1.Treatment of Femoral Neck Fractures

Clinical and roentgenological follow-up study is carried out on 66 cases of femoral neck fractures, treated at the department of orthopaedic surgery, National Medical Center with at least 8 months after treatment. And gained the result as follows; 1. Sixty six cases were comprised of 30 males and 36 females, and 40 out of 66 were over 50 years old. 2. The most common cause of fracture was due to direct or indirect trauma caused by falling or stumbling accidents. 3. Sixty one percent of the cases were rather old and neglected cases after injury. 4. Among 66 patients, 34 were treated by internal fixation, 17 Austin-Moore prosthetic replacements, 1 hip fusion and 14 cast immobilization only. For internal fixation, devices were 26 Smith Petersen nails, 4 compression hip nails, 3 multiple pins and I sliding nail. 5. The incidence of avascular necrosis was 12 percent in 34 cases treated by internal fixation and 36 percent in non-operative treatment. The incidence of non-union was 6 percent in internal fixation, and 36 percent in latter group. 6. Avascular necrosis and non-union after internal fixation were most common in Pauwels type III. 7. Austin-Moore prosthetic replacement has been prefered in elderly patients regardless of the duration between injury and treatment.

3.Effect of Metal Components in Seminal Plasma on Seminal Parameter and Male Fertile Ability.

To determine the concentration and the physiologic role of metal components in blood plasma and seminal plasma in relation to male infertility, the concentrations of twelve metal components in blood plasma and seminal plasma including Na, Mg, K, Ca, Cr, Mn, Fe, Cu, Zn, Se, Cd and Pb were measured by atomic absorbance spectrophotometery or ion selective electrode analysis. Semen and blood samples were obtained from a total of 110 men including 70 male infertility patients, 20 vasectomized persons and 20 fertility proven volunteers visited to the Male Infertility Clinic of Pusan National University Hospital. The concentrations of Ca, Zn, Mg, Cr and Cd in control group were higher in seminal plasma than in blood plasma, and additionally Pb were higher in infertility group. The concentrations of all metal components revealed no significant difference according to patients' age, resident, occupation, sperm density, motility and hormone level in blood plasma, but some metal components including Ca, Mg, Cu, Mn, Cd and Pb revealed a significant difference according to each these parameters except patient's age in seminal plasma. The concentrations of Mn, Cd and Pb in the vasectomy persons were higher than in the infertility group III including testicular and epididymal factors, but not in blood plasma. We conclude that the quantitative changes of metal components in the seminal plasma may have effects on not only spermatogenesis and sperm function, but also contribute to diagnostic parameter according to organ specificity of the metal in the male reproduction.

5.Induction of Dendritic Cell and Cytokine Gene Expression by In situ Delivery of Flt3 Ligand Plasmid.

Dendritic cell (DC)s are protessional antigen presenting cells and they have been used for antitumor immunotherapy or cell vaccines. However therapy using DC is restricted because the number of DC available from tissue is very low. Flt3 ligand (FL) has been known as a hematopoietic growth factor that increases proliferation of hematopoietic stem cells and progenitor cells, and recently it showed inducibility of dendritic cell (DC)s and signiticant antitumor effects in vivo. Thus FL will be frequently used for DC induction and antitumor immunotherapy in future. Here we constructed FL plasmid and studied its in vivo effect. FL plasmids were made by cloning of partial FL cDNA into pcDNA3 plasmid, and gene expression and protein producibility of FL plasmid were confirmed in Renca cells transfected with FL plasmid. Mice were injected with FL plasmid (100ug/mouse) three times and 20 days later mouse spleens were harvested for staining and RT-PCR. There were lots of blastogenic cells in the spleen of mice treated with FL plasmid. FL plasmid also induced DEC205, IL-12 and GM-CSF gene expression in mouse splenocyte. All these data suggest FL plasmid may be used for induction of DC and antitumor therapy as DNA adjuvant.

6.Soft Tissue Change After Single Jaw(mandible) Surgery in Skeletal Class III Malocclusion.

The purpose of this study was to evaluate the amount and interrelationship of the soft and hard tissue change after mandibular setback surgery in skeletal Class III malocclusion. The sample consisted of 25 adult patient (12 male and 13 female) who had severe anteropostrior skeletal discepancy. These patient had received presurgical orthodontic treatment and surgical treatment which is bilateral sagittal split ramus osteotomy. The presurgical and postsurgical lateral cephalograms were evaluated. The computerized statistical analysis was carried out with SPSS/PC program. The result were as follows: 1. After mandibular bilateral sagittal split ramus osteotomy, lower facial soft tissue horizontal posterior changes were high significance value. but vertical soft tissue changes were low significance value. 2. After mandibular bilateral sagittal split ramus osteotomy, relative upper lip protrusion increased(p<0.01) and relative lower lip protrusion decreased(p<0.01) and lower facial soft tissue thickness increased(p<0.01).

7.Changes of the Immunocompetent Cells in the Lymphatic Organs of the Rabbits with Dermatitis Induced by the Hyman Serum.

This study was performed in order to explore the immunologic relationship between the skin lesions induced by the human serum and the lymphatic organs. For these purposes, the authors induced the dermatitis in the rabbits by the injections of pooled human serum with Freunds complete adjuvant into the skin, and administered prednisolone and azathioprine as immunosuppressive drugs to the rabbits. Immunologic asmy was performed by the measurement of the plasma ceIls and immunogobulin-bearing B lymphocytes in lymph node and spleen. The results were as follows: 1. By the injections of human serum pyroninophilic cells and immunoglobulining B lymphocytes in lymph node and spleen were increased. 2. By the administrations of prednisolone and azathioprine, pyroninophilic cells and immunoglobulin-bearing B lymphocytes in lymph node and spleen were all decre RSCd.

9.A Case of Reticulate Acropigmentation of Kitamura with Hyperpigmented Macules on the Flexural Areas.

Reticulate acropigmentation of Kitamura (RAPK) is an autosomal dominant dermatosis comprising of reticulate slightly clepessed pigmentation of the extensor surfaces of the hands and feet with palmar pits. It has been suggested that this may be the same disease as Dawling Degos disease (DDD), an autosomal dorninant condition which is characterized by a reticulate pigmentation of flexures, comedo-like lesicns and pitted scars. We present a case of RAPK in a 49-year-old female who had reticulate, brownish, slightly depressed pigmentation on the extremities with t,he involvement of flexures, the predilection sites of DDD.

10.Radiologic assessment of Endoscopically reconstructed ACL using Bone - patellar tendon - bone.

Endoscopic ACL reconstruction using bone-patellar tendon-bone has been considered the gold standard in the field of reconstructive ACL surgery. Technically, graft must be placed at isometric point. But it is difficult to evaluate the placement of graft postoperatively. The purpose of this study is to determine the radiological ideal position of graft by comparing postoperative results with the graft placement. Seventy cases of endoscopic ACL reconstruction were reviewed and classified according to the femoral and tibial graft position on radiologic imaging. The femoral graft position was classified in relation to angle of graft on anterior position view and distance from posterior margin of graft to the inner surface of posterior cortex on lateral view. The tibial graft position was classified in relation to intercondylar eminence on anterior posterior view and lateral view. Knee score (modified Marshall, Lysholum), manual anterior instability test (Lachman test, Pivot shift test) and arthrometer measurement were checked to evaluate postoperative results in each case. The results of this study implicate that knee joint in which femoral graft was oriented at direction of 11 o clock centring around 68 degree respect to tibial joint and placed within 3mm from posterior cortex showed higher knee score and lesser laxity. In cases of tibial side, the graft oriented to intercondylar eminence (AP view) and placed anterior to intercondylar eminence (lateral view) showed higher knee score and lesser laxity.
