1.Genotyping of human papillomavirus in male patients with anogenital warts in Lishui area, Zhejiang province

Objective To investigate the distribution of human papillomavirus (HPV) genotypes in male patients with anogenital warts in Lishui area,Zhejiang province.Methods Tissue specimens were obtained from the lesions of 150 male patients with anogenital warts.PCR-reverse dot blot hybridization was performed to detect the presence of 3 low-risk HPV types (HPV 6,11,and 43) and 16 moderate-or high-risk HPV types (HPV 16,18,31,33,35,39,45,51,52,53,56,58,59,66,68 and CP8304) in these specimens.Chi-square test was used for statistical analysis.Results HPV was detected in 91 (60.67%) of the 150 male patients.Of the 91 positive patients,74 (81.32%) were infected by single or multiple low-risk HPV types,whereas 17 (18.68%) by single or multiple high-risk HPV types.Thirty-one (34.07%) patients harbored 2-5 HPV genotypes,including 20 (64.52%)patients infected with both low-risk and high-risk HPV types,and 6 (19.35%) patients infected with two low-risk HPV types.The coexistence of moderate-or high-risk HPV types with HPV 6 was observed in 13 (41.94%)patients,and that with HPV 11 in 6 (19.35%) patients.The most prevalent genotype was HPV 6 (28.68%,39/136),followed by HPV 11 (26.47%,36/136),16 (8.09%,11/136),52 (5.15%,7/136),53 (5.15%,7/136),51 (4.41%,6/136),58 (4.41%,6/136) and 43 (4.41%,6/136).Conclusions Low-risk HPV genotypes predominate in male patients with anogenital warts,and there are large differences in the distribution of multiple infections and HPV genotypes.
2.MRI Appearances of Toxoplasmosis Encephalopathy

Objective To study the MRI features of toxoplasmosis encephalopathy.Methods There were totally 20 cases in the group,all patients accepted the examinations of MRI and immunisation sylum(EIISA) with Gd-DTPA enhanced MRI in 17 cases and repeted MRI in 15 cases.5 cases undengone stero-orientation biopsy under CT guided.Results Most of the focus situated at the juncture of gray and white matter of cerebral hemisphere (8 cases) and around the lateral ventricle (6 cases).The lesion was multiple .The lesions had different degrees of contrast enhancement after enhanced scans (17 cases),among them,the patchy enhancement was most commonly seen (9 cases),secondarily was circular or semicircular enhancemeut (7 cases).The immunological serum examination in all the cases were positive IgM or IgG.The treatment for anti-toxoplasma was in effect for all the cases.Conclusion MRI is of significant value in definiting the involved location,extent,time course of toxoplasmosis encephalopathy,and also evaluating the effect of therapy.
3.Effect analysis of hybrid operation for the treatment of intracranial complex ruptured aneurysms

Objective To investigate the safety and short-term effectiveness of the hybrid operation for the treatment of intracranial complex ruptured aneurysms.Methods From December 2014 to March 2017,14 consecutive patients with complex ruptured aneurysm treated with hybrid operation at the Department of Neurosurgery,Nanfang Hospital,Southern Medical University were enrolled retrospectively,including 13 with acute spontaneous aneurismal subarachnoid hemorrhage and 1 with hemorrhage in the recurrent aneurysm embolization.Twelve aneurysms were treated with shape clipping.Digital subtraction angiography (DSA) was used to evaluate the clipping effect of aneurysms.Two patients with aneurysm were treated with extracranial-intracranial (EC-IC) bypass and aneurysm trapping.Endovascular balloon occlusion for trapping aneurysms was performed after DSA evaluation of the patency of bridge vessel.Results Of the 14 patients,11 were treated with emergency hybrid operation after angiography,2 were treated with elective surgery,and 1 with emergency surgery for rescue because of bleeding during embolization.DSA revealed that the aneurysm clips in 3 of 12 patients needed to be adjusted,including 2 parent artery stenosis and 1 with incomplete clipping.After adjustment,the clipping was satisfactory.In intracranial and extracranial bypass surgery,angiography revealed that the blood vessels were patent.Trapping of the aneurysms was performed in the one-stage operation.One patient discharged voluntarily after procedure because of serious vasospasm.Onepatient had perfusion pressure breakthrough after surgery and received hematoma evacuation and decompression.The Glasgow outcome scale (GOS) score was 3 at discharge.Other patients had no new neurological dysfunction after operation.Thirteen patients were followed up for 3-24 months after operation.There were no new neurological dysfunction,including GOS 5 in 8 cases and 4 in 5 cases.Six patients underwent DSA examination,in 4 of them the aneurysm clipping did not show aneurysm recurrence,and the parent arteries were patent.Two patients treated with vascular bypass.There were no recurrence of aneurysms,and the parent arteries and anastomotic vessels were patent.Conclusion After preliminary observation,using hybrid operation for the treatment of complicated intracranial ruptured aneurysms was safe and effective.
4.Non-functioning pituitary carcinoma: report of two cases and review of literature.

Pituitary carcinoma is extremely rare. We report two cases of non-functioning pituitary carcinoma treated in our hospital. The two female patients, aged 39 and 40 years, were diagnosed by CT and MRI and postoperative pathological examination. The tumor was surgically resected followed by stereotactic radiotherapy. Despite of the treatments, both of the patients showed tumor recurrence and died at 1 and 2 months after discharge. The diagnosis of this disease relies on a combined evaluation of the clinical, radiological, cytological, immunohistochemical features. Due to the highly malignant nature and poor prognosis of this disease, early surgical intervention and systemic evaluation of the patients' condition are imperative. Currently more effective therapeutic approaches are needed to control the tumor progression.

5.Relationship of treatment outcome with thyroid hormone content in patients with biphasic disorder and depressive disorder

Objective:To explore the correlation between thyroid hormone content and treatment outcome in patients with biphasic disorder (BPD) and depressive disorder (DPD) before and after treatment.Methods:From June 2017 to June 2019, 72 patients with BPD(BPD group), 46 patients with DPD(DPD group) who met the diagnostic criteria of International Statistics and Classification of Disease and Health problems-10th Edition (ICD-10) in the Third People′s Hospital of Jiangmen, and 20 normal controls(control group) in our Hospital were enrolled in this study.The serum concentrations of total thyroxine (TT 4), total triiodothyronine (TT 3) and high sensitivity thyrotropin (HS-TSH) were measured at baseline, and the above hormone concentrations were measured in the study group after treatment for 12 weeks.The 17 Hamilton depression scale (HAMD-17) and Hamilton anxiety scale (HAMA), Bech-Rafaesdn Mania Rating Scale(BRMS) were used to evaluate the severity of symptoms at baseline and at the end of treatment.The curative effect was compared between the two groups, and the correlation between the curative effect (score reduction rate) and thyroid hormone level (concentration change) was observed. Results:At baseline, the concentrations of TT 4, TT 3 of patients with depressive phase in BPD group[(54.75±12.26)nmol/L, (86.26±15.29)nmol/L] were lower than those in the control group[(145.64±12.15)nmol/L, (156.78±36.54)nmol/L], and the TSH level in BPD group[(68.97±5.98)mIU/L]was higher than that in the control group[(45.59±9.28)mIU/L]; the levels of TT 4, TT 3 of patients with manic and mixed phase in BPD group[(166.75±12.25)nmol/L, (186.36±35.15)nmol/L] were higher than those in the control group, and the level of TSH in BPD group[(7.87±3.56)mIU/L] was lower than that in the control group; the concentrations of TT 4, TT 3 in DPD group[(65.59±23.11)nmol/L, (92.21±19.36)nmol/L] were lower than those in the control group, and the TSH level in DPD group[(69.38±14.75)mIU/L] was higher than that in the control group, the differences were ststistically significant ( t=5.867, 4.657, 2.369, 2.789, 3.247, 6.356, 4.358, 3.265, 2.365, all P<0.05). After treatment for 12 weeks, there were no statistically significant differences in TT 3, TT 4, HS-TSH concentrations among the three groups (all P>0.05). After treatment for 12 weeks, the scores of HAMa+ d, BRMS in BPD group and DPD group were significantly lower than those at baseline ( t=8.854, 12.321, 6.756, all P<0.05). The score of HAMa+ d was negatively correlated with TT 4, TT 3 concentrations, and positively correlated with HS-TSH concentration.The score of BRMS was positively correlated with TT 4, TT 3 concentrations, and negatively correlated with HS-TSH concentration. Conclusion:The level of thyroid hormone in patients with biphasic disorder and depressive disorder is different from that in normal subjects, and the level of thyroid hormone is closely related to the curative effect.
6.Effects of Gentiopicroside on the Apoptosis of Human Pancreatic Cancer Cells PANC- 1 and IL- 6/JAK2/STAT3 Signaling Pathway

OBJECTIVE:To study the effects of gentiopicroside on the apo ptosis o f human pancreatic cancer cells PANC- 1,and to explore its mechanism from the perspective of IL- 6/JAK2/STAT3 signaling pathway. METHODS :Using PANC- 1 cells as model , the proliferation inhibition rate of cells was tested by MTT assay after treated with 0(negative contro ),2,4,8,16,32,64,128 mg/L gentiopicroside for 72 h and IC 50 were calculated. The cells were divided into negative control group ,gemcitabine group (positive control,4 mg/L)and gentiopicroside low-concentration ,medium-concentration and high-concentration groups (15,30,60 mg/L). After cultured for 1,3,5,7 d,Trypan blue exclusion staining was used to count the survival cell ,and the growth of cells was investigated. After cultured for 72 h,colony formation assay was used to observe colony formation rate of cells ;the apoptotic rate of cells was detected by Hoechst 33258 staining;the mRNA and protein expressions of IL- 6,JAK2,STAT3 in cells were detected by RT-PCR and Western blotting assay. RESULTS :4-28 mg/L gentiopicroside could inhibit the proliferation of cells (P<0.05 or P< 0.01),in concentration dependent trend ;IC50 was 9.54 mg/L. Compared with negative control group ,survival cell count (cultured from 3,5,7 d),mRNA and protein expressions of IL- 6,JAK2 and STAT 3 in cells were decreased significantly in gemcitabine group , gentiopicroside medium-concentration and high-concentration groups (P<0.05 or P<0.01),while the apoptotic rate was increased significantly (P<0.01). The colony formation rate of cellswere decreased significantly in gemcitabine group and gentiopicroside high-concentration group (P<0.01). mail:hb.gz@163.com Compared with gemcitabine group ,there was no statistical significance in above indexes of gentiopicroside high- 6716008。 concentration group (P>0.05). CONC LUSIONS:30,60 mg/L gentiopicroside could inhibit the proliferation and induce apoptosis of PANC- 1 cells,and 60 mg/L gentiopicroside is similar to gemcitabine in the effects. Its mechanism may be related to inhibiting the activation of IL- 6/JAK2/STAT3 signaling pathway.
7.Relationship between expressions of P21,P27 and PCNA in glomerular mesangial tissue and poor renal prognosis in patients with IgA nephropathy

Objective:To explore the relationship between the expressions of P21,P27 and proliferating cell nuclear antigen(PCNA)in glomerular mesangial tissue and poor renal prognosis in patients with immunoglobulin A(IgA)nephropathy.Methods:A total of 145 patients with IgA nephropathy treated in Xiaogan Central Hospital from April 2017 to August 2019 were selected as the research object.The expressions of P21,P27 and PCNA in glomerular mesangial tissue were detected by immunohistochemistry.All patients were followed up for 24 months,and the prognosis were counted.The expressions of P21,P27 and PCNA in glomerular mesangial tissue of patients with different prognosis were compared and the influencing factors of poor prognosis in patients with IgA nephropathy were analyzed by Logistic regression analysis.Results:The expression rates of P21,P27 and PCNA positive cells in glomerular mesangial tissue of patients with IgA nephropathy were(38.69±6.83)%,(55.94±8.08)%,(33.47±5.72)%,respectively.The incidence rete of poor prognosis in patients with IgA nephropathy was 17.24%,and the expression rates of P21 and PCNA positive cells in glomerular mesangial tissue of patients with poor prognosis were higher than those in good prognosis group(P<0.05),while the expression rate of P27 positive cells was lower than that in good prognosis group(P<0.05).Logistic multiple regression analysis showed that elevated diastolic blood pressure,increased 24 h proteinuria,mesangial cell proliferation,segmental glomerulosclerosis,renal tubular atrophy/interstitial fibrosis,crescentic body,increased expression rates of P21 and PCNA positive cells and decreased expression rate of P27 positive cells were all risk factors affecting the poor prognosis of patients with IgA nephropathy(P<0.05).Conclusion:There are positive expressions of P21,P27 and PCNA in glomerular mesangial tissue of IgA nephropathy.The expression rates of P21 and PCNA positive cells in glomerular mesangial tissue of of patients with poor prognosis of IgA nephropathy are higher than those with good prognosis,while the expression rate of P27 protein positive cells is lower than those with good prognosis,which are risk factors for poor prognosis of patients with IgA nephropathy.

9.Perioperative PBM in liver transplant patients: establishment and implementation

【Objective】 To establish a practical and feasible blood management (PBM) model for clinical liver transplant patients, so as to optimize the perioperative blood usage and improve patient prognosis and survival rate. 【Methods】 Led by the Medical Department, a questionnaire survey Establishment and Implementation of a Blood Management Model for Organ Transplant Patients on the importance of establishing a PBM model for liver transplant patients in clinical practice was conducted among 71 staff from relevant departments such as the Anesthesiology Department, Hepatobiliary Department, Blood Transfusion Department, and ICU Medical Department. After statistical analysis, the important items of PBM for liver transplant patients were determined, and a (trial) plan was organized by the Medical Department and training was conducted by Blood Transfusion Department for medical staff training from departments as Anesthesiology, Hepatobiliary and Pancreatology, and ICU; PBM measures for liver transplant patients before, during and after surgery were determined by the Anesthesiology Department, Hepatobiliary and Pancreatic Department, and ICU; PBM was fully implemented in the experimental group with liver transplant patients in our hospital in 2021 as subjects; 30 cases of liver transplantation performed in our hospital in 2020 were selected as the control group, and the general data, preoperative laboratory examination results, and preoperative, intraoperative, and postoperative blood transfusion of the two groups of patients were compared. 【Results】 Compared with the control group, the experimental group did not receive any blood component transfusion before surgery. Intraoperative blood transfusion volume of allogeneic components: red blood cells(U) were 12.75(8.75, 18.63) vs 15.25(8.38, 26.13)(P>0.05), apheresis platelet (treatment volume) was 0(0, 2) vs 1(0, 4.25)(P>0.05), plasma(U) was 2 300(1 550, 3 763) vs 3 650(2 075, 5 400)(P<0.05); cryoprecipitate(U) was 0(0, 10) vs 10(0, 30)(P<0.05); Postoperative blood transfusion volume of allogeneic components were as follows: red blood cells(U) 8.00(3.38, 12.88) vs 11.25(4.75, 19.63)(P>0.05), platelets were not transfused, plasma(U) was 0(0, 0) vs 0(0, 600)(P>0.05), and cryoprecipitates were not transfused. 【Conclusion】 The establishment of PBM model for liver transplant patients and the application of its management measures before, during and after transplantation significantly reduced the intraoperative blood transfusion volume and improved the prognosis of patients after transplantation.
