOD: This study involved 16 newborn infants with diarrhea who were admitted to NICU at Dong San Medical Center between January 1995 and June 1996. We investigated the age of onset of methemoglobinemia, sex ratio, level of methemoglobin in the blood, feeding methods, clinical manifestations, arterial blood gas findings, the results of culture findings and the response to therapy. RESULT: 1) The age of onset was beyond the second week of life in most cases, no sex predilection was noted and formula feeding was used in all cases. 2) Methemoglobin level in the blood was 10.1-20.0% in 7 cases, 20.1-30.0% in 6 cases, 30.1-40.0% in 2 cases and above 40.1% in one case. 3) Clinical rnanifestations on admission: moderate to severe dehydration developed in all patients with diarrhea along with respiratory distress in 13 cases, cyanosis in 6 cases, fever was noted in 5 cases, vomiting in 5 cases, while 2 cases presented with abdominal distention. Combined diseases included metabolic acidosis in most cases (93.8%), hypokalemia in 6, failure to thrive in 3, chronic diarrhea in 2, hyponatremia in 2, necrotizing enterocolitis in 1 and hepatitis was found in one case. 4) In all cases, there were no pathogenic organisms found in either blood or urine cultures along with similar negative findings in stool cultures, and Rotazyme test results using ELISA method were negative. 5) Rehydration and correction of acidosis with sodium bicarbonate was accomplished in all patients and 11 cases with greater than 15% methemoglobin were treated with methylene blue, 2mg/kg as a 1% solution in normal saline. Response to methylene blue was indicated in 1 to 2 hours in all cases although there was a reoccurrence of methemoglobinemia after an initial response in 2 cases, they both responded favorably with retreatment with methylene blue. CONCLUSION: In all newborn infant with diarrhea, dehydration and metabolic acidosis, screening tests for early diagnosis of methemoglobinemia should be considered with prompt fluid replacement therapy.

9.A case of angiolymphoid hyperplasia with eosinophilia and its treatment with carbon dioxide laser.

A 45-year-old female noticed a pruritic erythematous firm pea sized nodule on the left earlobe about 18 years prior to uisiting our clinic. The lesion has increased in size gradually and involved the whole earlobe. On March 1988, the earlobe became enlarged with extreme pruritus, and showed a tendency of easy bleeding by scratch. The skin lesions consisted of multiple pea to bean sized, shiny dusky red colored, moderate firm nodules or tumors with edema on the left. ear helix, antehelix, external auditoy meatus, and retroauricular fold. The histopathologic studies revealed vascular proliferation, presence of histiocyt.oid endothelial cells, and prominent inflammatory infiltration including large numtaer of eosinophils with fibrosis in the dermis. Hematological studies revealed eosinophilia(9%). Excision of several nodules and intralesional injection of trinmcinolon acetonide did not show complete favorable effect and treatment with carbon dioxide laser was attempted. With the patient under local anesthesia with 2% lidocaine, the lesion were treated with three t,o four passes at 25,100 second intervals with ieradiance 5W(cm. The pro- cedure was repeated until no abnormal findings could be seen in the treatment zone. Treatment with carbon dioxide Iaser on the patient obtained a satisfactory result.

10.A Case of Dilated Pore of Winer.

We report a case of dilated pare of Winer occuring in a 33-year-oid female patient. Dilated pore belongs to adenoma or organoid group of benign hair differentiation tumors. The patient has had a giant comedone on the right submandibular area for 1 month. Histopathologic features revealed rnarkedly dilated pilar infundibulum lined by epidermis that is atrophic near the ostium but hypertrophic deeper in the cystic cavity. Three months after excisional biopsy, there was no recurrence.
