1.The activation effects of electroacupuncture combined with transcranial magnetic stimulation on endogenous neural stem cells in rats with cerebral ischemia

Objective To study the effect of electroacupuncture (EA) combined with repetitive transcranial magnetic stimulation (rTMS) on endogenous neural atem cells, as well as learning and memory in rats with cerebral ischemia. Methods One hundred and twenty male Wistar rats were randomly divided into 5 groups: a normal group, a model group, an EA group, a rTMS group and an EA plus rTMS group. After establishing acute middle cer-ebral artery occlusion, the rats in the various experimental groups were treated with EA, rTMS or EA plus rTMS. The rats' ability to avoid an electric shock was observed using a jumping stand, Immunohistochemical techniques were used to measure the infarct area and the number of Brdu-positive cells. Results A small quantity of Brdu labelled cells was observed in the subgranular zone (SGZ) and the subventricular zone (SVZ) in rats in the normal group. In the EA, rTMS and EA plus rTMS groups, the numbers of Brdu labelled cells in the SVZ and SGZ was significantly higher than in the model group after both 7 and 14 days. The expression of Brdu around the SVZ and SGZ of the 3 treatment groups increased, especially in the EA plus rTMS group. At 7, 14 and 28 d there was significant improve-ment in the neural function and electric shock avoidance test scores in the EA, rTMS and EA plus rTMS groups com-pared with the model group,with the EA plus rTMS animals scoring best. Conclusion EA plus rTMS can promote the proliferation of nerve stem cells, facilitate neurological recovery and improve learning and memory in rats after cerebral ischemia.
2.Does bariatric surgery decrease the risk of obesity-related tumor: a meta-analysis.

OBJECTIVETo explore whether bariatric surgery can decrease the incidence of obesity-related tumors in obesity patients.
METHODSRelevant studies comparing the incidence of obesity-related tumors in obesity patients between bariatric surgery and non- bariatric surgery were identified by search of PubMed, Medline, EBSCO, High Wire Press, OVID, EMbase, China hownet (CNKI) and Wanfang databases since the self-built database. In strict accordance with the standard after the screening, literature quality and extracted data were evaluated. Review manager 5.2 software was used to perform meta-analysis and sensitivity analysis. Inverted funnel chart was used to investigate the publication bias.
RESULTSFive articles including 108 954 patients were enrolled in the analysis. Among them, 26 218 cases were bariatric surgery group, and 82 736 cases of non-surgical weight loss were the control group. Meta analysis showed that bariatric surgery could obviously decrease the incidence of postoperative obesity-related tumor(RR=0.60, 95% CI:0.45-0.80, P=0.0005). Subgroup analysis showed that cancer risk difference of obesity-related tumor in male patients was not significant between two group, while the postoperative incidence of obesity-related tumor of female patients in bariatric surgery group was significantly lower compared to those female patients in control group(RR=0.68, 95% CI:0.61-0.77, P<0.01). During follow-up of 1 to 10 years, the incidence of obesity-related tumor in bariatric surgery group was significantly lower than that in control group(P<0.05). When follow-up was more than 10 years, the incidence of obesity-related tumors was similar between two groups(P=0.70).
CONCLUSIONBariatric surgery can decrease the overall risk of obesity-related cancer, especially for female patients, but with the prolongation of time, such effect of bariatric surgery is not obvious.
3.Impact of spasmolytic on polyp and adenoma detection during colonoscopy: a meta-analysis.

OBJECTIVETo investigate the impact of intestinal spasmolytic on colon polyps and adenoma detection rate during colonoscopy.
METHODSLiteratures related to the effect of intestinal spasmolytic on colon polyp or adenoma detection rate were retrieved from PubMed, Medline, EBSCO, High Wire Press, OVID, EMBASE, and the China National Knowledge Articles, etc. published before July 2014. Unified data were extracted by two researchers independently and organized using Jadad scale to evaluate the quality of the enrolled studies through Review manager 5.2 Meta-analysis software.
RESULTSSix articles were enrolled with total 47,509 cases, including 16,867 cases in the scopolamine group and 30,642 cases in the placebo group. Meta analysis showed spasmolytic could increase the detective rate of polyps (OR=1.24, 95% CI:1.19-1.30), adenoma (OR=1.25, 95% CI:1.19-1.30) and high-risk adenoma (OR=1.22, 95% CI:1.16-1.29).
CONCLUSIONUsing colonoscopy spasmolytic scopolamine can increase the detection rate of colonic polyp and adenoma.

4.Correlation between gallbladder polyp and colon lesions in different positions

Objective:To investigate the correlation between gallbladder polyps and colon polyps of different locations.Methods:Healthy subjects who underwent colonoscopy in the Physical Examination Center of the Affiliated Hospital of Yangzhou University from January 2020 to October 2022 were included. They were divided into the proximal colon group and the distal colon group according to the location of colon polyps. Gallbladder polyps were compared with other known risk factors for colon cancer. Different parts and types of polyps were further explored.Results:A total of 2 811 patients had colon polyps, including 1 668 males and 1 143 females, with an average age of 47.3 years. One hundred and seventy-one (6.1%) had gallstones, and 459 (16.3%) had gallbladder polyps. The incidence of gallbladder polyps was higher in the proximal colon polyp group (21.7%, 93/429). Gallbladder polyps were independently associated with proximal colon polyps, including hyperplastic polyps ( OR=1.525, P=0.029) and adenomatous polyps ( OR=1.425, P=0.017). There was no significant correlation between gallbladder polyps and distal colon polyps. Conclusion:Gallbladder polyps are associated with proximal colon polyps, and colonoscopy screening is recommended for people with gallbladder polyps.
5. Quality indicators of diagnostic gastroscopy

Gastroscopy is a widely used examination for the diagnosis and treatment of upper gastrointestinal diseases in most countries and regions. High quality gastroscopy is much important for the diagnosis, treatment and prevention of gastrointestinal diseases. Gastroscopy is an operation highly dependent on the skills of gastroscopy physician, and the difference in the quality of operation may have an important impact on the quality of gastroscopy. It is an urgent problem for gastroscopy clinicians to improve the quality of gastroscopy. The improvement of gastroscopy quality not only can reduce the morbidity and mortality of upper gastrointestinal diseases, but also can improve the overall medical quality. This article reviewed the progress of research on the quality indicators of diagnostic gastroscopy.
6.Preliminary study on the clinical application of four cytokines in serum of autoimmune diseases

Objectives:the purpose of this study was to systematically evaluate the clinical value of cytokines in autoimmune diseases (AID). It was a kind of complex disease, and its pathogenesis involved cytokines, autoantibodies, immune cells and other immune factors. Especially some AID, such as Adult still′s disease (AOSD) and Takayasu arteritis(TA), had no specific biomarkers at present. This study was a retrospective case-control study.Methods:the data of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-8(IL-8) and interleukin-10(IL-10) in 834 AID patients from January 2019 to August 2022 were collected, and the serum levels of those cytokines in 30 healthy controls (HC) were detected at the same time. And AOSD, TA, systemic lupus erythematosus (SLE) and Behcet′s syndrome (BS) were divided into active group and inactive group. In addition, we also made a subgroup analysis of two important organs involved in SLE (kidney and nervous system). GraphPad Prism 9 and R 4.2.2 software were used. Nonparametric tests (Kruskal-Wallis H test, Mann-Whitney U test) were used to compare the differences among groups, and Dunn′s method was used to correct the false positive caused by multiple tests. Results:To compare the level of IL-6 in each group, except Behcet syndrome (BS) group and antiphospholipid syndrome (APS) group, the serum IL-6 level of AID group was higher than that of HC group, with antineutrophil cytoplasmic antibodies associated vasculitis(AAV) [3.85(2.00,8.55) pg/ml], idiopathic inflammatory myopathies(IIM) [7.80(2.50,6.50)pg/ml], IgG4-related disease(IgG4RD) [3.65(2.08,12.83) pg/ml], rheumatoid arthritis (RA) [5.50(2.20,16.10) pg/ml], SLE[4.70(2.75,16.55) pg/ml], Sj?gren syndrome(SS) [3.20(2.00,8.90) pg/ml], systemic sclerosis(SSc) [2.70(2.00,8.90) pg/ml], TA[3.40 (2.00,6.50) pg/ml], other AID diseases[4.40(2.00,11.10) pg/ml], especially AOSD [15.20(2.10, 39.20) pg/ml]. After correction, the differences were statistically significant ( P c<0.05). At the same time, the levels of serum TNF-α [7.40(5.60,10.95) pg/ml]and IL-10 [5.00(5.00, 7.58) pg/ml] in AOSD group were significantly higher than those in HC group[7.15(5.93,8.00) pg/ml,5.00(5.00,5.00) pg/ml] after correction ( P c<0.05). At the same time, the levels of serum TNF-α and IL-10 in AOSD group were higher than those in HC group. The serum levels of IL-6 and IL-8 in patients with active AOSD, BS, SLE and TA were significantly higher than those in patients without active disease (all P<0.05). In addition, the level of serum IL-8 in lupus nephritis group was significantly higher than that in non-lupus nephritis group ( P<0.05). At the same time, the serum levels of IL-6, IL-8 and TNF-α in neuropsychiatric lupus erythematosus group were significantly higher than those in non-neuropsychiatric lupus erythematosus group ( P<0.05), but there was no significant difference in IL-10 between neuropsychiatric lupus group and non-neuropsychiatric lupus erythematosus group. Conclusions:there was a close relationship between AID and cytokines. At present, the change of serum IL-6 level was the most classic one in clinical routine.
