Objective To investigate the association between the genetic polymorphisms in GSTA1 and the clinical outcome of breast cancer patients treated with cyclophosphamide-based adjuvant chemotherapy. Methods A total of 137 breast cancer patients receiving cyclophosphamide-based adjuvant chemotherapy were recruited ( 124 cases with infiltrative ductal carcinoma, 5 cases with infiltrative lobular carcinoma and 8 cases with other histological types). PCR-LDR method was used to detect the genotypes of GSTA1. Survival curves were generated by the Kaplan-Meier method, and verified by the log-rank test. Cox proportional hazards regression analysis was used to estimate the prognostic factors in multivariate analysis. Results Of the 137 breast cancer patients, the genotypic frequencies of the GSTA1 * A/* A,* A/* B and * B/* B were 67.2% ( 92/137 ), 31.4% ( 43/137 ) and 1.5% ( 2/137 ), respectively. No significant differences were found between the genotypic frequencies and groups categorization according to age, stage, lymph node metastasis, ER or PR status (x2 = 0. 722,1. 967, 3. 303, 0. 226 and 0. 709, all P >0. 05 ) ;through Fisher exact test, also no significant differences were found between the genotypic frequencies and group categorization according to tumor size, histological types and grading ( all P > 0. 05 ) . The recurrence rates in patients with GSTA1 * A/* A and * A/* B or * B/* B genotypes were 47. 8% (44/92) and 31.1% ( 14/45 ), respectively, and the mortality rates were 22. 8% ( 21/92 ) and 17. 8% ( 8/45 ),respectively. Patients with GSTA1 * A/* B and * B/* B genotypes were significantly associated with reduced hazard of relapse (x2 =18.723, P<0. 01)and mortality (x2 =7.352, P<0.01), compared to cases with the common * A/* A genotypes, according to Kaplan-Meier survival analysis and log-rank test. Moreover,Cox multivariate analysis showed that GSTA1 polymorphisms appeared to be an independent risk factor for recurrence-free survival ( OR =0. 222, 95% CI:0. 108-0. 458, P <0. 01 ) and overall survival ( OR =0. 362,95% CI:0. 145-0. 902, P < 0. 05 ). Conclusion These data indicate that GSTA1 polymorphism may be a potential prognostic factor for recurrence-free survival and overall survival in breast cancer patients treated with cyclophosphamide-based adjuvant chemotherapy.

Objective To evaluate the function and synchronization of left atrial in patients with chronic heart failure(CHF) by two dimensional-speckle tracking imaging (2D-STI) and real-time threedimensional echocardiography(RT-3DE).Methods Thirty-two healthy controls and twenty-nine patients with CHF were studied. The mean strain and strain rate during systole, early and late of diastole(mSs,mSe and mSa,mSRs,mSRe and mSRa) were measured at each left atrial wall by 2D-STI.From the volume-time curve of RT-3DE workstation, the maximum volume (LAVmax),the minimum volume(LAVmin),the volume before contraction(LAVp) of left atrial were acquired.Accordingly, the total, passive and active ejection fraction could be calculated. The synchronization parameters derived from 2D-STI and RT-3DE were the dispersion of time to peak strain and time to the minimum volume (T-SD and Tmsv-16-SD).The index of volume and time were corrected by the body surface area and interval of R-R,respectively.The correlation were analysed betwee them. Results () Comparing between two groups, the mSs,mSe and mSa were shortened and the mSRs,mSRe and mSRa were slower in CHF group (P < 0.05).② All the index of volume were gradually increasing from control to CHF group, but the LATEF,LAPEF and LAAEF were just the reverse, with all parameters had significant difference between two groups (P <0.05).③ The synchronization parameters were significantly higher from control to CHF group by the two kinds of skills (P <0.05).Comparing with 2D-STI,the indexes of synchronization were bigger and the asynchronization rate of CHF group were higher by RT-3DE.④The indexes of synchronization had a negative correlation with the strain rate and the ejection fraction, but had a positive correlation with the index of volume. Conclusions The function of left atrial are reduced in patients of CHF,there are asynchronization.2D-STI and RT 3DE may paly important roles in the evaluation of function and synchronization of left atrial. The relevance ratio of the left atrial asynchronization are higher from the RT-3DE than 2D-STI.

Objective To explore the relationship between immune functions of patients with acute leukemia (AL) and invasive fungat infection (IFI). Methods T lymphocyte subpopulations and natural killer (NK) cells in 61 AL patients complicated with IFI at first visit, AL remission, the time of IFI onset and 4 weeks after antifungal treatment were measured by flow cytometry. Meanwhile, levels of IgG, IgM and IgA were detected by immunoturbidimetry. The statistical analysis was done using ANOVA, t test and chi-square test. Results CD3+ , CD3+ CD4+ , CD8+ CD28+ T lymphocyte as well as CD4+/CD8+ ratio at the time of IFI onset in AL patients were all lower than those at first visit, AL remission and 4 weeks after antifungal treatment (F= 25.6,26.6,13. 1,167.9; all P<0.05), while CD8+ CD28- T lymphocyte were higher than those at first visit, AL remission and 4 weeks after antifungal treatment (F= 220.2,P<0.01). CD3+ , CD3+ CD4+ and CD4 + /CD8+ ratio of patients who responded effectively to antifungal treatment were all higher than those of non-responders (t=3.75,8. 61,3.17; all P<0.05). The serum levels of IgG, Igm and IgA at first visit, ALremission, the time of IFI onset and 4 weeks after treatment were similar (F=0.78,0.72,0.81; all P >0.05). The effect rate of antifungal therapy in AL remission group was higher than that in nonremission group (87% vs 53%,x2 = 7.62, P<0.05). Conclusions The cellular immune functions are impaired severely in AL patients complicated with IFI, while the levels of IgG, IgM and IgA are similar during IFI. Therefore, the efficacy of antifungal therapy may partly depend on the recovery of cellular immune functions and remission of AL.
