1.Observation of the effects of 5-aminolevulinic acid photodynamic therapy combined with fulguration in treating perianal condyloma

Objective To evaluate the relapse rate and the effect of 5-aminolevulinic acid photodynamic therapy(ALA-PDT) combined with fulguration in the treatment of perianal genital warts.Methods 67 patients with perianal genital warts were randomly divided into the three groups.22 cases in the fulguration and ALA-PDT treatment group performed the ALA-PDT treatment immediately after removal of dominant wart by fulguration,once a week,treatment for 3-4 times.22 cases in the ALA-PDT treatment group only underwent ALA-PDT treatment,once a week,treatment for one month.23 cases in the fulguration group were given the traditional fulguration methods one by one electrocautery fulguration gasification treatment for all skin lesions,a weekly batch treatment,treatment for a month.Follow-up six months after the last treatment to determine efficacy and recurrence rate.Results The cure rate of combined treatment group was 90.9% (20/22),and recurrence rate was 9.1% (2/22) ;cure rate of photodynamic group was 54.5% (12/22),and its recurrence rate was 22.7% (5/22) ; cure rate of fulguration group was 39.1% (9/23),and the recurrence rate was 43.5% (10/23).There were statistically significant differences in the cure rate and relapse rate between combination therapy group and electrocautery treatment group (P =0.000,P =0.017).Conclusion The fulguration combined with ALA-PDT in the treatment of perianal genital warts has high cure rate,low recurrence rate,and less side effects.
2.Biomechanical study on the impact injure of human mandible by finite element method

Objective:To investigate biomechanical changes of mandible in the impact injure simulated by finite element method (FEM).Methods:Mimics and Comsol software were used to build a FEM of human craniofacial bone based on CT scan data of a normal adult.LS-DYNA and Hypermesh software were used to simulate the impact with different quality,velocity and angulation pro-duced injures of human mandible,the biomechanical parameters of the mandible in the impact injury process were analysed.Results:A FEMof human maxillofacial bone was established,and the dynamic process of different impact force produced damage was simula-ted.Mandibular chin,angle and condylar neck was the stress concentrated area in the process of mandible injury.There was higher stress peak at the site which was closer to the impact position,the stress peak arrival time was also earlier.When the impactor with the same quality,the bigger the velocity,the greater the stress peak.When the impactor with the same velocity,the bigger the quali-ty,the greater the stress peak.When the impactor with the same velocity and quality,there was greater stress peak under the impact to mandible from angulation of 0 degree.Stress transfered to the surrounding bone from the impact position radially and gradually re-duced.The bone area with small cross-section was prone to high stress and more serious damage.Conclusion:The quality,the ve-locity,the impact angle and the impact site are the factors affecting the severity of impact injury.
3.Experimental study on establishment of a 3D finite element model of human craniomaxillofacial region and its use in impact injury simulation

Objective To develop a 3D finite element model for the human craniomaxillofacial region in an attempt to offer basis to the research of simulation of craniomaxillofacial injury.Methods A healthy adult male was submitted to head CT scan, and the data was imported into the Mimics 15.0 software for threshold segmentation and 3D reconstruction according to the classifications of bone tissue, skin tissue and subcutaneous tissue.The reconstruction data was imported into 3D reverse software Geomagic Studio 2012, and the images were optimized and the 3D model was generated.The three parts of the model were fitted according to the actual proportion using the 3D control software Solidworks 14.0, and then the boundary conditions were derived.Hypermesh 12.0 finite element processing software was used to build the volume mesh, and the model was established.Each layer of the model was given to the material parameters, and the simulation conditions were provided to test the model.Results This model was completely composed of volume meshes, including 214,250 hexahedral meshes and 411,920 nodes.This model can clearly show the stress distribution, the trend of fracture line, the displacement of fracture block of soft and hard tissue during the simulation, and the results are consistent with clinical practice.Conclusion A three-dimentional finite element model with good performance is established, which can be used for biomechamics simulation analysis of multiple sites on the head or the whole structure, and has a certain significance in clinical and scientific research.
4.Effects of Pingan qushi prescription on traditional Chinese medicine syndrome,adverse events,and compliance in young and middle-aged patients with hypertension

Objective To investigate the effects of the Pingan qushi prescription on traditional Chinese medicine syndrome,adverse events,and compliance in young and middle-aged hypertensive patients.Methods One hundred thirty-two young and middle-aged patients with hypertension were selected and randomly divided into two groups.After elimination,64 cases were included in each group.For 2 months,the control group was treated with irbesartan tablets,and the observation group underwent the Pingan qushi prescription based on the control group.The blood pressure,curative effect,traditional Chinese medicine syndrome score,safety,adverse events,and prognosis-related indicators were compared between the two groups.Results The total effective rate of the observation group(96.88%,92.19%)was higher than that of the control group(82.52%,78.13%),and the difference was statistically significant(P = 0.005,P = 0.025).After 1 and 2 months of treatment,the blood pressure,syndrome scores,ALD,PRA,AngⅡ,and ET-1 in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).There was no sig-nificant difference in the incidence of adverse reactions between the two groups(P>0.05).There was no statistically significant difference(P>0.05)in the incidence of adverse events during treatment between the observation group(1.56%)and control group(10.94%).Conclusion The treatment of young and middle-aged community patients with hypertension undergoing the Pingan qushi pre-scription can further improve patients'symptoms,increase blood pressure control,promote prognosis improvement.
5.Risk factors and nursing intervention for pneumothorax in performing CT-guided radioactive 125I seed implantation in patients with lung cancer

Objective To explore the risk factors and nursing intervention for pneumothorax in performing CT-guided radioactive 125I seed implantation in patients with lung cancer.Methods 40 patients with lung cancer who performed CT-guided radioactive 125I seedimplantation from January 2014 to November 2015 were included in the study. The retrospective analysis was used and the participants were hospitalized either in Peking university third hospital or in Hebei Cangzhou hospital of integrated traditional Chinese and western medicine. Results After the implantation, seven patients had implantation including five mild cases and two moderate cases. Six patients got better after the implantation (i.e. five of them had no obvious clinical symptoms and one of them had chest distress and cough). One case had hemothorax combined with chest distress and cough, but got better after air extraction and closed thoracic drainage.Conclusions The risk factors for pneumothorax in performing CT-guided radioactive 125I seed implantation may include the type and location of the lesion and the number of puncture times. The important factors for preventing the incidence of pneumothorax may include preoperative analysis of the type and location of lesions, reducing thepuncture times, less harm for pleural layers,and reasonable needlepoint. The major nursing strategies to prevent pneumothorax include preoperative evaluation and health education, cooperate with the doctors during the operation,and close observation of patients'postoperative symptoms.
6.Efficacy of Fuyang Tongluo therapy combined with conventional western medicine in elderly patients with coronary heart disease and heart failure

Objective To explore the therapeutic efficacy of the Fuyang Tongluo therapy combined with conventional western medicine in elderly patients with heart failure due to coronary heart disease.Methods In this single-blind randomized controlled trial,96 elderly patients with coronary heart disease and heart failure were randomly divided into three groups,each consisting of 32 individuals.Control group A was treated with conventional western medicine(atorvastatin combined with metoprolol),control group B with Fuyang Tongluo therapy,and the observation group with Fuyang Tongluo therapy and conventional western medicine.The three groups were compared in terms of therapeutic efficacy and traditional Chinese medicine symptom points as well as blood interleukin(IL)-23 and IL-17 levels,left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),and left ventricular ejection fraction(LVEF)values before and after treatment.Results The total clinical efficacy rate in the observation group was 93.75%,which was significantly higher than those in control groups A(68.75%)and B(65.63%)(P<0.05).After 4 and 8 weeks of treatment,the primary and minor symptom scores,total symptom scores,LVESD,LVEDD,and IL-23 and IL-17 levels in the observation group were significantly lower than those in control groups A and B(P<0.05).Conclusion The combination of the Fuyang Tongluo therapy with conventional western medicine exhibited good therapeutic efficacy in elderly patients with coronary heart disease and heart failure.These effects were achieved by inhibiting excessive expression of the inflammatory axis factors IL-23 and IL-17 and promoting the recovery of cardiac function.
7.Effect of shensongyangxin capsule on myocardial remodeling and ventricular fibrillation threshold value in rat with coronary artery ligation.

OBJECTIVETo study the effect of shensongyangxin capsule on myocardial remodeling and ventricular fibrillation characteristics in rat with coronary artery ligation.
METHODTwenty-three male rats were randomly divided into sham-group (n = 5), model group (n = 6), anmiodarone group (n = 6) and shensongyangxin capsule group (n = 6). Drugs were administrated after modeling of 2 days, lasting four weeks. Two dimensional and Doppler images were acquired by a 15 MHz high-frequency linear ultrasound transducer at 4 weeks after operation, and chest was opened to detect ventricular fibrillation threshold value and persistent time.
RESULTAfter administration of four weeks, echocardiogram was detected. Compared with model group, shensongyangxin capsule group diastasis interventricular septum thickness (IVSTd) and left ventricle diameter (LVDd) were significiently different between them (1.20 +/- 0.49) vs (0.78 +/- 0.08) mm and (6.77 +/- 1.34) vs (7.95 +/- 0.92) mm, (P < 0.01 and 0.05); echocardiogram result had no difference in amiodarone and model groups (P > 0.05). LVMI measured by practicion was different between shensongyangxin capsule and model groups: (17.12 +/- 1.91) vs (18.95 +/- 1.41) g x m(-2), (P < 0.05), while amiodarone group had no difference compared with model group. Electrophysiology was used to detect ventricular fibrillation threshold value and 1-5, 6-10, 11-15 V three stages' ventricular fibrillation threshold persistent time were significiently different among each group (P < 0.01), 16-20 V stage's ventricular fibrillation persistent time were also different among each group (P <0.05). Sample "average ranks" showed ventricular fibrillation threshold value of amiodarone group and shensongyangxin capsule group were four times than model group; and amiodaron group had best effect of holding-back ventricular fibrillation persistent time.
CONCLUSIONThe coronary artery ligation can result in myocardial remodeling by increasing volume load, and at the same time influencing electrophysiology function of heart. Amiodaron elevated ventricular fibrillation threshold of heart, this effect maybe relate to influencing many ion channels of myocardial cellular membrane; shensongyangxin capsule also elevate ventricular fibrillation threshold of heart, this effect maybe also relate to influencing many ion channels of myocardial cellular membrane, and on the other hand this effect maybe relate to hold-back ventricular remodeling after coronary artery was ligated, accordingly improve electrophysiological base material of heart.

8.A prospective study of clinical characteristics and prognosis in patients with persistent inflammation, immunosuppression and catabolism syndrome after polytrauma

Objective:To prospectively assess clinical characteristics, potential causes and prognosis in patients with persistent inflammation, immunosuppression and catabolism syndrome (PICS) after polytrauma.Methods:Totally 1 083 patients with polytrauma admitted to Department of Traumatic Surgery of Tongji Hospital from Janury 2019 to July 2020 were enrolled. Exclusion criteria included age<18 years old, length of hospital stay<15 days, previous medical history of malignancy, or immunological, consumptive, and metabolic diseases. According to the diagnostic criteria of PICS, all enrolled patients were divided into two groups: PICS group and N-PICS group (without PICS). The patient’s clinical characteristics, ISS score, GCS score, SOFA score, and prognosis were collected. The χ2 test or Student’s t test was uesd to compare the difference between the PICS group and N-PICS group. Results:The incidence of PICS in patients with polytrauma was 11.7% (127/1 083). The majority of PICS patients were middle-aged and elderly men, 68.5% with traumatic brain injury and 59% with thoracic injury. GCS score was significantly lower, while ISS, APACHE II and SOFA scores were significantly higher in the PICS group than in the N-PICS group ( P<0.01, P<0.05). Among PICS patients, 79.5% were treated with mechanical ventilation and 76.3% were associated with pulmonary infection, with a 28-day mortality of 5.5% and a 180-day mortality of 16.5%, which were siginifcantly different from those without PICS. Conclusions:PICS has a high incidence after polytrauma and is commonly seen in middle-aged and elderly male patients with severe polytrauma, especially accompanied by traumatic brain injury or/and thoracic injury. Patients with PICS after polytrauma have poor long-term prognosis, so early identification and intervention should be strengthened in clinical practice.
9.Clinical observation of ticagrelor combined with Compound xueshuantong capsule in the treatment of unstable angina pectoris

OBJECTIVE To observe the efficacy and safety of ticagrelor combined with Compound xueshuantong capsules in the treatment of unstable angina pectoris. METHODS Totally 120 patients with unstable angina pectoris with deficiency of Qi and Yin combined admitted to Sanmenxia Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were randomly divided into clopidogrel group (group A), ticagrelor group (group B) and combined medication group (group C), with 40 patients in each group. In addition to conventional treatment, group A was given clopidogrel orally; group B was given ticagrelor orally; group C was given ticagrelor and Compound xueshuantong capsule orally. After 12 weeks of treatment, the clinical efficacy, frequency of angina attacks, coagulation function indicators, cardiac function indicators, traditional Chinese medicine syndrome efficacy, and the incidence of adverse cardiovascular events and bleeding incidence were evaluated in the 3 groups. RESULTS The total effective rates of group A, group B and group C were 77.5%, 85.0% and 90.0%, respectively. The incidence of adverse cardiovascular events and bleeding events were 7.5%, 7.5% and 5.0% in the respective groups, with no statistically significant difference (P>0.05). The frequency of angina attacks, duration of angina attacks and duration of dyspnea were significantly reduced or shortened in all 3 groups after treatment compared to before treatment (P<0.05). The frequency of angina attacks in group C was significantly lower than that in groups A and B after treatment (P<0.05). The levels of fibrinogen (FBG) and D-dimer in all 3 groups were significantly lower after treatment compared to before treatment (P<0.05); group A had significantly higher levels of FBG and D-dimer compared to group B and C (P<0.05). The left ventricular end-diastolic diameter and left ventricular end-systolic diameter in all three groups were significantly shorter after treatment, and the left ventricular ejection fraction was significantly higher compared to before treatment (P<0.05), but there were no statistically significant differences among those groups (P>0.05). The total effective rates of traditional Chinese medicine syndrome efficacy in groups A, B and C were 67.5%, 80.0% and 87.5%, respectively, with group C being significantly higher than group A (P<0.05). CONCLUSIONS In addition to conventional treatment, ticagrelor combined with Compound xueguantong capsules can more significantly reduce the frequency of angina attacks in patients with unstable angina pectoris, reduce the levels of FBG and D- dimer, improve traditional Chinese medicine syndrome efficacy, and do not increase the risk of bleeding.
10. A study on urinary iodine levels and influencing factors of urban and rural residents in some areas of Gansu Province

Objective:
To analyze the urinary iodine status of urban and rural residents in some areas of Gansu Province and its related influencing factors, and to provide certain references for scientific iodine intake of urban and rural residents in Gansu Province.
Methods:
Subjects in Gansu Province with normal thyroid function were recruited in the investigation of thyroid diseases and iodine nutritional status, conducted a questionnaire survey and measured height, weight, waist circumference, collected 1 urine sample, and tested their urinary iodine levels. The urinary iodine levels of different groups were compared based on different gender, region, age, body weight, the waist, and salt intaking. The affecting factors of urinary iodine levels were analyzed by multiple linear regression models.
Results:
Totally 1 964 subjects were recruited, including 1 099 males, and 865 females. The age was (41.23 ± 14.75) years old. The median urinary iodine of the selected group was 225.60 (158.80, 311.58) μg/L. The urinary iodine level of rural residents was significantly higher than that of urban residents [μg/L: 249.80 (180.58, 336.88)
