ObjectiveTo investigate the therapeutic effect of acupoint pressing plus gentle moxibustion on lower limb swelling after total knee arthroplasty.MethodOne hundred and twenty patients diagnosed with knee osteoarthritis who required unilateral total knee arthroplasty were allocated, in order of operations, to observation and control groups, 60 cases each. After the operation, all the patients received routine nursing care including elastic bandage pressure dressing, operated-on limb elevation, early knee immobilization, local ice compress, intermittent pneumatic compression, active ankle pump and quadriceps contracting-relaxing exercise, CPM mechanical exercise, use of elastic stockings and oral administration of rivaroxaban. The observation group received postoperative acupoint pressing plus gentle moxibustion additionally. Limb circumferences 10 cm superior and inferior to thepatella were measured and operated-on limb pains were scored (using the VAS) in the two groups of patients before, on the day of and at one, three, five, seven and fourteen days after the operation. Knee joint range of motion (ROM) was measured before and at three, seven and fourteen days after the operation.ResultThe incidence of swelling was 47.46% in the observation group and 67.24% in the control group. The duration of swelling was (7.31±3.19) days in the observation group and (10.92±3.12)days in the control group. There were statistically significant differences in the VAS score and operated-on knee joint range of motion between the two groups of patients at the same time points after the operation (P<0.05).ConclusionAcupoint pressing plus gentle moxibustion can effectively intervene in lower limb swelling and promote the recovery of knee joint function after total knee arthroplasty.

Objective To evaluate the effects of laparoscopic radical resection of mid-low rectal cancer on sexual and urinary functions of male patients. Methods The clinical data of 150 patients with mid-low rectal cancer who were admitted to the PLA General Hospital from May 2006 to March 2009 were retrospectively analyzed. Sixty-eight patients were allocated to laparoscopic group and 82 to open group. The incidences of erectile and ejaculatory dysfunction and the short- and long-term urinary dysfunction of patients in the 2 groups were compared seperately at 6 and 12 months after the operation. All data were analyzed using the chi-square test. Results The incidences of erectile and ejaculation dysfunction at 6 months after operation, incidence of ejaculation dysfunction at 12 months after operation, and long-term urinary dysfunction were 15% (10/68), 16% (11/68), 10% (7/68) and 0 in the laparoscopic group, and 22% (18/82), 23% (19/82), 21% (17/82) and 2% (2/82) in the open group, respectively, no significant difference between the two groups was found (x2 = 1. 285, 1. 137, 3. 013, 1.681, P>0. 05). The incidences of erectile dysfunction at 12 months after operation and short-term urinary dysfunction were 7% (5/68) and 4% (3/68) in the laparoscopic group, and 17% (14/82) and 20% (16/82) in the open group, respectively, a significant difference between the two groups was observed (x = 4. 565, 5.930, P <0.05). Conclusion Laparoscopic radical resection of mid-low rectal cancer can reduce the injury of pelvic autonomic nerve and improve the life quality of patients.

Objective:To explore the application value of ultrasonic layer-specific strain technology in evaluating left ventricular systolic function in young male strength athletes. Methods:In October 2018, 30 professional young male wrestlers from Henan Provincial Heavy Sports Management Center (athlete group) and 30 healthy young males matched with age (control group) were randomly selected.Using traditional echocardiography combined with stratified strain technique, heart rate (HR), left ventricular end-diastolic diameter(LVDd), interventricular septal thickness on diastole(IVSTd), postwall thickness on diastole (PWTd), relative wall thickness (RWT), end diastolic volume index(EDVI), end systolic volume index (ESVI), ejection fraction (LVEF), stroke volume index (SVI), cardiac index (CI), spherical index (SPI), endocardial myocardial global longitudinal strain(GLSendo), mid-myocardial global longitudinal strain(GLSmid), epicardial myocardial global longitudinal strain(GLSepi), peak strain dispersion(PSD) and whole myocardial longitudinal strain cross-wall difference (ΔLS) were measured respectively. The differences between the two sets of data were analyzed.ROC curves were plotted to analyze and compare the stratified strain parameters performance to predict left ventricular systolic function or synchrony in athletes. Results:The values of LVDd, PWTd, IVSTd, RWT, SPI, EDVI, ESVI and SVI in athelete group were higher than those of the control group. And the values of HR, GLSendo, GLSmid, GLSepi and ΔLS were lower than those of the control group. The differences were statistically significant (all P<0.05). There was no significant difference in LVEF, CI and PSD between the two groups (all P>0.05). The areas under the ROC curve of GLSendo, GLSmid and GLSepi for athletes′ left ventricular systolic function were 0.753, 0.747 and 0.726, respectively, and the optimal cut-off values were -22.34%, -19.95%, -17.35%, respectively. Conclusions:Long-term high-intensity exercise can lead to subclinical changes in left ventricular systolic function, and ultrasonic layer-specific strain technology can more accurately and specifically evaluate left ventricular systolic dysfunction. Among the GLS parameters, GLSendo is the optimal parameter for testing the left ventricular systolic function of athletes.

【Objective】 To investigate the effects of formononetin (FMN) on cardiomyocyte apoptosis and HSP90/AKT in rats with dilated cardiomyopathy-mediated heart failure. 【Methods】 Echocardiography, ELISA, histological staining, and TUNEL staining were used to observe the protective effect of different doses of FMN on dilated cardiomyopathy-mediated heart failure in rats and the apoptosis of cardiomyocytes. The potential targets of formononetin on dilated cardiomyopathy-mediated heart failure were obtained from TCMSP, DisGeNet, GeneCards, and other databases, the key targets were obtained according to the protein-protein interaction (PPI) network, and the key targets were verified by molecular docking. Western blotting was used to further verify the regulatory role of key targets in the treatment of dilated cardiomyopathy-mediated heart failure with formononetin. 【Results】 Formononetin could reduce the levels of LVIDS, LVIDD, NT-pro BNP, cTn-T, CK, CK-MB, and LDH in rats with dilated cardiomyopathy-mediated heart failure, increase the levels of EF and FS, and reduce the apoptosis of cardiomyocytes. FMN had a strong binding effect on 10 key targets (AKT1, HSP90AA1, CASP3, MAPK1, MMP9, SRC, ALB, HRAS, IGF1, and EGFR) screened by network pharmacology, with HSP90AA1 and AKT1 having the strongest binding effect. Formononetin decreased the expression of HSP90, AKT and downstream CASP3 protein, but increased the expression of p-AKT in myocardial tissue. 【Conclusion】 Formononetin may inhibit the expression of HSP90, promote phosphorylation of AKT to p-AKT, and inhibit the expression of CASP3, thereby reducing the apoptosis of cardiomyocytes and improving myocardial tissue damage, so as to achieve the purpose of treating dilated cardiomyopathy-mediated heart failure.
