1.A Case of Recurrent Melanocytic Nevus.

Recurrent melanocytic nevus forms after incomplete removal of a melanocytic nevus and is chatacterized by somewhat proliferation of atypical melanocytes at the dermoepidermal junction and epidermis or recurrent junctional activity. A 73-yeat-old man had a pigmented lesion on the volar aspect of the right heel, which was excised 20 years ago. Five years thereafter, the pigmented lesion recurred in the previous operation scar and had been excised again. But pigmented lesion reappeared in the scar as an irregular patch of variegated pigmentation of browm, gray and black. Histopathologic findings showed fibrosis in the dermis and proliferation of normal and atypical rnelanocytes in the overlying epiderrnis.

2.A Case of Allergic Contact Dermatitis to Nitrofurazone Ointment.

Nitrofurazone is a well known sensitizer and is widely used particularly in surgical department for the treatment of secondary infection, but the reports of all ergic contact dermatitis to nitrofurazone are rare in Korean literature. We experienced a case of allergic eontact dermatitis to 0.2% nitrofurazone in 31-year-old female after dressing the grafted wound on the left index finger with the gauze embedded with 0.2% nitrofurazone olintmnt. The patch test results revealed a strong positve reaction to 0.2% nitrofurazone ointment after 48 hours.

3.A Case of Incontinentia Pigmenti.

Incontinentia pigmenti is an uncommon neurocutaneous genodermatosis characterized by three stages; vesicular lesions, verrucous lesions and hyperpigmentation. A two-week-old female infant showed grouped erythema-based vesiculopustules on the whole body since birth. One month later, vesiculopustular lesions began to disappear gradually. At this time, she developed linear verrucous plaques on the dorsum of the feet and hands and reticulated hyperpigmented patches on the lower extremities. At 2 months of age, vesicular lesions completely disappeared and the pigmented patches spread to the abdomen which became darker with time. The verrucous lesions diminished at 3 months of age. There were no neurological or ocular defects. We describe a case of incontinentia pigmenti with typical clinical and pathological features at each stage.

6.A Statistical Survey on 2205 Skin Tumors.

A statistical study was done on 2205 skin tumors filed at Department of Clinical Pathology, Han Kang and Kang Nam Sacred Heart Hospital during last 10 years from 1980 to 1989. The results were summarized as follows : 1. The number of skin tumor surveyed was 2205 among total 67931 biopsy specimens, 2052 benign skin tumors, 153 malignant tumors. 2. The specimens were requested from the Department of Surgery(707 cases, 32.1 %), Dermatology(618 cases, 28.0%), Plastic Surgery(354 cases, 15.79o), and Or thopedic Surgery(223 cases, 10.1% ). 3. Among 2052 benign skin tumors, the frequently observed tumors were epidermal cyst(577 cases, 28.1%), lipoma(398 cases, 19.4%), melanocytic nevus(121 cases, 5.9 %), dermatofibroma(74 cases, 3.6%), cavernous hemangioma(72 cases, 3.5%), ea.ch of pilmatricoma and neurofibroma(63 cases, 3.1% ), granuloma pyogenicum(57 cases, 2.8%), neurilemmoma(53 cases, 2.6%), seborrheic keratosis(45 cases, 2.2%), soft fibroma(45 cases, 2.2%). 4. Among 153 malignant tumors, the frequently observed tumors were squamaus cell carcinoma(50 cases, 32.7%), basal cell carcinoma(45 cases, 29.4%), malignant melanoma(8 cases, 5.2%), Bowens disease(7 cases, 4.6%), metastatic skin tumors(7 cases, 4.6%) in order. Male to female ratio of malignant tumors was 1.6:1 and the peak age incidence was in 6th decade. 5. Male to female ratio of squamous cell carcinoma was 3.2:1 and of basal cell carcinoma 1: 1.3. The peak age incidences of both squamous cell carcinoma and basal cell carcinoma were 6th decade. The predilection sites of squmous cell carcinoma were face, neck and genitalia and those of basal cell carcinoma were face, lower extremities, neck and trunk.

8.A Study on The Effect of Hyperoxia on EKG Findings of Rabbits.

To investigate the effect of hyperoxia on EKG findings and to evaluate the applicability of EKG as noninvasive monitoring index of oxygen toxicity, 38 rabbits were continuously exposed to 6 different conditions-3 hyperbaric oxygenations (HBO-2.5, 3.5 and 5ATA, 100% O2), normobaric oxygenation (NBO, 100% O2), hyperbaric aeration (HBA-5ATA, 21% O2) and normobaric aeration (NBA, 21% O2)-for 120 minutes and their EKG and time to dyspnea and convulsion were recorded. Dyspnea and death were observed in exposure conditions of HBO-3.5 and HBO-5 (Positive rate of dyspnea; 10%, 100%, death; 10%, 25%, respectively) only, and convulsion in 4 oxygenation groups (NBO; 20%, HBO-2.5; 20%, HBO-3.5; 20%, HBO-5; 88%). Abnormal EKG findings included arrhythmia and ST-T changes and the incidences was increasing with doses(partial pressure of oxygen). In addition to EKG change, findings observed during exosure were dyspnea and convulsion in the order of appearence and when non specific ST-T change was accepted as positive (abnormal) finding, the frequency of abnormal EKG was statistically significant(p<0.01), but when it was excluded from positive results, the frequency of EKG change was not significant(p>0.05). These results suggest that the effect of hyperoxia on heart is myocardial ischemia and arrhythmia, that oxygenation more than 3.5ATA causes myocardial damage in 120 minutes exposure, and that EKG is valuable as monitoring index of oxygen toxicity.

10.Neglected Monteggia Fracture in Children: Report of Three Cases

A neglected Monteggia fracture can pose a significant clinical problem because of decreased range of motion of the elbow, increased cubitus valgus, progressive damage and elbow instability. Factors leading to misdiagnosis include swelling of soft tissue, overlying of bones on radiographs and lack of inclusion of the elbow on the radiograph at the ulnsr fracture. Therefore, a radiograph that includes the elbow at the ulnar fracture should be taken. A line drawn through the shaft and head of the radius into the elbow should align with the capitulum in any projection, thereby assisting in the correct diagnosis. Three cases of neglected Monteggia fracture were treated by open reduction of the radial head with repair or reconstruction of the annular ligament and osteotomy of the ulna, and satisfactory results were obtained.
