1.Clinical and histopathologic feature and treatment of early colorectal carcinoma

Objective The gross appearance and histopathology as well a s the treatment of early colorectal carcinoma were investigated. Methods The clinical documents and histopathology of 105 cases(106 lesions) with early color ectal carcinoma detected by colonoscopy and proved pathologically after operatio n in past 16 years were retrospectively reviewed and analysed. Results 91.5% of the early colorectal carcinoma were located distal to the sigmoid.The macro scopic type of these lesions under colonoscopy was classified as Ip type in 34 l esions,Ips type in 13,Is type in 26,Ⅱ a type in 11,Ⅱ a+Ⅱ c type in 22(in whi ch 21 lesions are Sm carcinoma).Lymph node involvement was observed in 3 cases o f the 25 with Sm carcinoma (accounting for 12% ).13 lesions(in 12 cases)were re moved under the colonoscopy,local recurrence was found in one case of Sm carcino ma during the follow up period and treated with radical operation.93 lesions(in 93 cases)were treated by local resection or radical surgery. Conclusions Most of the carcinomas withⅡ a+Ⅱ c type were sm carcinoma.Histopathologically mos t of Sm carcinomas were poorly or moderately differentiated adenocarcinomas.Lymp h node involvement only appeared in Sm carcinoma.Radical operation should be per formed in the following conditions (1)the macroscopic type isⅡ a+Ⅱ c type,(2) the histopathologic type is poorly differentiated adenocarcinoma,(3)involvement of submucosa or lymph node metastasis
2.A kinematic study of the tibiofemoral joint after total knee arthroplasty using in vivo fluoroscopy and digital model

Objective To investigate the relative motion between the femoral component and the tibial insert through the technology of fluoroscopy and digital model.Methods Sixteen female patients (16 knees) with knee osteoarthritis were performed TKA (GENESIS Ⅱ) from July 2007 to June 2008.The mean age was 66.4 years (range from 56 to 76 years).All patients were followed up 48 months to 60 months,with the mean of 56±3 months.The postoperative clinical function was evaluated by Knee Society Score (KSS).To match the digital model to the imaging data of fluoroscopy using 2D-3D automatic registration technology and reconstruct motion of the knee after TKA.The movement of contact position between femoral medial and lateral condyle and tibial insert and tibial internal rotation during flexion were measured.The contact time and range between femoral cam and tibial post were analyzed.Results The cases were statistically improved in KSS postoperatively,knee score was 93±5 and function score 88±13.The range of movement for femoral medial condyle was 8.5±2.5 mm,and the lateral condyle was 9.5±4.8 mm,the range of tibial internal rotation was 2.5°±8.4°.The contact of cam and post was observed after 30°-40° flexion of the knee,and the range was 8.0±1.8 mm.The greater tilting angle of the tibial plateau was,the later contact between cam and post happened.Conclusion The kinematics of tibiofemoral joint after TKA is different from the kinematics of normal knee.With knee flexion within 10°-30°,femoral medial condyle moves forward.When flexion is greater than 40°,medial and lateral condyle begins rollback.The time of contact of cam and post is relative to tibial prosthesis slope.
3.Effect of subcutaneous immunotherapy on serum levels of human beta defensin-2 in children with allergic rhinitis

OBJECTIVE To investigate the effect of subcutaneousimmunotherapy(SCIT) on levels of the serum human beta defensin-2 in children with allergic rhinitis. METHODS 30 cases of children with allergic rhinitis who were treated by SIT were selected as the treatment group, 20 cases of healthy children as the control group. Serum HBD-2 concentration of the control group was tested. Serum HBD-2 concentration of the treatment group was tested at three different time points: before SCIT, half a year after SCIT and one year after SCIT. And total nasal symptom scores(TNSS) and medication scores were recorded at each time point. RESULTS The serum HBD-2 concentration of the control group, that of the treatment group before SIT, half a year after SIT and one year after SIT were 4.62[4.08; 4.87], 3.74[3.37; 4.61], 4.62[4.13; 5.54], 4.79[4.45;6.19]ng/ml. The HBD-2 concentration gradually increased after SCIT. The TNSS of the treatment group before SCIT, half a year after SCIT and one year after SCIT were 7.43±2.15, 4.17±2.16, 4.20±1.92, The medication scores of the treatment group before SCIT, half a year after SCIT and one year after SCIT were 1.25[0.75; 1.38], 0.25[0; 0.75, 0.25[0; 0.75].There was no correlation (all P>0.05) between the serum HBD-2 concentration and TNSS or medication scores of the treatment group. CONCLUSION The serum levels of HBD-2 in patients with allergic rhinitis were lower than those in normal persons. The specific immunotherapy raised the serum HBD-2 levels of allergic rhinitis patients.
4.Epidemiology of nosocomial infection in recipients after renal transplantation: a single-center analysis

Objective To investigate the epidemiological characteristics of renal transplantation recipients, effective prevention and control measures. Methods A total of 456 renal transplant recipients were monitored from January 2014 to December 2017. Postoperative infection including baseline data, infection site and infectious pathogen type was analyzed. Results Among 456 renal transplant recipients, 78 cases (17.1%) developed nosocomial infection. Postoperative infection time was 9(3-21) d. The clinical pathway of Traditional Chinese Medicine (TCM), a medical management model learned abroad, is a kind of medical management model implemented in order to improve efficacy, reduce the economic burden on patients, and increase the utilization rate of medical resources. This article reviews the literature on the clinical path of TCM in recent years, outlines its development characteristics, research models, evaluation methods, management methods, and then analyzes its advantages and disadvantages in clinical implementation, and thus provides reference for future implementation.

OBJECTIVETo investigate the safety and efficacy of trastuzumab administered concurrently with anthracycline-containing adjuvant regimen for breast cancer.
METHODSIt is a prospective, randomized and controlled trial. Participants were randomized to receive trastuzumab administered concurrently or sequentially with anthracycline-containing adjuvant regimen. The primary endpoint was cardiac safety. The second endpoints were disease-free survival (DFS) and overall survival (OS).
RESULTSOne hundred and nine breast cancer patients were enrolled and randomized in this trial. Fifty-five participants received trastuzumab administered concurrently with anthracycline-containing adjuvant regimen and 54 patients received trastuzumab administered sequentially with anthracycline. The primary cardiac event was asymptomatic decrease in the left ventricular ejection fraction (LVEF). There was no significant difference between concurrent and sequential groups in cardiac event rates (9.1% vs13.0%, P = 0.556), neither of LVEF values at basline or at 3, 6, 9 and 12 months during trastuzumab treatment (P > 0.05). Four patients (7.3%) in the concurrent group suffered local recurrences or distant metastases, and 6 participants (11.1%) in the sequential group had distant metastases. There was no significant difference between the two groups in DFS (P = 0.724). There was no death in both groups.
CONCLUSIONSTrastuzumab administered concurrently with anthracycline is a safe adjuvant regimen for breast cancer and does not increase cardiac events. Further research is needed to determine the efficacy of this treatment regimen.

Objective To explore the safety and cosmetic effect of nipple-sparing modified radical mas tectomy and immediate tissue expander implantation with single circumaereolar incision.Methods 30 patients were enrolled in Peking Union Medical College Hospital between Jan.2014 and Dec.2015.All the patients were categorized according to surgical incision (single circumaereolar incision group vs double incisions group).Data on clinicopathological parameters,average hospital stay,complications and overall cosmetic effect were retrospectively collected.Data was performed with Chi-square test,Fisher exact test and t-test.Statistical significance was defined as P<0.05.Results 19 patients were enrolled in single circumaereolar incision group,and 11 patients in double incisions group.There was no significant difference for operation duration (P=0.093) and average hospital stay (P=0.339).After follow-up for 19.1 months,ranging from 8 to 31 months,no patients developed seroma or arm lymphedema.There was no statistical significance between the two groups in terms of sensation in nippleaereolar area (P=0.973),bilateral symmetry (P=0.650) and overall cosmesis (P=0.483).Conclusion single circumaereolar incision nipple-sparing modified radical mastectomy and immediate tissue expander implantation can be one of the preferable surgical procedures with benefits of minimal invasiveness,reliable oncological safety and decent cosmetic effect.
8.Epidemiological study on nosocomial infection after liver transplantation from organ donation after death

Objective To investigate the epidemiological characteristics of nosocomial infection after liver transplantation from organ donation after death of citizens,and to provide evidence for the development of effective prevention and control strategies.Methods Targeted monitoring was conducted on liver transplantation patients in a hospital from January 1,2014 to December 31,2017,and then descriptive statistical analyses were carried out on postoperative infection after liver transplantation.Results The nosocomial infection occurred in 118 cases (55.40%) among 213 cases of liver transplantation.The infection sites were lower respiratory tract,blood system,and intraabdominal infection,accounting for 40.30%,22.39%,and 19.40%,respectively.186 strains of pathogens were isolated,mainly including gram-positive cocci (including 113 strains,60.75%),followed by gram-negative bacilli (56 strains,30.11%) and fungi (17 strains,9.14%).There was significant difference in the infection of the respiratory tract,post-operative wounds and other part s (P<(0.05).The most common pathogens were Enterococcus faecium,Staphylococcus,Klebsiella Pneumoniae,and Stenotrophomonas maltophilia,accounting for 15.05%,15.05%,10.22% and 9.69%,respectively.There was significant difference in Gram-negative Bacillus,fungi strains,Acinetobacter strains,Enterococcus faecium strains,Pseudomonas aeruginosa strains,Stenotrophomonas maltophilia and staphylococcus (P < 0.05).Conclusion The patients after liver transplantation from organ donation after death have a high incidence of nosocomial infection.The infection was mainly distributed in the lower respiratory tract and blood system,and predominantly caused by bacteria and fungi.Infection surveillance of liver transplant patients should be strengthened and comprehensively effective prevention and control measures for nosocomial infection should be developed.

Objective:
To explore the association between the 21-gene recurrence score (RS) and clinicopathologic characteristics as well as prognosis in patients with axillary lymph node negative, hormone receptor (HR) positive breast cancer.
Methods:
The clinicopathologic data of 439 early breast cancer patients who underwent 21 gene RS testing was retrospectively analyzed. According to the 21 gene RS, the patients were divided into low risk (295 cases), intermediate risk (111 cases) and high-risk (33 cases) group. The relationship between the 21 gene RS and clinicopathological characteristics, treatment, recurrence and metastasis was analyzed. Univariate and multivariate statistical analyses were used to analyze the risk factors for relapse free survival (RFS).

OBJECTIVETo discuss radiological characteristics and clinical manifestation of isolated lumbar foraminal stenosis.
METHODSFrom March 2011 to March 2014, 21 patients with isolated degenerative lumbar foraminal stenosis accepted lumbar decompression and fusion in Beijing Luhe Hospital. Intervertebral disc space was evaluated by measuring the position of joint-body line on preoperative X-ray. Bilateral foraminal area of the corresponding segment in CT (sagittal view of 2D reconstruction) and MRI (T2W1 sagittal view) were measured by Surgimap software. For patients with unilateral symptoms, foraminal area of the affected side was compared with that of the contralateral side. Foraminal area of the same segment on CT was also compared with that on MRI. Preoperatively and at the final follow-up, visual analogue score (VAS) and Oswestry Disability Index (ODI) were used to evaluate clinical outcomes.
RESULTSAll patients had a follow-up over 6 months and the average follow-up was 16.8 months (7-42 months). Of the 21 patients (26 segments), 12 segments showed gross narrowing and 14 segments showed slight narrowing. After preoperative measurement on MRI, 6 patients had foraminal stenosis of grade 2, and 15 patients had foraminal stenosis of grade 3, showing no significant difference in clinical outcomes. Compared with the foraminal area of the unaffected side, the affected side showed a decrease of 16% on CT and 28% on MRI, and the difference was statistically significant (t = 3.453, P < 0.05). The foraminal area measured on CT was larger than that measured on MRI (P < 0.05). Compared with that preoperatively, VAS (back pain), VAS (leg pain) and ODI showed significant improvement at the final follow-up (P < 0.05).
CONCLUSIONSRadiological examinations as X-ray, CT, MRI and intervertebral foramen block technique play an important role in the diagnosis of foraminal stenosis. Soft oppression caused by hyperplasia and hypertrophy of transforaminal ligment or joint capsule may be important promoters of degenerative lumbar foraminal stenosis. Lumbar foraminal decompression and interbody fusion can satisfactorily improve preoperative symptoms.
